Trans migration：screw + 3D lever arm｜【Chris Chang Ortho】CC793

[case report]
In this case the patient has two impacted upper canines and one transmigrated lower canine. First, create spaces for spontaneous eruption by open coil springs. Later, perform full-thickness flap to access the transmigrated tooth and remove the bone around the crown and in the traction path with a bur. The force system generated by the 3D-lever arm and miniscrew successfully brought the tooth into occlusal surface.
這個案例中患者上顎有兩顆阻生的犬齒，下顎有一顆游離牙。首先透過open coil springs打開空間，讓牙齒自然萌發。之後再透過手術將游離牙黏上矯正器，透過3D-lever arm和橡皮筋連結骨釘，成功將牙齒拉至咬合面。

That’s wild. Should we take care of the mental nerve when flapping? How to avoid the adjacent teeth root resorption?

1. The mental foramen is located between the two premolars. So no need to worry about damaging the mental nerve.
2. During the traction of the impacted canines, the main arch wires were not engaged, so it kept the teeth as free body to avoid root resorption of the adjacent teeth.
