Same mistakes all the time in game development yup totally and I have a theory Most leads, managers and producers in games grow into their managing position from art, IT or design roles (in my experience). Which means they have experience in their field but 0 in MAnagement (which is a science and should be studied). So they end up simply improvising the way they manage projects (based on their own narrow experience) rather than following proven techniques. The opposite is true too, with some managers simply folllowing text book what they have read on the management wikipage, without any adaptation to the reality of the situations they are facing.
Timing plays an important role, nowadays youngsters in any job is kind of dead end to be honest
