My view on the song is that it is about the social media. They call him the sick boy because everything he posts show how sick he is (how awesome he is). But he also states that you have to watch out because we are very narcissistic when it comes to social media. We show only the best of us and how good and beautiful our life is while we don't get to see the other side of it. (where lies are turned into fairydust). It is not always as beautyfull as we show it to the world. and the ones that are called the sick boys (the popular ones on social media) start to notice this once they start to get recognized. Also once you are being called a 'sick boy' there is no escaping from it. no matter where you go (And we can pick sides, but this is us, this is us). Because they are called these 'sick boys' everybody is somehow forcing them to continue to show their awesome lifes wich can take a high price to pay from them. They start living in the way everybody wants them to live and become alienated from who they really are/were. They don't recognize themselves anymore after a while and that is why their inner personalities feel sad. (They live in a prison that they made themselves)

Are you familiar with Sleeping at Last? I think he has some of the most inspiring poetic lyrics of our generation. You would have a field day with any of his songs. In my opinion, they are two great (or at least pretty good) music producers/djs that are now finding a way to express something through their music. I can't deny that it sounds a lot like blurryface or heathens, but instead of taking that as a "copy" I preffer taking it as an "inspiration". What I mean is that it appears that tøp has like opened a creak in the pop hits industry and the chainsmokers are like some of the first to see that creak and wanting to go through it. Maybe Andrew isn't an awesome songwriter yet, but I'll admit that he's being really brave by trying to be one and still making the music he likes (or them like). I really hope that my theory is accurate because I would LOVE to see more even better songs like this one, i'll admit that I like bravery in music when talking about something that really matters to you about what you do, and I think that this song is a great breakthrough for this guy
My two cents: He comes from the East side but lives in the West and he's learned some bad habits from both. I think he recognizes fame requires him to be prideful and lie and he's not especially happy with these aspects of his character. As an industry insider he's telling us we shouldn't believe his public face or "the narcissism," because he's "spinning lies into fairy dust," just as he knows others are. If he's acting, he's not being his true self - which means he is representative of the problem with social media or "the sick boy." In other words, he's sick with amorality. The risk he's taking is in pointing out to you that you're sick too. He's trying to wake you up. Many of us view social media as problematic but do nothing to change our habits or work around using it. This is our "indifference." "Feed yourself with my life's work / How many likes is my life worth" is repetitious and representative of the cycle of social media that he's caught up in and can't escape from. In summary, he's the sick boy because as someone famous he's more likely to be our scapegoat. He's the reason we gives ourselves for forming our own bad habits and not being our best selves - when in reality we're all sick and drowning in social media together.

Conversely, they might be capitalizing on common themes anyone with a social media following in the digital age. I didn't like this song particularly, but I like to analyze things. They have mastered the formula for Pop music success, but they will not achieve true artist status until they actually voice what they feel in an authentic (rather than easily relatable) sense.
