It's randomised. You get different companions every playthrough - though the last names are indicative of their skills I.e. Surgeon will always have high healing, while Spicetrader will have high trade. It's random because if you enable character death at start, Bannerlord needs a way to repopulate all the hero companions that will be dying. Thus, they randomly generate new ones from a pool of first names and second names, with the second names being their class and skill identity.
