What is Mania and how is it related to Bipolar Disorder?

  • This video answers the questions: What is mania and how is mania related to bipolar disorder? A manic episode is not the same thing as bipolar disorder. Technically, a manic episode would almost always indicate some sort of mental disorder, but we don't know which one until we go through their criteria and then look at the criteria for different mental disorders. There are four main criteria for a manic episode and the second criterion (Criterion B) contains the symptom criteria. There are seven symptom criteria. Let's take a look at Criterion A. It indicates that a manic episode is a distinct period of abnormally and persistently elevated, expansive, or irritable mood and abnormally and persistently increased goal-directed activity or energy. This must last a week and it must be present most of the day nearly every day, unless an individual's hospitalized, in that case there's no time limit. Taking a look at criterion B, which is the criterion that contains the symptom criteria for a manic episode. It also supplies additional information about how to evaluate those symptoms. It indicates that three or more of the seven symptoms are required for a diagnosis or four or more are required if we only see an irritable mood only. If there is an expansive mood an elevated mood combined with the irritable mood, it would still be three or more. We also see other information here, which is that the symptoms must be present to a significant degree and represent a noticeable change from usual behavior. Taking a look at the seven symptom criteria in Criterion B we see increased self-esteem or grandiosity, a decreased need for sleep, an individual is more talkative than usual, a flight of ideas or racing thoughts, distractibility, increased goal-directed activity or increased psychomotor activity, and excessive involvement in activities that have a high potential for painful consequences. Criterion C says that this must cause marked impairment and criterion D indicates that the effects of substances or a medical condition can't be what is causing these symptoms.
    Bipolar disorder has two symptom criteria. The first one (Criterion A) is the presence of a manic episode. Criterion B indicates the manic episode can't be better explained by the presence of another mental disorder including schizoaffective, schizophrenia, schizophreniform, and delusional disorder as well as other related psychotic disorders. There could be another explanation for a manic episode other than bipolar disorder. Another interesting point with the definition of bipolar disorder is there's no mention in the criteria about the presence of a major depressive episode, yet we almost always think of bipolar disorder as having both manic and major depressive episodes. It's important to recognize that delusions are fairly common with mania even though they're not one of the symptom criteria. Delusions are present in about 75% of manic episodes. Another note relates to irritable mood. Irritability in mania tends to increase over the course of a manic episode. A manic episode could start with elevated or expansive moves without irritability, but we may see the irritability later on in that same manic episode. Also, irritability tends to increase if the goal-directed activities are interrupted.

    I never knew that a manic episode could be indicative of other mental health disorders. I always associated with Bipolar disorder I only (which I have and may be why I only ever heard this) but even in my psych courses, I don't remember being taught or learning that manic episodes can be associated with something other than bipolar, so thank you very much for this clarification.

    This is very interesting and it cleared up some misconceptions that I held myself about these two, most notably I did not think that mania was by itself, just a symptom. I also did not realize there were different criteria that came with Bipolar one and two.

    I think it is interesting that a manic episode cannot be related to medication. A lot of antidepressant medication if not administered correctly could cause a manic episode.

    The video was helpful in distinguishing between mania and BPD by highlighting the the length of time of a manic episode as well as mania being accompanied by irritability in particular when interrupted during a goal activity. BPD only requires one manic episode. I also assumed that BPD included major depressive episode.

    I appreciate this overview. It makes sense to clarify whether an episode is manic or hypomanic as well as rule out other medical conditions. In addition, since 75% of manic episodes involve delusions, consistency of delusions could be anticipated.

    This video explained in detail the criteria for manic episodes. I found it interesting that in Bipolar I only a manic episode is required while a hypo manic episode and major depressive episode are required for Bipolar II

    Dr. Grande,
    I know this video is old, but I hope you see this. Have you heard of the Elisa Lam case from 2013? She was a college student from Canada who took a trip to California alone, and was diagnosed with bipolar. She had many medications, including anti-psychotics and anti-depressants. She was found in a water tank on the roof of the hotel she was staying at, and the last video of her is elevator footage from the hotel where she is seen seemingly talking to no one, making strange hand waving gestures, and pressing a bunch of buttons while at times appearing to hide. Further investigation found that she was not taking her anti-psychotic on the trip, but did take her SSRI and Adderall. Could this contribute to heightened mania, and in your opinion, does her behavior appear manic? Note: a few days prior to her death, she was sharing a joint/hostel type of room, but was given her own room because the roommates described her as odd/bizarre behavior (no elaboration).

    Hello and thank you very much for the valuable information you provide.
    A question : i was hospitalized for reccurent depressive psychotic depression and put on 2 antidepressants plus aripiprazole. I was released with double diagnosis of borderline PD and MDD. After 5 months on treatment i developed hypomania and then full blown mania. My diagnosis then switched to Bipolar Disorder type 1. But till this day i think that my mania was from the effexor. Please how can we differentiate the drug induced mania from the classic mania ?

      If you have bipolar disorder, antidepressants will induce mania.

    Your affect should calm the worst case. Can you explain or have you explained the difference between natural excitability over crisis type life events- and bi-polar anger? Can and will narcissists trigger bi-polar behavior in normal people in their childhoods through abuse? Is this organic or behavioral or both?

    Seems I get a major lift at times, when a new cognitive understanding opens up. But it’s not mania, it’s refreshment from the Lord.

    At divorce and parenting plan court my x said he wasn't bipolar.. that he was diagnosed manic.. but am I understanding here that he is still diagnosed bipolar 1? The delusions.. mine is very delusional. One time he thought that I had a sniper outside ready to shoot and kill him. He pulled me over to stand in front of him. I wasn't scared because I knew there wasn't though I did question if he had one out there since sometime around this time his sister told me he plotted my death and she had to explain to him why he couldn't kill me because he felt like the fact I was his wife made him justified to take me out. He's a damned narcissist and I understand that bipolar and narcissism are often confused with one another. The thing is my x is a major drunk and loves pills and kratom. I didn't see bipolar 1. I saw an addict and one never in recovery.. not even when he was sober.. extremely entitled.. abusive.. classic narcissist.. classic malignant narcissist that's out of control. From all I understand he isn't likely going to see his kids again. His shame..not theirs and not mine..

    I have bpd, and i experience hypomania that can last up to a week or two. I also get severly depressed for months.
    In my group therpy, no one else seem to have this "trait".. could that mean that i have bipolar 2 as well as bpd, or could longer lasting hypomania occur in bpd?
