I thought I'd share my one medical experience in Thailand, although it's a few years old. Prices are up everywhere, I'm sure, but I expect the general relationship still holds.
Back in October 2016 I experienced severe chest pains. I went to an Urgent Care facility here in the USA where I was seen by a Nurse Practitioner. She pointed out the red rash on my chest, which I hadn't noticed, and diagnosed Shingles. The bill submitted to my insurance company was $350. The insurance company allowed $202, which I had to pay since I had a high-deductible plan.
Two weeks later I had a follow-up visit to my family doctor's practice where I was again seen by a Nurse Practitioner. The bill submitted to insurance was $144. (doctor office visits are typically less expensive than Urgent Care, which in turn are less expensive than a hospital Emergency Room) The insurance company allowed $55, which I had to pay.
A year later, October 2017, my wife and I were in Chiang Mai and I experienced chest pains that I quickly recognized from the year before. My wife took me to a hospital where we went to a floor offering services similar to the American Urgent Care facilities. Not the the hospital's Emergency Room. Here I was seen by a Doctor. I was charged 400 baht for the doctor and 96 baht for the hospital, total of 496 baht = ~$15 at 33 ThB/$. I didn't bother sending the bill to insurance.
Yes, the Thai health system costs are a fraction of the US.
Retired expat from the US living in Thailand 4 1/2years. I'm on an OA visa, which I got before I came. The govt just raised the insurance requirement for this visa to 3 million Baht coverage from 400k. If you're going to retire try to get an O visa, no insurance requirement. Talk to an expert like the fellow in this video.

The doctors and healthcare depends on where you live in Thailand. So please state that.
Health insurance? Private or Public hospital both have the same good through services.
The difference is "Cost" and conveniency where private hospitals don't have a long queue. Beside majority of the doctors in the private hospitals are the ones that employed in public ones.
Having said that you need to show them you have the money to pay on any big operation for both public and private.
Having a health insurance is a good thing if you can afford it but that might not be a necessity. Health care Cost is not that high but in relative to a living standard.
