    As a PA student, I have to say that I expected these comments to be a lot more negative, especially on a Medical School/Physician - focused channel. I'm loving the positivity towards the many roles on the healthcare team and I hope that many of us folks currently in school & new grads continue to foster a culture of respect and friendly competition amongst all healthcare professions, especially the diagnosing + prescribing folks. We would be able to change our professions for the better if we all worked to build each other up as hard as we did to get into school, through it, and practice!

    Love a medical profile that covers the topic of PAs with respect. The only downside I saw with becoming a PA when I was applying was how negative a lot of the physicians viewed PAs and I was so scared that I would be doomed to being some doctor’s b*tch while getting no credit or respect. I wish doctors understood that people who chose PA did it because that role fit their career and lifestyle goals and to still make a huge difference, not to undermine the importance of physicians. Most of us WANT someone above us. I promise we are not trying to take your spot or pretend to be doctors.

With what PA's knowledge and clinical skill set, they provide to patients and have a capacity in the same vein even if not at the extremely extensive, intensive level that physicians do. And fortunately, even many physicians. Say to the $40k/year residents, "How's that working class pay going?" Hahaha.

  • @matty9447
    @matty9447 4 роки тому +92

    Yea, I noticed a few inconsistencies in this video but its okay. Yes, on call almost a guarantee, esp. if your working a hospital or surgical setting. I did not want to cause any controversies but there are many grey areas that were covered in this concise video. My best recommendation if considering this field, be sure to do your homework, and ask PA's; as even though PA have and must try to maintain such a broad knowledge of medicine; it can widely vary between each PA depending on their practicing State, scope of practice, years of experience etc...
    Some PA will never go into surgery post grad. Therefore, their surgery background was limited (ended) after rotations were completed. For example, depending on how much surgery background one has under their belt; PA can play a big role in the surgical aspect. He made it seem as if PA only do the things listed. However, surgery and PA scope of practice is a massive grey area. A lot also varies in the State (USA) a PA practices, scope of practice, physicians expectations, experience etc...
    Also, 2-2.5 years of school is because its accelerated. Nowadays, most schools have began to adopt this accelerated model into their program. People need to remember that its 3 years worth of school in 2-2.5 years without summer breaks. Meaning, you go all year with very little breaks. Trust me when I say, you do not want to fall behind during this program.
    Also, PA initially have superficial knowledge over nearly all aspects of medicine, but once they specialize; PA must then gain a deep, extensive, and vast knowledge of that particular specialty. This is where the Lifelong Residency he was referring to in the video. Also, physicians only requires a re-cert board in only the area they practice medicine> However, PA are required to test on all aspects of medicine for re-cert (along with CME hours etc.). This is a big deal. Because if you do not want to maintain extensive knowledge in an area you may not like to learn; it may be better to go that NP or Physician route.
    Lastly, do not forget about all the exams. In most cases, before a PA can be a licensed/certified PA in their State(USA), PA must also pass a multiple hour board exam covering nearly all aspects of medicine post grad; in addition to that re-cert board exams every 10 years. The re-cert exam is when PA have to retake another similar version the exam they took post grad. This can be difficult because if you specialize i.e.(Orthopedics) for 10 years; and when re-recert comes back around, it may be smart to take a refresher course, as the exam will cover areas of medicine you may not seen in nearly 10 years. There's also multiple hours of continued educational hours one must log in order to maintain board certification and license.
    Therefore, even though he kindly helps to give everyone a general idea of a particular medical profession, as he did in the video with the PA profession, it is however, hard to cover all those in and outs to any medical profession. Thus, its always recommended to do your homework on the desired profession before dedicating and sacrificing a big chunk of your life to school.

    Generally good content and accurate, has a 9-year PA in family practice I just wanted to clarify a couple of points: you mentioned that PA's do not follow patient's longitudinally, this is incorrect, it depends on which specialty as in primary care we absolutely follow patients and we have our own panel. I have some patients who have been exclusively seen by me for the last 8 to 9 years for example. As far as taking call overnight this also is something that often is done by a PA. I have been taking call anytime my clinic is closed for the last 5 to 6 years. Lastly, and this is a important pain point for many PA's and nurse practitioners, the term mid-level is demeaning and has been replaced by the more appropriate advanced practice clinician or APC

  • @ryanrafferty5949

    As a PA, we are able and do parts of the surgery besides retracting and suturing. I know many plastic PAs that will do one implant while the doctor does the other. I also know a few that perform organ procurement on their own etc. We also can handle the nuanced more complicated cases in the ICU. Take home point is your PA can do and handle as much as they choose to pursue.

It seems like he talked only to 1 PA, and he spent a ton of time talking about how limited PAs are in surgery, while the vast majority of PAs don't work in surgery. I just finished my surgery rotation, and the PAs did a lot more than just retracting and suturing. They were creating tendon grafts and inserting PEG tubes, and they were intimately acquainted with the anatomy and the minutia of every procedure. They also took call! And saw patients on their own in clinic! I'm really not that impressed with this channel when it comes to anything besides medical school.

    I would like to add that in my experience in the ER, PA's act completely autonomously with supervising Physicians a phone-call away. But all diagnostics, prescriptions and care plans are decided on by mid-level providers. (This is in a fast track area not the entire Emergency Room).
    I think it is to be considered that there is more autonomy in some specialties than in others.

    For clarity and insight, I'm about to graduate PA school in < a month. This is a good intro for those deciding between the varying professions. Ultimately, as he said your decision should be based off your career goals AND the lifestyle you want for yourself. If you're new to healthcare and really want to take care of patients it shouldn't be about prestige, money, and ego IMO. It's also not about who's more intelligent and other things some people in the comments say comparing going PA/NP/physician etc. We should all be working as a team.
    To note: PAs may have a perceived better work-lifestyle, but that'll be based on the specialty you're in; you can transition between specialties or work in two specialties at the same time, and have more flexibility in your career which I find appealing after years in military medicine and research. Surgery...yes you will always just be a first assist in the OR so if you want to do surgery go to medical and do a surgical residency. You may have more autonomy and experiences in various federal agencies vs private sector, but ultimately the role is the same as a PA.
    For all the pre-med/health students be sure to do your research. You don't want to realize one profession is not for you and drop out...or go through it and hate your job when you start working. Either way you'll still have to pay back any school loans you take out.

      If you really wanted to provide good care to patients at the highest level, wouldn't being an MD/DO be the best option? Why as a patient would I want to see a PA over an MD? The wait times could be less and that seems to be the only advantage.

    • @yohickey
      @yohickey 3 роки тому +2

      @@billr5842 I guarantee you've probably received excellent equivalent care from a PA or NP and didn't realize it. Having MD/DO doesn't mean they will necessarily provide "good care" any more than a PA/NP. If you want only want to see a physician that's your choice. Many people choose to see a PA over a physician. I've personally preferred to see a PA twice over a physician because they didn't seem to know much about/or just ignorant to my situation. The PA had more experience so opted for that individual and my situation was handled during the first visit. So degree level is one thing, but clinical experience is equally important for any clinician.

    Neurosurgery PA here, it’s an inaccuracy to state that first assist PAs NEVER do the actual surgery. A more accurate statement is to say that a first assist PAs role will vary greatly depending on institution (community hospital versus teaching hospital) and the relationships they build with attending surgeons.
    A comparison of pilot and co-pilot is most appropriate.
    I may be biased, but I have yet to see a more beautiful and efficient relationship of surgeon and PA working together. My hard work frees up time so that my surgeon can impact more lives. As my attending surgeon states, PA stands for Pain Alleviator…

    PA do take overnight call

    • @matty9447
      @matty9447 4 роки тому +22

      Some PA never going into surgery post grad; and their surgery background was limited (ended) at their rotations.
      Also, 2-2.5 years of school is because its accelerated; which is were most programs are going. People need to remember that its 3 years worth of school in 2-2.5 years without summer breaks. You go all year.
      Also, PA initially have superficial knowledge over nearly all aspects of medicine, but once they specialize, PA will need to dive deeper with a extensive vast knowledge of particular specialty. This is where the Lifelong Residency he was referring to in the video. Also, physicians only requires a re-cert board in only the area they practice, where PA are required to test on all aspects of medicine for re-cert (every 10 years post grad). This is a big deal. Because if you do not want to maintain extensive knowledge in an area you may not like to learn; it may be better to go the NP or Physician route.

      Lastly, do not forget about all the exams. In most cases, before a PA can be a licensed/certified PA in their State(USA), PA must also pass a multiple hour board exam covering nearly all aspects of medicine post grad; followed by re-cert board exams every 10 years. The re-cert exam is when PA have to retake another similar version the exam they took post grad. This can be difficult because if you specialize i.e.(Orthopedics) for 10 years and then re-cert comes back around, it may be smart to take a refresher course as it will cover areas of medicine you have not seen in nearly 10 years. There's also multiple hours of continued education hours one must log in order to maintain board certification and license.
      In the end, even though he kindly helps to give a general idea of a particular medical profession, as he did in this video with the PA profession; it is however... hard to cover all the in and outs to any medical profession. Thus, it's always recommended to do your homework on a desired profession; before dedicating and sacrificing a big chunk of your life to school.

    PAs do take work home, absolutely keep track of patients longitudinally, and definitely participate in overnight call. Many PAs are left unsupervised, especially critical care / IM PAs who work overnight with no supervising physician in house. PAs will run codes, and can preform any procedure a MD can (with permission / experience). The average compensation is actually $115k (AAPA) and rising with 95-100k being a good starting salary. PAs are not just “superficial” (you mainly focused on surgery) - many PAs are designated to be their specialtie’s consulting provider when needed, showing their depth of knowledge in their field of practice outweighs a MD’s knowledge who is practicing in a separate field (despite the education difference).

    At this point, I have been a PA-C for over a decade with practice in Neurosurgery, Orthopedic- Foot and Ankle, prolonged volunteer position W Africa inpatient and outpatient Internal Medicine. I have followed my own patients and I collaboration for years, complete basic procedures. This has been a varied and rewarding career.
    A few updates to request/suggest
    - Please use a different term than midlevels, much as physicians would advocate for use of clinicians or practitioners over providers, this term implies a lot. Associate Practitioners or Advanced Practice Practitioners are common
    -PAs take call and work longer hours than you report leading to burnout and work life balance issues, lesser than than MD/DO counterparts, but still significant.
    - additional draw back is often very little influence to MD/DO business practice decisions/responsibilities and changes- this is an added driver of burnout/career stressors
    Thank you for highlighting that PA's are always learning and gaining practical knowledge with colleagues- we are all better together in medical teams!

    There are many different providers within health care, value discussions and various streams and pools of influence help to bridge gaps in care. When we consider the back end connections of how systems work and how truly valuable expertise is, the Value propositions shift some. For instance, PA's are able to help with a lot of collaborative needs... social workers and translational medicine concepts add value to those realities. The concept of finite need or finite resources isn't exactly accurate. The spheres of influence shift as better understandings grow.
    I was fortunate enough to see some great doctors, nurses, PA's, social workers, on and on.... when the career of caring takes the shape of understanding the larger "cogs"; the conversation brightens. Doctors and the decades of training can never be undercut in estimation, but the needs of connections and positive feedback loops while strengthening patient care... these understandings illuminate a lot.
    I'm always in awe of providers...

    The first was in the ER . I was bleeding days after a fibroid surgery . After the bleeding stopped she told me they saw some a cyst . And I asked her what do I do about the cyst or is the cyst part of expecting my period . She told me she doesn’t know and was very nonchalant about it . She then went to discharge me told me to follow up with my ob/gyn . Will never forget the look on her face and how she talked down to me .
    The second was when I was at a physical therapy place . My usual Dr that was good wasn’t around and they had me see this pA . He was in his phone just watching cats on UA-cam . And when he spoke to me he was very dismissing saying they will discharge me from physical therapy . Knowing full well I was still in pain .
    My point is , People should go into PA if they have passion for helping people now as a quick way to make money without having any of the responsibilities Drs have of actually treating a patient

    One important thing to also consider is the learning environment. If you put medicine into a book during PA school you learn like 70-80% while medial school you learn the whole thing, of course medicine is still expanding but generally that’s the case. PA school is like a race you it starts right off with high intensity a bunch of quizzes every week and a LOT of information crammed in just 2 years but it’s much shorter. Medical school it’s still a bunch of information crammed but medical school is more like a marathon gotta keep a steady rythme but it’s much longer and even after medical school you still got residency. Of course each school has aspects of the other but down to the core pa school was all about getting people competent in medicine quickly and getting them in the workforce and medical school is about getting people very knowledgeable and good at whatever speciality they go in. I think how you learn and what environment you want really is important cuz you don’t wanna burn out while still being in school.

    What he says definitely applies to relatively new PAs, but with experience the line can get blurry. I’m a physician who works with PAs in the critical care space, and depending on the physician, a PA job can be like an ongoing fellowship in a specialty if the physicians are willing to make the effort to expand the knowledge and skills of their PAs. An experienced PA who over the years has gradually assumed more responsibility and autonomy as the knowledge and skill base has grown ( very much like a progressing resident or fellow) can become sort of a junior physician, depending of course on the nature of the supervising physicians and their trust and comfort level with PAs.

    I'm not so much worried about cost, as I am blessed enough to have assistance with schooling. I am 41 years old, have an 8 year old and 3 year old, as well as somewhat of an introvert......BUT, that being said, I made it through 3.5 years of school 2016-2019 and am currently a rad-tech. Currently working at an UC clinic that also has occupational medicine. My biggest concerns are these: length of schooling (young kids, only an Associates degree currently), classes such as microbiology, chemistry and pharmacology, and lastly having the nerves to be able to present cases to physicians, professors, etc (yay for being an introvert)....but both PAs at my clinic say that I should go for it. They are very, very supportive. I'm SO TORN on what to do. I have taken statistics, Anatomy and physiology lecture and lab as well. Tested out of biology bc of scoring a 24 on ACT

    I hope you can do a video about being a psychologist as well. I'm a fourth year medical student and was considering psychiatry for my residency. On the other hand, as I've learned the field of psychology in my informal research , and talk to several bright psychological practitioners, I've become to be so much curious about the field than I do with psychiatry. I watch you're video about it, and I loved. I hope you can do one solely for psychologists. Thank you

    Physicians: anywhere they offer your specialty, there is a job for you. You can even start your own practice.
    PAs: anywhere there is a physican, there is a job for you. More limitations on starting your own practice but possible, this is rare.
    Physicians will be your boss, which is not bad, physicians are great to work with, find a physician that you can vibe with and gives you autonomy to work independently within the clinic/hospital. If you don't match energy with your physician, then the amazing part of being a PA is that you can move down the road to the next clinic or hospital. You are not limited to location, you can work anywhere and with any specialty you choose too, you can even do locums (travel PA). Just be the best and most competitive PA you can be and you will be an amazing provider.
    My personal opinion, being a PA has more benefits than being a Phycisian. Flexibility, hours, salary, and location. You get less money, and are not the boss, but with $100k you will be happy and who wants to be the boss of a clinic or department in a hospital when you can just work with a great boss and leave work at work when you clock out. Then enjoy your life at home with amazing income and time. You don't spend 6 additional years learning, you don't stress worrying about picking the "perfect" specialty because you can just switch as a PA. Time is the most valuable thing we have, and being a PA saves you a lot of time that you can be using making life long memories. Be smart, invest, save money, get more sources of income, and with a 6 figure salary the world is your oyster. Enjoy the amazing life you will have, but do not be comfortable, always look at what else you can do, always have a new goal, and something to strive for even after you are in the career you want to be in.

      Current medical student here. Just do well in middle and high school and establish good study strategies that will set you up for success in your college academics. Relax and have fun. Be a teenager. Do some light shadowing in high school if you get the chance. No need to worry about doing things for medical school until you are in college.

      @@chaeglows your brain is still developing, don't worry about judging yourself now! Focus on understanding topics rather than memorizing them only. Make flashcards to memorize a good base of knowledge about a topic, and then do practice questions to apply that knowledge. Doing well in school isn't determined by having a good memory or not. Don't worry about it, have fun as a kid.
