Therapy Progress Notes: How much is too much?

How much is too much for my therapy notes? This is one of the most common questions I get!

- Is this information necessary, does it support (or ruleout) a diagnosis? *Documentation, insurance, accuracy.
- Is this information clinically relevant to ongoing work with client? *Meaning, themes, significance.
- Would this be important to know for another clinician for something like transition of care. * Quick synopsis/conceptualization of the client.
- Is the information factual, can it be evidenced by something that was observed? *Differentiate observation and facts vs subjective interpretation.
- Consider if a client quote would be a better option. *Sometimes they capture more than a description can.
