This particular instance of samarie has a bugged Sprite. When you decapitate a character the game sets their spite to State A which shows them without a head and doesn’t allow you to cut their head off anymore. This samarie’s decapitated sprite is State C (usually reserved for devour) so attempting to change it to State A fails resulting in the sprite remaining the same.
It only works with Black Orb. The way that it's coded in The Hexen, the game never sets a flag that says that a specific character learned the spell, so when Spice Forge checks that flag to figure out who to give the Spicy version of the spell to, it just gives it to everyone. This only works if you learn Black Orb after learning Black Smog and not Blood Golem.
