The chance of getting cancer is low but adding several spices like they do in India, Papua New Guinea and other countries that have high affinity for spices added to the chew has higher inflammation and cancer. The three main components are betel nut, betel leaves and lime while a vast amount adds a little bit of tobacco.

It's surprising that in Taiwan the habit of chewing betel nut still a thing. In Indonesia, the last time I saw people chewing betel nut was decades ago, done by old people and they were the last generation to do it. However the main reason for the disappearance of the habit is not related to health, but more about aesthetics and hygiene. Chewing betel nut makes someone's teeth and mouth red as if bleeding, and the streets littered with disgusting red saliva that those people spit.

In North india they call it"Beeda". Ingredients are Betel leaf, betel nuts, lime paste and dried tobacco leaf. Chewing it causes Oral cancer. Remove tobacco from it and no worry about oral cancer.

In Bangladesh, it’s more common to allow the fruit to ripen, sun dry & then throw away outside husk. Very common to only chew dry nut with leaves & you can add other ingredients like tobacco, sweet, sour spices etc and this does lessen the effect but also lessons the harmfulness on your body. Some people do chew the raw nut but it’s less common & some will mix dry & raw nuts.

What I don’t understand is other countries consume betel in far greater numbers and don’t have the same issues with mouth cancer. Could it be the additives they are using with it, or something else besides betel nut? There has been cases where lime has a high concentration of radon, which would 100% cause cancer in higher numbers.

I got a tin of betel nuts from an Indian grocer here in Canada before. It wasn't great. It was diced into little dried cubes that were covered in lime (mineral, not fruit) so it looked like kitty litter. There wasn't really any taste, and they were rock hard, so chewing them was difficult. The actual effect wasn't even as strong as a cup of coffee. I couldn't even notice it. Paan masala, betel nut with various spices mixed in is much more enjoyable. I've had some very tasty blends. Would highly recommend if you're in India or near an Indian grocer. Not so much the betel nut itself, though.
