    Can you do a video on how you organize a night shift, including some tips for those of us who have 5-6 patients and how to handle admissions and discharges throughout the shift without messing up your time management.

I wasn't quite sure on how the organization of the day would be and what to look for in the day. This may sound silly, but its true. Where were you when I started working as a nurse.....I was all over the place when I started, & no real guidance baptized by fire they call it...what a mess and since then I feel I can't get it together ppl think I am organized but I tell the 'don't let me fool you!' I may.look it but I am not. I am now a visiting nurse working in Adult Foster Care. I would love to write you more. And another thing in the 2 videos I have watched, you are realistic, you don't pretend to know it all but make sure you have the tools to reform yourself and your patients to show your competency and that you are an efficient and diligent person and nurse! You go girl!!!

    Thank you! I'm in week 3 of my orientation as an LPN (1.5 semesters left of my ADN) and I've basically been thrown onto my post-op unit to care for four to five patients at a time. I've been drowning. I've tried to develop a report sheet, I try to stay on top of my patients but I feel like I'm just treading water, trying to stay afloat. Thanks for some great tips. The only tips I've really gotten from my preceptors are "You don't need to chart all that" and "Delegate!" I'm trying your method next shift to see if I can get head-to-toes done and meds passed more efficiently, and thus make my day flow more smoothly. Thanks!

    Love this video! Even following a nurse around and working with them in clinicals, it's not easy to figure out how they organize their shift. I don't know how the nurses who delay charting do it! I'd be terrified to forget something! :-/ It sounds like you are going in a constant circuit. first asssessments then meds, then vitals, dressings etc.

    thank you or telling you patients what the meds are used for. Everytime I went to my doctor the "nurse" would say are you still on such and such or itching and I would say I wasnt prescribed anything for itching. So last time I went I asked the DR if he was talking about my anxiety med he was like oh yes its for itching to and preceded to act like i was stupid i informed him NOBODY mentioned any other uses for the med. :rolls eyes:

    I loved this! Question: do you ever have time to eat or drink water? That sounded like you had no time to breath. I understand no lunch break but is a 5 minute break to eat a fast meal an option? My health is very important to me which is why I ask. Thank you for your great videos!

    Any tips for staying organized with medications due? Also, what is best way to understand the plan of care for pt for the day?

      I created a chart with boxes to just keep track of when I have meds due. For care plans, I review progress notes from the last couple of days to understand what the overall plan is.

    Hi Nacole thank you so much for your videos! I have just started as a graduate nurse in ICU and am struggling with time management skills! Especially the day shift with trying to fit in a wash, meds, ½ hourly obs and also keeping up with the many doctors rounds to listen in and clarify orders! I am having trouble with trying to prioritizing interventions and care. When I get behind I start to panic and feel incompetent as a nurse. I also find it difficult, as I cannot leave my patient without a nurse watching them in order to gather supplies of linen, medications, disposable bedpans etc. Do you have any advice from your experience as nurse? Thank you for you help ☺

      How many times of you clarify orders a day? When you go in for your wash, is it DURING your hourly obs? Are you giving meds DURING your hourly obs? Since you have soo many things to do, you have to cluster were you can. Going in a room five times in 10 minutes isn't good. I'll do a video for you next week about this okay?

      ***** with clarifying orders i mean when the doctors decide to change a medication, or start a new infusion or plan a procedure and if I'm not there listening i don't find out until i read the notes and usually i don't have time to sit down and read the notes! I'm just so paranoid that I'm going to miss something really important! I've become better at clustering care for e.g. if someone is on 3 hourly neurological obs i try to do the wash on the hour its due for better assessment. thank you for your time!

      Priscilla Robinson I make a note to check my charting for MD orders every two hours. I make it apart of my "things to do" because nothing is worse than getting ready to clock out and finding 5 new orders you haven't done and never knew about. You could try that maybe...     Do you take your clip board into the patients room? I think about all the germs in the room, we can’t put it on the patients bed and the table is probably not clean or full of “stuff”. I like using a clipboard also, but I find it gets in the way when going into patients room. But it has all the information on it I need.

    How many minutes do you spend in each patient's room for the head-to-toe assessments in the morning?

      As a bedside nurse 10-15 mins per assessment, as a nurse practitioner 15-30 mins per assessment, and that includes conversation.

      I work nights, I do it before midnight or before day shift arrives 6AM.

    Hello NurseNacole, I think I am confused here a little (sorry new nurse literally) you said you do all your HTH assessments first than meds. If you have 4 patients than how does that work with meds if it takes around 4 hours for your HTH? Sorry for a dumb question I haven't found job yet but I will eventually, LOL.

    Hi there! I am a senior year nursing student. I love your videos.
    Question...and it might sound a little silly, but do all physicians round when they want to? I am having a hard time keeping track of physician rounding and updates. Some of them are very complicated. They neglect to keep nurses updated with changes, etc.
    So how exactly do you determine if a physician will be rounding or if you should page/call them?

      They round when they want to BUT I hold onto the chart so they HAVE to see me before they write their progress note. Nurses are too buy to me running around people, they way… they have to see me haha. Rounding is confirmed with a progress note in the chart.

      We use EPIC. Not sure if that made it's way to your hospital yet but the physicians chart all their things on the computerized chart. It's convenient because we don't have to make out chicken scratch, but it makes it harder to have contact with the physicians. The whole physician-nurse relationship is mind boggling to me.
      My second dumb question.
      What did you have to learn about the physician-nurse relationship as a new nurse. Was anything new? I feel so unready haha

      We are phasing out verbal orders but from time to time we do process them BUT they are few and far between. My advice is respect everyone, clarify, clarify, clarify and if you don't remember ASK again haha. I'll do a video for you tomorrow : )

      I meant when you will have your break/lunch. From your video, it seemed you didn't mention it rather than a short bathroom break. So I wonder.
      Also, as a nursing student, I learned to plan ahead, but how much can I plan for each patient? I realized that I would miss some tasks if I didn't put them on the paper, or if something came up and I didn't mark down. So does it mean I have constantly add new tasks throughout the day? What are your strategies? Thank you so much!!!!
