Dr. Bauer is a board certified veterinary surgeon and a Diplomate of the American College of Veterinary Surgeons. He shares his thoughts on canine Anterior Cruciate Ligament (ACL) repair and TPLO. Dr. Bauer has performed thousands of canine ACL surgeries.
The two current techniques addressing the underlying biomechanical instability in a canine ACL tear are the tibial plateau leveling osteotomy (TPLO) and the tibial tuberosity advancement (TTA). The TPLO, which changes the angle of the tibial plateau, has been popular for about nearly two decades and has a high success rate (about 95% of dogs return to 95-100% of normal). The TTA is a technique that changes the relationship of the patellar tendon and tibial plateau angle via an osteotomy (bone cut).
Non-surgical treatments are usually unsuccessful in canine ACL tears due to the chronic biomechanical stress. Surgical options are broken down into replacement techniques and biomechanical repairs. Fishing line used to replace a torn ACL has been used for decades and in our opinion is an antiquated technique. More recently better material and placement techniques have been proposed. This newer replacement technique is called the Tightrope Technique.

If a large dog has an old injury and already has arthritis in the knee, do you still recommend surgery? I would worry the arthritis would interfere with rehab/diminish benefit of repair. Also, are there any situations where a brace + medical management are preferable?

Turmeric Curcumin is the best thing for Dogs with joint problems. I have given my dog it for a few years. Without it she cant walk on one leg due to knee problems. It takes 4 to 8 weeks to start working. I give her 1 pill a day of Turmeric Curcumin with BioPerine and Ginger for Anti-inflammatory. It has to be 95% Curcuminoids, very important. The best one is Dr. Matthew's. It's $20 for a 2 month supply. Just 1 pill a day not 2.
