  • Kicking off our Late Night with Dr. Kim Live Stream series. Today we are chatting about clinical depression. Going over basic fundamental education about clinical depression, how it is diagnosed, how it is treated. Problems with how we approach treating depression in the United States and answering live stream followers questions.

    What is your view on Dr Amen clinics .the scans are expensive.

      Not worth it. They can claim how it’s “cutting edge”; but it’s actually not considered standard of care to perform all those scans. They’re taking an enriched sample of people who are already reporting being depressed, something you can confirm with interviews, to then find findings that are not pathognomonic for depression, bipolar, etc. and then once someone is better, they show you a scan that shows some differences compared to the prior one. But how does one confirm that the patient is better? …. By asking the patient. They can’t blindly look at the scan and simply say the patient is depressed or better. I’m all for advancing the field, as I worked in clinical trials for many years, and some institutions are making great advances in neuro imaging like Brigham & Women’s hospital in Boston; and I truly hope imaging can become a more practical diagnostic tool in the future, but we’re not at the point in the field where we can simply scan someone and tell them an accurate diagnosis. In 11 years of practice, even for challenging cases, I have never referred or recommended someone pursue this. Medically not necessary and in my opinion they are charging vulnerable populations an excessive amount of money in a relatively predatory fashion with claims that are not evidence based.

    Feel no will to smile or laugh, no will to show mannerisms, no will to engage random conversation, no internal feeling of motivation, no will to want to explore new things to try new things, felt like this my whole life, hardly ever laugh to jokes, never ever had will to get out of bed when woke up ever, kind of like in a simulation. Not simply a period of down feelings. SNRI seems work little better than SSRI, helps with anxiety but little with mood/motivation/will/engagement......... but unsure if i need something else. Experienced Psychs 3 month wait at least if they accept referral , might try find another scared i'll get a crappy psych then stuck either way thats why i want to die instead
