Once the dudes in Midway Chicago(Ed Boon, John Tobias, Eugine Jarvis, etc.) rose up, they were alongside Rockstar, id, and 3D Realms as the bloody, edgy, over-the-top arcade game company your parents and authority figures hated, and tweens and teens subsequently loved as a result. Smash TV, Narc, Total Carnage, MK, NBA Jam, NFL Blitz, WWF the arcade game, Rampage, Carnevil, Ready 2 Rumble, Gauntlet Legends/Dark Legacy, Area 51 (both the arcade one and the console FPS featuring MARILYN MANSON)... all violent, bright, and/or crazy arcade games, and many of them sparking controversy as a result. It's still a shame to me that they went under. Warner and EA don't have the balls to live up to their rep outside of MK.
I think the reason why MK fatalities are easier to look at is because they're intentionally made to be outlandish and unrealistic. no one could pull out your blood, freeze it in mid air, and stab you with it. meanwhile the injuries in this game are frighteningly realistic
It really does make a world of difference. When Johnny Cage gets his leg broken he just gets up like nothing happened. But the players here have to slowly limp off of the field. It's legitimately disturbing.
