    Clomid monotherapy really only works as a treatment for secondary hypogonadism which is a problem with the HPTA and effects LH and FSH levels just like you explained in this video. In the case where LH and FSH are both on the high end of normal or are out of range high and Testosterone is low then the patient is most likely Primary hypogonadal and clomid monotherapy is not going to work to treat the low T.

    The drawback of clomid is that it raises SHBG. I found adding proviron 25-50mg a day a very good combination

      SHBG is also the cellular transport method for Test. Going up isn’t bad in and of itself, so long as free T is also ideal

    Doctor! A year ago I had injected Testosterone Enanthate 250 every 2 weeks for 10 weeks. I did no pct and once my testicles shrunk more than 80 percent.
    Now I checked my blood with Urologist for the results of my total Testosterone of 680. But since the last injection I have severe depression and low energy and all the other symptoms.
    What should I do to feel better like I was before the T injections

    I’m curious would this be to kickstart the system again or would this clomid be ongoing like TRT?

      There is no evidence nor clinical experience to support that Clomid can kickstart your LH and then you can get off of it and maintain those levels unfortunately.

    For Secondary Hypogonadism which is the best way..HCG or Clomid? Heard clomid can kill your libido more and have tons of side affects

      There are a lot of factors that are considered when deciding the optimal treatment for the patient. If you are strictly just comparing these 2 medications for secondary hypogonadism we would treat with Clomid.

    Hey Doctor! You have brought some light on my street, but here i would like to put whole profile of mine. I have never been through TRT or any such stuff but i don't why my testosterone has dropped to 300ngdl, now i am wondering what treatment is good to fix hypogonadism however in this meantime I got married and i have children too but facing some challenges with Mild ED and PE and even hairloss due to low T levels. It is my humble request as to what tests should I get done and if given with clomophene citrate how should I take to mitigate its side effects and i heard someone saying on UA-cam that initially we should take HCG cycle then start clomophene or temoxiphene in order to wakeup your HTPA access. And afterwards giving gap of certain time period start taking some natural testosterone boosting supplements such shilajit etc. Please help me to choose correct path.

    Can clomid cause leydig cells desensitization like HCG does ? Will High LH produced by Clomid, say above 10 IU/L for several months can down regulate your testes ?

      Clomid is prescribed off label to raise testosterone in men. With that said, the long-term research is lacking to answer this specific question.
      Clomid is communicating with the pituitary gland to stimulate the increase in LH and FSH. From there, the testes produce more testosterone. The signal is coming from within one’s one body, so the testes will not produce more than they are capable of.
      Down regulation typically comes from using medications that are replacing that specific hormone from an outside source (ie: exogenous testosterone or melatonin).

    Doctor I am 26 and I was wondering if I have secondary or primary hypogonadism. My lab tests are : Total Test = 350, SHBG = 19, LH = 6.15, Fsh = 2.7 . Also I am suspecting either Kallmann or Klinefelter. Can you give a little diagnosis based on the bloodwork?

      These diagnoses would require a provider’s examination for other factors. Nothing jumps out from your bloodwork that points toward these genetic disorders. However, that is not to say you would not benefit from treatment to relieve any symptoms you may be experiencing.

    Curious is Clomid a long term option for some of your patients? From my reading it seems Clomid is a short term use to determine if one is a Phase one or Phase two hypogonadism. Thanks for any feed back.

      Diagnosed with secondary hypogonadism in 2015. Been on Clomid 25 mg 5x a week. I can say that the first 8 weeks it felt great, like I was back to my old self. Total Testosterone levels went from about 185 to 831. However, I've noticed that using it long term it seems to lose its effectiveness. I don't know if this is an SHBG elevation issue or due to the long half-life of the ZuClomiphene trans-isomer. A couple other channels theorized that having the estrogen receptors bound 24/7 for long periods of time leads to the negative side-effects of Clomid. I'm curious Victory Men's Health Provider to hear if using enClompihene only would be a better option since the half-life is about 10 hours compared to 6 days with the zuClomiphene?

      We would look at other issues that could be causing an androgen imbalance such as inflammation, insulin resistance, visceral fat, and metabolic syndrome. Typically estrogen rises in relation to total and free testosterone increase.

    Thank you for the informative video! I would really appreciate your thoughts on some questions I have?
    Does the Clomiphene act anywhere else besides the hypothalamus?
    In your experience, do your patients report increased libido? If not, what are your thoughts on adding something like Proviron to their treatment?
    What is the likelihood of a patient coming off Clomid and maintaining healthy levels?
    I've heard reports of increasing SHBG over time with Clomid, is that true and is there anything you might use to lower it?

      @@maxgg8415 I'm not saying we would keep a patient on clomid their whole life but just to be clear there are actually long term safety studies on clomid. "These data demonstrate that clomiphene citrate is safe and effective with few side effects when used as long-term treatment of hypogonadism."

    How long can someone be on Clomid if the medication is working for them? Can they be on it long term ( 5+years) or is it just temporary ( 1 year or less)?

      Hello - Typically in young men a low LH & FSH is a good indicator Clomid could work.. Patient results always vary though.

      Thank you for your question. We typically review a patient's labs every 8-12 weeks to determine if the patient's treatment plan needs to be adjusted. Some patients respond very well to long term use of Clomid and others will need their course of treatment changed after several months of therapy.

      @@ebp1234 it's not. if you're truly low you feel it QUICK when your levels come back up. i also noticed it 2 days later after starting. i'm sure it's man to man so everyone may not have this happen but i did. brain fog gone, alert in the morning etc. hadn't had that happen in years.

    Hey doc, thanks for the info.
    If total testosterone is in the low 300 range, and LH is mid range at 6, e2 at 35, and FSH is at 12.45 (which is on the higher end) could one start clomid or will it cause issues with FSH?
    When does one factor in arimidex with e2 level checks?
    Lastly, how to prevent eye damage, are there warning signs?

      We have had patients with similar blood work still respond well to Clomid if the goal is raising LH for symptom alleviation.
      We do not prescribe Arimidex due to the long-term side effects of that medication.
      Out of 100’s of patients on this medication we haven't experienced any eye issues or damage. However, blurred vision can effect 1.5% of patients.

      That's a good question. It would depend on how high the Estrogen is prior to treatment. In more cases than not, clomid increases the men’s estradiol levels. It is also harder to manage a man's estradiol levels when they are increasing due to Clomid.

    I'm 63 and have been taking very low dose Clomid for 4 years. I was taking 6.25 mg twice a week, test was 750. Lowered dose to just 6.25 mg once a week and test is still 742. Anyone else have such high levels from such a low dose?

    Hey doc, my testosterone is back to normal range from taking Clomid but if I stop taking it will my testosterone drop to low again? or will it just drop little? I didn’t alway have low testosterone.

    Been on TRT FOR 5 MONTHS, 52 yrs old T= 252 PRIOR TRT. Urologist wants to start up my T with CLOMID 25mg for two months and then get off to see if my body can do IT itself ( When I first tested for T I also tested +covid, year prior also test in the 200"s, I was also 40 pounds heavier . Should I stay on TRT or moved and ween myself of TRT with Clomid for two months.

    What could cause a man to experience extreme overstimulation of LH and FSH ,but only a small increase of Testosterone to about 450.ng/dl in clomid treatment

      Basically clomid is sending a message for your testes to work which causes that increase of LH & FSH. If they are stimulated and trying to produce but can't achieve the testosterone level your physician is looking for then you would need to discuss alternative options with your physician. Additionally, it is not about your Total T level. It is more about your Free T level that needs to be considered.

      @@victorymenshealth hypothetically speaking, how would you handle a patient with LH 12.65 HIGH and FSH 25.59 HIGH while taking 25mg clomid eod? Ftest 15.58ng/dl and free Testosterone at 3.15. what would cause a reaction like this?

    Can we cycle Clomid during monotherapy to avoid potential visual side effects? For example, taking 4 weeks on and then rest 4 weeks off and then restarting...

      1.5% of patients may experience blurred vision according tp the Medscape prescription drug monographs that are based on FDA-Approved labeling. There is no evidence that supports that sort cycling to try an avoid that side effect.

      @@victorymenshealth But wouldn't continuous use also be blocking the E receptor sites and therefore be detrimental on the long term? The estrogen would not be doing its job on these sites as expected for health purposes, i.e. vision etc. Unless a low dose doesn't block all E receptor sites and leaves some room for E to still do its work?       @@memeducacao Being that clomiphene is a selective estrogen modulator, it doesn’t apparently have the ability to antagonize any/all estrogen receptors throughout the body other than those related to the hypothalamus, so the beneficial effects of estrogen on other target areas would still remain in place.
      Several animal studies have indicated that clomiphene actually has estrogen agonistic effects in the skeletal and cardiovascular systems … estrogen receptors in the hypothalamus are not necessarily the same as estrogen receptors elsewhere in the body and clomiphene would not always be an antagonist at those sites. This is most likely due to the fact that clomiphene citrate is racemic mixture of two stereoisomers (zuclomiphene and enclomiphene) which are a weak estrogen agonist and potent antagonist, respectively.

    any info on hcg monotherapy for low T?i tried andro gels before and it helps for alike a month then i feel like shit again after my own T goes down. i started test injections and they are much more effective but i dont want to go into shut down and become infertile. i ask my doctor about HCG and they say they never heard of anyone using it before...... i took clomid for 2 months and my T shot up to like 700+ and i felt great until the side effects from clomid kicked in and effected my eyes so i had to stop. im def secondary and can make my own T with the right product to force production HCG maybe? cost of hcg?where to buy it?

      We do also use mono HCG as a treatment option. There are some online telemedicine companies that you could speak with about it as a possible treatment option. I would say on average a bottle of 10,000IU HCG is $100-ish.

    Could clomid not work even with higher free test and total. I went from 240 to 900 total and after about a month I felt amazing and after 2-3 months I have all my negative symptoms again. Is this possible even while seeing higher end of the ranges for t levels and still being symptomatic

      Hi Jack - Do you know what your Free T is? With Clomid, SHBG can increase over time binding up the testosterone therefore causing the free testosterone to be less available.

      @@victorymenshealth yes my free T was 18.9 pg/ml and my shbg was 40.9 about a month ago but not the most recent I haven’t checked it recently. I know shbg can have a affect but could it be that great to where clomid isn’t helping relieve my symptoms. Could trt possibly work or is this a sign that it may not be testosterone that’s making me feel this way. Also the month I felt amazing all my lifts in the gym went up and my mood was amazing. I feel there is some correlation there

      @@jackcornblatt6183 why aren't those labs being ran at follow-up? A medical provider should be monitoring all of that to truly optimize your hormones. It is likely you could respond to testosterone better than you did clomid. Just because you haven't gotten the results you want from clomid doesn't mean you won't from testosterone. There is a different feedback loop involved. Different things work for different people. Without your medical history there is no way to make a call on if your symptoms are related to something else.

      @@victorymenshealth yeah I’m not sure but I heard a lot of people say that after your negative symptoms subside on clomid they come back and I told the doctor I wanted to go on TRT but he said since my test was in the higher normal range that trt would not do anything

    1) Is it safe to say on low doses, clomid should be able to increase the numbers and the most appropriate time to retake blood test is after 8 weeks from the day of starting clomid treatment?
    2) Assuming it does increase LH,FSH and Total Testosterone, but the levels are not optimal.... Is it safe to continue? If yes, for how long

      Hi Kanishen - Typically, we do labs every 12 weeks. There are not a lot of studies done on long term use of Clomid. So, how long you take clomid will have to be a conversation between you and your physician.

    My doctor just prescribed 50 MG once per day. Is that the best way to take it? Or should I cycle it?

      @@deluxeassortment We have the mindset to try to get a result with as little medication as possible. Same holds true if we were doing testosterone. Also, It is going depend on how your LH & FSH responds to the dose he currently RX you. I can't think of any patients we have on 50MG per day.

      A "normal" range doesn't necessarily mean optimal for your age. So, your treating physician would have to look where in the range you are based on your age.

      That really isn't a complaint we have heard from our patients. You should consult your treating physician and see if they will run some labs on you to see what is going on.

    I have a question can you also take clomid for 3-4 weeks 25mg/day if so how should you stop? just drop off completely as usual and go out completely?

      You would need to consult your doctor but typically we don't see people tapering off clomid. Not sure what your goals are for taking it 3-4 weeks.

      The recent literature does not support the use of AI's. Clinics still prescribing AI's to manage estrogen are doing their patients a disservice. The testosterone dosing and frequency should be addressed prior to prescribing an additional medication. Men do need some estrogen. Eliminating all estrogen is not the answer.

      Hi Alex - To know if clomid would be an option for you, you would need to get blood work and have your LH and FSH function tested. This tells us if your body will be able to stimulate more testosterone production for you.

      Hi Kevin - There is a lot to unpack in that question. Are you only taking Propecia because you feel like Clomid will cause hair loss? Or were you on Propecia prior to starting Clomid? Do you know what your DHT levels are? Is the same doctor prescribing both to you? Propecia is NOT going to be your friend when trying to optimize your hormones which is why when you look at that medications top side effects erectile dysfunction is #1.

I have been taking Propecia for 2 years now and just recently started using Clomid as I am experiencing fatigue and depression due to low T. I'd like to continue taking both to maintain my hair and T level, however I was wondering if there are any potential risks for taking Propecia and Clomid together......My intention is to increase overall T level but block only DHT to countermeasure hair loss.

      @@kevinkohama1412 I understand where you are coming from but unfortunately it isn't quite that simple. Dihydrotestosterone (DHT) demonstrates: (1) an affinity to androgen receptor four times greater than that of testosterone, (2) a rate of dissociation from the NR3C4 three times slower than testosterone, (3) an amplification of the androgenic signal by its more efficient conversion to the DNA-binding state, (4) increasing the so called “transformation” process. What this means is the conversion is necessary in order for testosterone to fully exert its positive and protective health effects throughout the body.
      DHT can certainly contribute and play a role in the hair loss process, but it is tough to justify going after an essential male hormone when it is not the causative factor of hair loss. Have you tried any thing else to prevent hair loss? Topicals? Etc? There is actually something called Post-Finasteride Syndrome (PFS). Finasteride is the generic Propecia. That is a very hot topic at our anti-aging hormone conferences.

      Hello Alex - “Normal” is dependent on the lab as most range between 1.24-8.62. Lab work should include SHBG as that will determine the free testosterone available. Free testosterone is more important than total testosterone.

If we use testosterone and/or testosterone with HCG it really is specific to each individual patient, what their goals are, and if they are still planning on having children or not.

      It would just be ease of use. Some people would rather swallow a pill every other day. I’d say do 25mg once a day. Keep a more consistent level.

      Good Question! You can take 25MG every day. It is more cost effective for our patients to purchase the 50MG tablets and take them every other day. They are scored in the middle so they could cut them in half if they chose to do so. Our results have always been very good with the every other day protocol so we have stuck to that vs taking it every day. That decision has been made based off of treating hundreds of men with Clomid successfully.
