Directed to comments, not op, pllleeeease stop confusing MGs & MLs they are NOT the same! Im pretty sure u were not on a dose of 550mgs as u would probably be dead! Which is what 55mls equals..the conversion is 10mgs per every 1ml thats how methadone is dispensed. Thank you for posting this, there's an awful backlash against methadone clinics here in the US. It saved my life. It doesn't matter how much methadone you take at 9 a.m. and then 24 hours later you go to the clinic you do not feel exactly how you felt the day before an hour or so after you dosed. It just doesn't work like that. Because when you go to sleep and wake up it changes how you feel it's like hitting the reset button. I don't care how much you take every morning when you go to sleep you're not going to wake up feeling great until you dose. What they should do is allow you to take half in the morning and maybe half in the late afternoon because the methadone wears out

@vincent insomnicide I have a question. I started methadone yesterday, as a short term detox plan from 1 year IM heroin addiction. Before going into methadone, I already tried cold turkey 3 times, but failed at around 40 hours mark, scored, but I'm sure my tolerance dropped quite a bit. So my starting dose is 10ml/day methadone, it holds withdrawal at bay 80%, my aim is to quit totally, not for maintenance. How do you suggest I should go about this plan? What dosage reduction and timeline should I follow? Thank you.

I'm really happy to hear other ppl saying that methadone saved their life. My experience was completely different. I live in the US. Methadone (even while clean for several years off of heroin) helped progress my addiction. I started craving cocaine and was constantly trying to come off of the methadone gradually. The only "advice" the clinics (which were horrendous places that trigger me to this day) had was to go up on your dose. Finally, I got off of it bc I had no choice- was kicked out of a clinic with no warning just before Christmas 2022 due to changes in ownership of clinic as well as my "defiance" as a patient (long story). I spiraled of course and almost died (seizures etc) but I was able to come out from the grips of the hell that was my life for years and am thrilled to report that I am clean and a totally different person now. That's just my personal story.

This doctor is so right 😎......When i joined the methadone program in my country i was given advices by many addicts like " You must have at least 100 mg to be without withdrawals or don't raise the dose above 60 mg cause you won't be able to taper of anymore and so on".......As i regulary visited clinic to take my daily dose and having discussions with my doctor about my well-being and changing the dose i remember, that he told me the same thing (he's not a tipical stubborn old school type , who has no sence of reasoning with his patients....He's very understandable and i consider myself lucky to have him)......I starded with 40 mg (they won't give you more for the first time) and gradually raised to 140 mg and that is clearly correct dose for me because as this doctor says, i feel very fine for at least 26 hours and when i take my daily dose same time each morning around 7 am, i don't feel any different.....I feel fine all day and night, full of energy, almost always in a good mood......And that's how the methadone should work for everybody i think.....Experiment on the raising (or lowering) the dose to achieve the constant, stable feeling all the time......I think it's worth taking time and trying different dose until you feel confortable.......

Its 10mgs per 1ml that methadone is dispensed at least here in us an most other places. So if ur talkin about a 60mg dose that would be 6ml etc. Just wantd to let u know not tryn to be an ass!

I’ve been on suboxone 2 years and always felt “off”, and still got cravings even at 24mg. I switched to methadone, and steady at 50mg. If you’re going on MAT, DO NOT listen to anyone, especially comment sections. Listen to your provider and your body. Don’t let anyone tell you otherwise. It’s your body, and there is no such thing as the correct number or medication. As long as it’s keeping your from using fentanyl, comfortable, then it’s working for you.

Some really good information and experience - based knowledge shared by this man here. You can tell he has been prescribing it over 2 decades...
I've had a few different experiences as a methadone patient. Early on the recovery agenda seemed to be overshadowing my needs as a heroin addict. Prescribers would be reducing the dose the moment I gave a heroin free sample, so I had to make sure there was always heroin in my pee to ensure they'd just keep me at 50ml, which was too low, but they were for some reason not prescribing people much back then. This was around 2010. In 2016 I switched providers and they listened and tapered my dose up to 75ml, which made a big difference, although it still doesn't carry me over a 28h period like this guy says. I've been able to say that I'm not interested in being methadone free and that my aim is to live life free of heroin (and crack). I wish I could get an increase to about 90ml.

I have a question. Thank you.

Best methadone related video I've seen on youtube. I had a counselor at a clinic in DC who was a bit like this guy. He insisted keeping a pragmatic, no bullshit view of methadone treatment. I let that program after 10 years. Best decade of my life. More motivated, less depressed, more energy. More follow through. I started a successful business when I was in that clinic. Still going after 15 years. Thinking about going back on methadone. I function better. That's the truth of it. I was born addicted to methadone and heroin. My mom was on a good run for her entire pregnancy. There are some people for whom staying on mmt for their entire lives works. I may be one of those people. Right now, I use occasionally. I never go looking though. Only if it comes to me. And then it's maybe once a month. A day or two is enough to put me into mild withdrawal Then a full on PAWS.... usually until something shows up again. Purposely going into a situation that some call "liquid handcuffs" (with no habit) may seem crazy. It does to me. But I just can't live feeling this way anymore. I'm getting old, and I have shit to get done before I die. What kind of handcuffs am I wearing right now? Because I am tied to this couch. This house. I can barely make it out of my county. So maybe liquid handcuffs are better than these.

My dose is actually 220 mg and has been for several years now. When I got pregnant 220 mg was no longer showing up in my urine less than 24 hours after my last dose and I actually began to go through withdraw and almost miscarried. Dr. raised my dose ...slowly over time...and I ended up at 350 mg. When my baby was born he had zero withdrawal and when I gave birth to my next baby...11months later....and on 350 msg he didn't experience withdrawal either. I was so incredibly thankful!! After the birth of my last two sons I went back down to 220msg and have been there since. I've had several peak and troughs throughout my time on Methadone and each and every time ...according to my Doctor....I'm at a therapeutic stable dose for me. I definitely agree that the "right"dose of methadone varies from person to person and what works well for some...doesn't necessarily mean it will work well for others.

Secondly....let me assure you that I'm being 100% truthful when I say I had two babies...less than a year apart...while being on 350mgs of Methadone...And neither of my sons experienced withdraw. Now...the fact that you "dont buy into it" means nothing to me. I know...I lived it. I've been assured that what happened to me and my babies (no withdraw after birth) is extremely rare. But it can happen...It happened to me. As far as coming off methadone some day, that's definitely my plan. And as long as I taper down slowly...and follow my Drs advice I should be fine. Regarding the statement you made about Methadone being in my bones....that is a myth. Look...I believe that there is no magical # of mgs that works for everyone. We are all individuals. What works for me...might not work for the next person. Everybody metabolizes methadone differently. My Dr. Has monitored my levels very carefully and I'm satisfied that I'm on a stable therapeutic dose that is right for me. Hope your Memorial Day was great. Have a fantabulous day!

I was getting under-dosed from my chemist. That was my catalyst to get off it for good. I managed to quit alcohol and cannabis, during the withdrawal process and haven’t picked up a drink or drug since. I have lost 10kgs, getting back into the employment game/job interviews. I eat very healthy food(intermittent fasting/detoxing) and my energy levels are almost back to what they were, prior to starting drugs(15 years old). Back before then I was an athlete, was heavily into Taekwondo competitions and was even in the Sydney 2000 Olympics Opening Ceremony - Deep Sea Dreaming segment. I could have had a life if I had made better choices then and later on, as the drugs went from the party scene to the ones that block out pain/feelings (that was the real killer for my life). I highly recommend to get to the point where “LIFE” is more appealing than isolation and scoring drugs from shifty people, as well as associated Police dramas/costly court appearances.

Hi. Somebody just asked about dose equivalence between methadone and bup. Not straightforward. Equivalent therapeutic doses would be roughly 4mg = 30mg then 80mg about 16mg. But cos you get a ceiling on bup above 16mg - bigger doses last longer I think you should focus on whatever dose works for you. 12-16mg will hold most people / but no magic number! If you get withdrawal on 8mg go to 12/16. Some props need more. Focus on how you feel listen to your body. Don’t focus on the number guys. Good luck. Thx for all the nice words. Got something brewing as a follow up to this coming next year! Stay safe. Adam

I'm almost 6 years clean, have my own apartment, a relationship with my daughter, a good paying job, real friends, happiness and I sleep with a clear conscience every night. I'm on 100mg of methadone and before being on it I lived like this:
Homeless, hadn't worked a job in 15 years, eating out of dumpsters, doing anything I could to get money, lying to ppl I love, stealing, cried every night from shame and guilt, lost my rights as a mother to the courts. Seems like I'm doing pretty well compared to my old life huh?

So refreshing to see someone who tells it how it is, and not dance around the subject, I was an addict to smoking heroin from april 2021- summer of 2022, and dabbling since July 2019, I began my treatment with 15ml, and then settled at 25ml, but was still getting sweaty and fucked in the mornings, seeing this now, probably shouldve went to 30ml, but it was okay. My highest amount of heroin was about 25 or 30 hits in a pipe per day, and that was im guessing around 50 or 60ml methadone equivalent, but I had reduced a lot from that
before starting my methadone treatment so I was not bad on 25ml. Thats my story, its very complex, now im on 12ml, so seeing this guy explain things straight, is like a teacher and a friend whos there for me, because this shit is fucking scary

I agree with everything he's saying currently coming down one mill a day from 70 and the biggest problem for me is higher dose longer come down period I figure I'll come down and when it stats affecting me I'll pause for a week until I'm okay then continue to come down that's my strategy. Plus being on it is like a get out of jail free card to use because there's no consequences like sickness when you run out and you can take a couple days off if you run out of money then resume use on pay day. Off of all opiates is the only way for me

If it's true, why don't they do it? Methadone has saved so many people, including me at one time, from alcohol use. Sorry to say I am on Suboxone now and drinking again. I need help.

Get off buprenorphine. It's a agonist/ antagonist drug by itself, for what the naloxone in Suboxone. UK? Try Oramorph.
You just need the right dose.
Alcohol will be avoidable easy imho
      Be blessed

I don’t know. Biggest dose I ever prescribed was 300mg. I guess it what you tolerate. Generally most people sorted < 150mg. Good Q. I just don’t the answer ! Maybe there is not one

Very wise words my friend. Methadone is very poorly understood. As a result, people are given poor advice. The ultimate goal work methadone is to cease all illicit drug use. That’s a priority. Coming down off methadone is always secondary to priority number one. If you’ve ceased using heroin, then you’ve been successful with your treatment, whether you’re coming off or maintaining permanently. In my case, I was an opioid user for 12 years. I never went with methadone as it felt like an arduous routine. So I tapered sharply at the end by using some OxyContin from a surgery I had. Basically, I had a bunch of 5mg pills. Started the taper with about 100mg and steeply dropped from there. Oh and also using rkodeine cough syrup for inter dose withdrawal

This gentleman is a rarety, he is golden!!!, unlike every doctor I've been dealing with ever since I was put on methadone in the hospital after emergency surgery on my back. I love that he understands how fundamentally different we all are and at different times in our lives. He really takes this into consideration and does not "go by the numbers" as my doctor continues to try drilling into my head, not a week ago, and for the 4th time in the past 3 years that I was requesting a conservative increase of 3mls. I am on it for pain and here in B.C., Canada it comes as a liquid. I am going to find another clinic or another doctor who will listen to me. I hope I can find one. I wish I could see this guy!!! Ha-ha

Been on 130 mg 12 years ...clean and sober all 12,, methadone saved my life. I was facing a terrible life and it was getting worse every day. I kno some people think it's a terrible drug or your only trading one for the other but it's not true. I sure don't feel high on methadone like I did on dilaudid, and I don't use iv so there's a big difference right there. I could talk all day about this but so much ugly negativity is out there and I know I'm going to get connotation associating with that after this post but I don't care. For me and many people this is what he discussed in the video. And that is reclamation of life...period.

Peak and troph numbers led me to a 205 mg dose! But the vast improvement in myquality of life has been tremendous and I firmly believe for ME I could not be where I am today without Medication Assisted Treatment (MAT). Each persons addiction is different from the next and I wish I could do it without medication however I have tried that route for YEARS with minimal success. God bless anyone still struggling, keep on trying no matter how many times or by what means it takes to kick a habit.

The problem here is we have clincs that chose not to listen to their patients about tolerance...i was wonder if i could speak to you or correspond as i could really do with some advice..i was in a car accident in January and the dose i was on for 7 yrs is no longer working (ive had a script 25 years but my usage has been habitual since i was 12 years old, theres a reason for that that i dont want to go into here)..i spoke to my worker in march but the management are insistent to the point of ignorance that they wont ajust my dose (they havent given me a reason only to intimate that "they dont like to give higher doses"..im now in a situation where im having to make complaints and ive been outright lied to as well..but all the time im struggling..i could use some help)

Thank u for this. My clinic makes me feel I should take the lowest dose possible. The pushback from the clinic have been putting me off wplus the 5mg increase is taking forever but screw what they want I'm going as high as I want till I feel right. This is maintenance not detox meds and I should be able to get a dose thats conformable for me not them.

Been on methadone since 2015. I was on 125 mg at one point. Now I'm on 50 mg and been there for the last 3 years and it's the swet spot for me. I also got take homes too which is very nice, don't have to wake up and go to the clinic everyday.

Unfortunately no doctor allows the patient to choose their dose. The war on opiates has hurt us badly in the USA to the point where pain management patients are unable to get small doses of methadone.
