    The lifestyle in Pattaya can be absolutely madness. I been on holidays for the last 2 weeks out there and I think is not easy whatsoever to manage a healthy lifestyle. All the drinking! The women and the lack of sleep..! Actually I’m looking forward to coming back home. Watch Netflix and drink a cup of tea. I hope everything goes well Webby.

    Drinking might not be the direct cause but it will be affecting your health indirectly through poor lifestyle choices due to the drinking. Poor diet and sleep etc. Wishing you a speedy recovery

      Hi mate, you are 100% correct, I've been on warfarin since 2005 so I know. Alcohol thins your blood, vitamin k has the opposite effect. If you want any advice on how to control your INR, feel free to contact me. Cheers

      ​@@welshboywebbyWebby a glass of red or equivalent DOES actually help thin the blood that's true, but heavy drinking definitely doesn't its actually much more likely you'll have a stroke if you're drinking heavily.
      You're just completely wrong about this sorry.

      Research suggests that in low to moderate amounts, alcohol may have blood-thinning effects due to it reducing platelet function. However, higher amounts of alcohol consumption may have the opposite effect and increase the risk of blood clotting.

    INR is international normalised ratio. Its actually the PT or prothrombin time that is measured. Different labs have different normal ranges so its converted to INR so it is comparable between all labs. It should be 1.0 normally. PT around 11 to 13 secs. If you have a clotting tendency then you can reduce the risk of a clot by extending the PT. The target value or range will depend on the underlying condition. The dose needed to maintain your chosen target range will differ from person to person and needs to be established by trial and error. Too much and the danger becomes a bleed instead of a clot. If on warfarin then alcohol may increase the PT making a bleed more likely rather than a clot. Go on something like apixaban instead as it wont be affected so much by alcohol. Hope this helps and dont worry. This is all very basic stuff anywhere in the world.

      My mother was on the old rat poison. One thing I did notice was that changes to other drug doses had a major impact on clotting times.
      Whenever any other drug was changed, it was important to get clotting times retested.
      My current understanding is that they don't really use warfarin, particularly for new patients. It's other drugs first to see how they work.

      @adenwellsmith6908 you are a bit behind the loop. He is on rivaroxaban not warfarin. Apparently they think it's not working as his INR is 0.9. This clearly shows that they do not know how this drug works as INR is completely irrelevant. Apparently their aim is to increase his INR while on this antiFXa medication. This is completely crazy not to mention dangerous. He doesn't want to hear this or other wannabe doctor comments. Wishing you well Webby.

    The INR blood test is for Warfarin. The test shows 0.9. So, your dose needs to go higher over a number of days to get your INR back to between 2 and 3. You dont have a clot but your at higher risk of developing one if you ignore. Unchecked, your more at risk of other things like bleeding to death if your not stable and you dont get your bloods checked regularly. Most of the blood clinics/labs in Pattaya should be able to test your INR for a few hundred bart and guide you on the correct dose. Alcohol does have effects on INR bloods. So, does some foods and things like Antibiotics. I,ve been taking Warfarin for 22 years after having a heart attack at 37. Soon to have a pacemaker fitted. Then, hoping to be in Pattaya.

      Also be aware that if the INR is less than 1.0 this may indicate a poorly taken sample. If the blood is not mixed with the anticoagulant in the bottle properly it will start to clot in the bottle consuming the clotting factors you are trying to measure . I would generally ask for a repeat sample on an INR of 0.9 from a patient on anticoagulants as it is does not make sense.

      It causes high blood pressure.Ive been on BP tablets for the last 30 odd years or so and has enabled me to carry on drinking beer.
      I'm 70 now.

    The new DOAC (direct oral anti coagulant) medication is brilliant. It has virtually replaced warfarin. You only need your blood checking twice a year. They are very expensive though. Apixaban cost roughly 4000B a month from a pharmacy - more from a hospital. Not great with large amounts of alcohol but fine with moderate drinking. But if a risk of bleeds though so not all plane sailing. Anyway best of luck.

    Hey webby , nurse from Sydney here. Are you on warfarin ? If you are you need regular bloods for INR and the doctors adjust the dose according to blood results, which is easily treated in Thailand. Here in Australia we don’t do warfarin too much anymore as that medication requires regular bloods and dose changes, most people recovering from clots are given Apixaben as it does not require constant blood tests and dose changes. While each case is different and following your doctors advice is important just be aware if your on warfarin you need constant bloods to be able to adjust your dose according to INR results , good luck buddy.

      You are correct. INR is irrelevant for rivaroxaban yet 0.9 result has made him panic. It means nothing in this context.

    I’ve been on warfarin for since I had my heart valve replaced when I was just 34,I ’m 47 now. You if your blood is too thick, you’ll have to stay away from food that are high in vitamin K. Vitamin K is a naturally thickens the blood. As soon as you get your INR to balance out, you will have to stick to a diet and exercise. That is pretty much the same day today. The doctors will adjust your warfarin intake based off of your diet in the amount of vitamin K you consume. This does not mean you have to stop eating foods that are high in vitamin K, for example, leafy greens are high in vitamin K. You will just have to consume the same amount of vitamin K on a weekly basis and that will help the warfarin balance out your INR.

    The best thing you could do webby is start introducing fasting into your lifestyle.
    Try and do water and OMAD fast with nutrient supplements. It will lower your cholesterol and give your body a well needed rest.
    1 week every month and you will see and feel the benefits. You’ll save a fortune too.

    For normal patients not on anticoagulants the normal INR is usually 1.0
    For patients on anticoagulants the INR ranges between 2.0 - 3.0.
    Have a look online , at recommended foods .
    Changing your diet can bring yout INR up .
    Can you consume alcohol while on anticoagulants, yes and no . Depends on the brand on medication

    Go to Bumrungrad in Bangkok. One of the reasons I'm considering retiring to Thailand is what I've heard about that hospital. I use one of the best hospitals in NYC, and Bumrungrad is the only hospital in Thailand I would consider coming anywhere close to that.

    You need to have your INR checked very regular, on Warfarin it starts weekly then fortnightly anf if you INR is stable between 2 and 3 then you will still need monthly INR checks

      I was on warfarin but now take riverroxaban you only need your blood checked once a year and you are on same dose all the while no different gram tablets.

    How can I increase my INR naturally?
    Those which increase INR values:
    Boldo and fenugreek.
    Cranberry juice (in large quantities)
    Danshen.
    Devil's Claw.
    Dong Quai.
    Garlic capsules.
    Ginger.
    Ginko biloba.

      Cranberry juice to be completely avoided if taking Warfarin. You dont need much to mess with INR and the ability to clot

      INR is a derived ratio used exclusively to monitor and dose warfarin levels. Diet can interfere with long term anticoagulants but to say you can increase it naturally is misleading as warfarin is rat poisin . Nothing natural about it.
      Some foods can have a natural anticoagulant effect, but the point is to maintain a stable target prothrombin time. Not possible with diet.

    I had my aortic valve replaced and put on warfarin for several months, I had to get blood levels checked every week. I hated that drug, everything you eat and drink can affect the inr levels. Currently on plavic due to a stroke from the operation. They have home monitors to check inr levels

    Inr is regulated by taking warfarin,I go have a blood test and self medicate also have 1 tiny aspirin a day.ive done this for years and learned how to do by my doctor in the Uk.i have 3 stents and a pacemaker and feel 100% with the use of blood pressure medication.

      @welshboywebby all medicines have a detailed product information sheet. The rivaroxaban one says it can't be monitored using the INR. Why are they testing your INR if your not on Warfarin? I would be happy to have an informal chat over a coffee if you think it would be useful. I'm an ex biomedical scientist who has worked in specialist coagulation labs in the UK.

    INR is only used to adjust warfarin levels, its not a serious thing people on thinners need to adjust often. It can change due to small diet/lifestyle changes

    Anyone on blood thinners would be told that drinking will kill the effects of the medication. Blood thinners and alcohol don't mix It's a fact. Keep drinking and your clots coming back simple.

    I’ve had about 6 clots now, in my leg and lung, it’s very scary , but I’ve never had one in thailand lucky, even after doing the long haul flight about 50 times, but Thailand does have some amazing hospitals , should be able to sort you out no problem, good luck

    I have been in Pattaya on many occasions for longish stays as it is hot I eat many salads and raw veg. I avoid cooking as I do enough of that at home. My health if anything improves well my cholesterol does. But then I am a teetotaller always have been always will be and have never smoked or done any type of mind altering substances.

    the only thing i can think of by what you say is,you should be on Warfarin,if you are,it needs to be increased,but in the meantime,maybe you should be in hospital now getting the Warfarin by infusion,all the very best mate

    Thanks for the info on your experiences with hospital visits especially costs. Have you had quotes for health insurance in the past? Was it too expensive with pre existing conditions? I’m wondering if it’s worth paying up front for insurance or pay as you go but I would want to go private. My fella and I are emigrating to Thailand next year. He’s had his wild times in Thailand when he was younger so now it’s my turn 🤣 only joking! We have a great life together but don’t want to waste the winters hibernating in the UK. We are checking out Koh Samui on our next trip. Sounds like the hospitals are good there too. Prevention is definitely better than cure everything in moderation so they say. Hope you get sorted soon.

    Webby I admire your faith in consultants but on many occasions a medic has requested I perform an INR. Good lab practice is if they have not indicated the patient is on warfarin we phone and ask. If not on warfarin the lab should refuse to report the INR as it is not relevant. They don't know everything and very often don't know the mode of action of modern anticoagulants. It's very frustrating for the coagulation biomedical scientist who has to argue with them. Keep well mate.

      Monitoring is by antiXa activity in test plasma compered to a rivaroxaban standard. Not a routine test and very few people trained to do it. (from someone who has done it. Doctors would not have a clue how to do this test).INR is inconclusive because it is completely irrelevant. That lovely doctor is a concern.

    Them repeatedly asking you about HIV tests make me think HIV is common in Pattaya and they think you may have it. Fingers crossed for the tests to be cleared and some changes to your prescription will do the trick.

    I have to say that your a fool to travel to Thailand without Travel insurance , I will reference Kev in Thailand, I don't care how much travel insurance is , I will make sure that i pay for it and to declare all conditions , Stroke , Blood pressure and other things . It doesn't matter how much money you put to one side, It may not be enough But Good luck

      Webby it wasn't clear you were on rivaroxaban. INR is only for warfarin hence natural to assume you were on that. Strange they think INR is relevant to an antiXa inhibitor.
