April is rude but she's also right. Emily needs professional help. It's not about choosing your gf vs your sister, it's about recognizing the self-destructive pattern of somebody you've been trying to help for years and recognizing you can't fix everything. That doesn't mean you'll abandon somebody, Ethan can still visit her regularly without taking full responsibility of her well-being and mental health 24/7.

“She is damaged, you can’t fix her” I’ve never heard a statement that is more counterproductive to the concept of trauma than that one right there.
She was only saying that Ethan can’t fix her. Remember that the sentence before she advocates for getting the girl professional help.

He didn't put his sister first at all. They are both medical professionals and both did their job...also you cannot pick your family but you DO PICK who you keep around you. If anything the girlfriend is the one who should be "chosen" . Regardless..he didn't choose her. He just did his job

Sometimes you can not help family. They need someone they are not related to help them. I'm not saying cut her off completely, but this family's dynamics isn't going to help this girl.
I was an addict. Sober for 5 years because my family didn't give up on me. Addicts are sick, not evil. And just turning her away is the opposite of what a health care professional is supposed to do.
I get that, but I can also understand the perspective of the family. Addiction is an illness and makes people resort to the worst habits, even to the point of hurting loved ones. Families aren't "right" for giving up on addict family members, but it's still a very valid and understandable reaction.
Luckily, I've never gone through it, but I think people who overcome addiction are some of the strongest in this world. In that note, so happy to hear you're doing great! 🥺 Congrats 🙌🏼

I understand April's meaning and intentions, all valid, but she could've worded it a lot better (yes i know it's a show...). Ethan was likely overwhelmed by guilt and in full on sibling mode so his first thought is "i'm responsible for all this" even if he isn't. Emily needs professional help, as we all can tell, and she isn't going to get it by staying with Ethan (not that he'd be around often enough to watch her), Ethan is an ED doc not a rehab/psych expert. April's points were valid that Emily messed up and put their jobs/lives at risk, but she could've worded it more as "you (Ethan) feel bad but you're not the help she needs" cause Ethan hovering over Emily is just gonna feed his own guilt instead of fixing the actual problem.
I’m a sibling and I adore my brother. We have both gone through rough times. But no matter what I will always have his back as I know he has mine.

The sister needs to get herself into therapy, and rehab. What her brother is doing is just enabling her behaviour, and she will repeat it. April saw that, she tried to tell Ethan that (even if she is super blunt about it), but the cycle will repeat, and it's sad that Ethan doesn't see that. Btw, this isn't about not helping Ethan and his family, April saw a pattern, and she tried to get Ethan to realize the real issue.
Unfortunately, people lose any objectivity where their loved ones are concerned because had Emily been a stranger, he would have agreed with April!
Totally right, people need to realize when someone from the outside sees it differently and tells you what's up you need to listen.
Except at that moment when he saw his sister almost dying and alone, April should have known that it was not the time for such talk. she needed to wait after the sister was ok.

You can't give up on your family, no matter HOW tempting they make it. In some cases, be better than them. But that also means realizing when you can't help them by yourself.
Sometimes family can not be the ones helping. They just make it worse. I'm not saying cut her off, but he needs to not be the main one helping her. Their family dynamic is broken and not helpful.

I can understand that Emily did wrong things in the past, but that doesn't mean you should not help her when she is homeless and has her life at risk. As Dr Manning said to Laura "You have hepatitis, you could die." Then April makes things worse by trying to convince Ethan to push Emily away, ultimately choose her over his own sister.

That's not the point. April love Ethan, so much, to the point that she doesn't want to see him destroyed because of his sister horrible choices. If he really cared about her he would make sure she goes to therapy, because otherwise he can't help her and she will probably die from the drugs sooner or later. And that is what April wanted him to do, when she was talking about group homes and about Dr Charles.
And by the way family comes only first, when all the members live by the same rule. When you have got a single family member abusing this mentality and not pulling their own weight, the family will break apart. Which is what East Wind was talking about

This is common because child protective services, like every public help service, in the US are rubbish and dont work well. Everything that is government funded is substandard over there. CPS place children in group homes or foster families where they are abused and neglected to a point where they run away and end up on the street and they are not even looked for properly by the police after, sometimes not even reported missing. Maybe they are left with their parents in abusive, horrible homes instead and end up running away anyway. I live in germany and I saw a group home once. It had a garden, a swimming pool and and indoor gym. And the children had extracullicural activities planned, schooling and everything they needed in there. Also foster families are monitored and scrutinized all the time to make sure the children are treated properly and if the children show the minimal sign of discomfort investigations are done immediately. The US is quite scary if you ask me.
This is mad in the uk if your under 18 your immediately put as high risk and into emergency accommodation for as long as your in education (thats how it worked from where im from in the uk. Im lucky to of never experienced homelessness but some of my friends did and they said thats what happened)

She was homeless and sick, people only improve when you give them external love and support how can you expect someone to get better without it, i mean do you honestly think that she has a better chance of getting therapy and being sober when she’s living on the street or when her brother is helping her through it
If your partner can't accept your family, than they're not worth it. No matter what they do, you never give up on family, they should always come first!
Family can often be what actually hurts drug addicted people the most. They don't help the situation at all. That's what April is trying to tell him, though very poorly.

Many of you in the comments sound so irrational. There's a reason why places like rehab exists. You're not any of your family member's therapist. April was right to recommend rehab and to not enable both Ethan and Emily.
She's actually correct, but her timing sucked... There's better way and time to tell ethan that, not in a time of crisis... She should've wait for the perfect time when everything was calm and better... At that time she was only thinking for herself because she wants Ethan's sister to get sent away immediately. You know she has feelings against Ethan's sister. If she really has nothing against Ethan's sister, she should've wait and has a better approach than what she's doing in that time.... You know Ethan was a good brother because he was thinking how to help her sister's hepatitis first than getting her into rehab, if she even cared about Ethan's sister she would also think that way. Picking up homeless people who are very sick and need to be admitted into the hospital for treatment? It does not happen in the USA.
Hospitals are obligated to take in patients who walk into Emergency Room, stabilize those patients and set them free once they are stabilized.
But they don't send out doctors to find these patients. Patients who can't pay.
