Both test (Forced Valgus - and forced Varus) are made with external rotation because this increase tension in the colateral ligaments (Medial and lateral). While for the internal rotation the cruciate ligaments makes the "stop" (End Fell).
Hi Pablo, you don't necessarily want maximal tension in the collateral ligaments. To stress them and to test for pain it's okay, but not if you want to feel for the degree of joint gapping. For the same reason, you don't do the Lachman test or anterior drawer test in maximal internal rotation to test for the ACL.
i got MCL injury.. its been 3-4 months .. i can walk and do normal things all fine..but when i try agility traning, squats, bend my knees and walk for a long time it pains.plz help me

Does any knowledgeable person have any information about new medical technology to repair torn pcl knee ligaments and osteoarthritis. I have heard there are new and exciting things being done with stem cells, spider silk, and new 3D cat scans.
The Valgus stress test is usually done in 30° of flexion for the anteromedial part of the capsule and the superficial MCL and in extension for the posteromedial corner which is basically the same as abduction.
Why should there be an exorotation when performing the valgus and varus stress-test? We are tought to do it without a rotation. As the Lig. collaterale laterale + mediale run more or less vertical, I can not see any reason to rotate the tibia. At least no reason to specifically exorotate...
Hi Robin. The collaterals don't run vertical. The MCL runs from dorsal on the femur to ventral on the tibia and the LCL runs from ventral on the femur to dorsal on the tibia - same reason why both ligaments limit external rotation of the tibia. Therefore, externally rotation the tibia puts more tension/stress on the collateral ligaments.
Physiotutors i know this. But to perform in exam, i wanted to see how to switch hands and i dont find it smooth. Please post link of any video that shows it, valgus on both sides of each leg and varus too
No, it's not palpating the MCL but assessing the gapping you feel (+pain provocation in more acute situations) through forcing the knee into a valgus position

Why do you assess in full extension and 20-30 degrees of flexion?
Full extension mainly tests the structure of the posteromedial complex (PMC), while 20-30° of flexion+valgus puts more direct stress on the MCL

You did valgus in right leg only and varus in left only, I was looking each for opposite side as well
