Clinical Perspectives in Preparing and Managing High Dose Psilocybin Sessions: Insights from Johns Hopkins
Brian D. Richards, PsyD and Mary P. Cosimano, MSW
This presentation will focus on difficulties occasionally encountered during conduct of the Johns Hopkins psilocybin studies in healthy volunteers and in psychologically distressed cancer patients. Procedures will be discussed which ensure adequate volunteer screening, preparation, management, and aftercare for high dose psilocybin sessions. We will also discuss our experience as primary guides in the psilocybin studies including techniques we have found helpful in supporting volunteers during challenging states of consciousness occasioned by high dose psilocybin. Case descriptions of some of these difficult sessions and follow-up will be presented.
Brian D Richards, PsyD is a Licensed Clinical Psychologist and Clinical Director and Education Center Coordinator for MedOptions, the largest regional provider of behavioral health services in southern New England and the Mid-Atlantic. Dr. Richards is also Faculty/Affiliated Investigator at the Johns Hopkins University School of Medicine, where he works on States of Consciousness Research at the Behavioral Pharmacology Research Unit, Bayview Medical Campus. He specializes in acute outpatient care, diagnostic psychological testing, long-term care/rehabilitation, drug and alcohol abuse, death and dying, spirituality in end of life care, mindfulness and brain-science-based approaches to personal growth and healing, and therapeutic lifestyle changes.
Mary Cosimano, MSW, is currently with the Department of Psychiatry and Behavioral Sciences at the Johns Hopkins University School of Medicine and has served as study guide and research coordinator for the psilocybin studies for over a decade. During that time she has served as a session guide for the psilocybin studies and has conducted over 200 sessions. She has worked as a clinician teaching individual and group meditation to breast cancer patients in research at Johns Hopkins, as a behavior modification counselor for weight loss, and has 15 years of experience with direct patient care as a hospice volunteer. Ms. Cosimano has been extensively involved in all five psilocybin studies, as well as Salvia divinorum and dextromethorphan studies conducted at Johns Hopkins. She will provide her perspective as primary guide and study coordinator in the Johns Hopkins Psilocybin Studies.
I’ve never gone past 5-7 grams but I find the absolute best thing to do is fast before the experience, do lots and lots of breath work and bioenergetics beforehand and then during the experience I always feel a strong need to stretch and move my body in creative ways I feel like it releases trapped emotions and relieves tension i maybe wasn’t aware of before the shrooms set in. Definitely helps me avoid falling into negative emotional spirals while tripping
Thanks for sharing this, I will try it!
None of those things make any difference but you do you.
Sometimes falling into the so called “negative” is needed
This is such important work for one of most important aspects of our 'health care' Mental Care. I dream of the day when plant lovers can use mind medicines in supportive, guided environments, accredited by groups like MAPS.
Ayahuasca ceremonies happen in the USA and many countries perfectly legally and therapists recommend these to their patients.
Amazing talk! So good to see. No longer will the stigma and misconceptions surrounding psychedelics be enough to stymie their medicinal usage.
The fact that world governments kept this from us and continue to use this to create wepons of war is terribly horrible and they need to be held accountable
Terrence McKenna would be proud as a new father with his child ... if he was around to see what’s going on today...
McKenna's method is also known as "Five dried grams in silent darkness" :-)
😍😍😍
@@landwirtschaft2116 did it today on penis envy. 6 grams in a dark room. by myself. my intentions were to grieve the recent loss of my sister and my mother 5 years ago. my ego died. I went through hyperspace. I saw the tree of life where my mother told me I was in a place beyond time. that infinite is "the same as one or two in this place" whatever in the hell that means....basically long store short this isn't the only life. it's all just one long eternal ride into the abyss. psychedelics are our birthright. shame on any government to take that from us.
Good structured presantation about a very important subject.
IMHO Their approach is very much in line with Grof's approach for psychlytical LSD Therapy. Very good.
The only thing I consider missing in their description is a swim or a long shower after the client is coming down or "boomeranging" back in to reality.
The reconnection with water has always been a very grounding one, and seems too important therapeutically to be left out.
Maybe a consideration for further sessions if it's not used already?
When you come back, you raise up from the bed and put your feet on the ground and start to stand. That's how you ground yourself after a big trip. I support your shower technique but really standing up does the trick. 4 me I always ingesting the 5g + shrooms wait 1 hour to then go to the shower to come up actually, after naked to the bed with the lights of. Happy trip
Christoffer Thorsen look up lemon tek
@@hellacooook i did, and just like I said. It's just a theory..
Christoffer Thorsen you should try it for stronger trips
It's great that this sort of stuff is being investigated again, after all these years, but - jeez - what a bunch of stiffs! To trip with these people would really bum me out! I much prefer Tim Leary's ecstatic Trickster approach, for all it's faults! ROCK AND ROLL!!
Can you link/recommend/provide a source for that "ecstatic Trickster approach"?
At the moment i'm learning about the history of psychedelics and was about to inform myself a little bit about Timothy Leary, but i don't wanna go necessarily too much into the cult of his personality. I'm more interested in his work and how is views differ from the other scientists of his days… what you mention seems to be like the exact bit of information i'm looking for :-)
I never take less than five grams. Great presentation!
After decades in a Deadpool level where everything skipped me leaving me out. Hurricane Psilocybin was a Major Hurricane that lasted 5 hours. It was like a category 5 Hurricane. I got tore up real Severely. This should be named HURRICANE Psilocybin. this was the wildest storm and most memorable experience I have had since the early 1990s I named this Session I had Hurricane Psilocybin and it was Major cat 5 direct hit and I will NEVER forget Hurricane Psilocybin.
Yes, look up Professor David Nutt & Dr Robin Carhart-Harris at the Imperial College London. In fact, they consulted this very team at John Hopkins before starting their work.
What can we learn from studies administering high doses of psilocybin? In this video from Psychedelic Science 2013, researchers discuss their experiences as primary guides in the Johns Hopkins University psilocybin studies.
What happens when you take 50g of dry golden teachers?
@@bigmac3049 check out the late Great Baba Kalindi lyi. He has led the way with high dose enthogens. He says it's light and dark differences. You break the veil when you high dose.
Wow im so so happy !!!!!
Thanks to all this awesome people for doing this research
This is the best time to be alive!!
MDMA + UA-cam = This comment.
Kanra yep, seems so ;)
Outstanding! 'Really enjoyed this presentation. Many thanks!
Been to center of earth seen the soil made up of humanity souls, seen roots to the tree of life ,that led to the foundation stone of earth covered in roots leading up to tree of life
Thankyou for sharing your knowledge with us! You are amazing!!!
All had no prior experience with any psychedelics whatsoever.
Some people agree that good quality cannabis (thc) ,greatly enhances the positive ,mystical , artistic, meaningful, controlled attitude of this experience.helps you see through the illusion of negativity. Human personal guides might be not such a good adjunct, during this time, other people being present and focusing on the person involved can be a hurdle that might be better not to have to deal with, for some .(maybe, maybe not),No rules .
Wonderful work! Thank you
Well done.
That's next level stuff. I've done high dose lsd sessions alone. Sometimes some help would've been very nice haha.
Bring to Philadelphia! :) I know how real these tools are and how SAFE and Natural they are
@Michael Ross Thank you. I'll be looking into this
I am legally blind and I have taken mushrooms. I live in Houston Texas but I would like to participate in this study
How can I be a part of this? I would sacrifice myself for science.
I am interested in trying a heroic dose (5.5g) & planing a meditating session
I have several old friends, that will tell you how crutial, having me around was for them. One will even tell you that I saved his life. Even 20 yrs ago when I was an inexperienced explorer, I would get someone to lay on the grass and let go, reminding them that there was nothing to fear. I relate to Terrance McKenna's pal he referenced...... "just let me stand a little while longer this time"
Face the Angel and the Demon inside, for they are not separate from ourselve. We are our own Heaven and Hell. Whichever way u wanna is a daily diligence and a choice. The devil is not a bad guy outside of ourselves that we can blame all of the bad in the world on. Just like God is not in some far away sky waiting waiting to bless us for being good boys and girls. Total Complete Heaven and Total Complete Hell can be acheived all alone with not a single other person or entity around to praise or to put the blame on. Once u figure this out, You Make The Creator Smile, for he sees through your eyes and we experience the lessons and that my friends is the whole point.

really nice video :)
I’ve actually been on a trial and all the people involved had taken the medicine. 🧚🍄
I wonder how the guides deal with political revelations, like the complete absence of love and compassion in domestic and international policies, and experiential implications which occur for test subjects.
Goodness, 2013? I thought this was a 1990s presentation the PowerPoint and the fashion outfit! Great talk though
If a person in 35> years old, does the second-degree relative exclusion still apply?
9:00 seems like letting go might have helped. Amazing talk
ya, that guy was encountering his resistance. I've been there.
One of my biggest fears is having an experience like the one portrayed by the gentlemen starting at around the seven minute mark. I realize that part of the experience is integration but how the hell do you integrate something like this? To me it just seems to be chaotic, random noise. What do you learn here? I can see with some sessions there is some content of a symbolic nature which might point to some unresolved issues that need to be dealt with but it didn't seem to be the case with this session. To me psychedelics are a dual edged sword. I think we have a long way to go before we truly understand how to "tame the beast." I would love to have a dialogue with people regarding this issue in general and the trip discussed in this video in particular.
What’s considered big dose?
I'm suffering from crippling, treatment-resistant depression and this is hopeful, but I feel like I have virtually no chance of a professional trip guide since this is in the experimental phase and this deeply ill government has criminalized both the sufferer and the solution. Since depression and addiction (sober addict also) tend to isolate you and/or connect you to other sick people, the chance of finding someone trustworthy in my own community are also next to nil :(
You don't need professional guides to make a trip.. And depressions you can cure with micro dosage you won't even notice through the day
@@christofferthorsen4508 The good news is I tried microdosing on my own since my post, and it saved my life. I mean this literally. Thanks for the reply.
@@hal900x I'm glad to hear that
Seeing you posted this 2 years ago I must ask, how are you doing?
What about having one of those locks that can be unlocked from the opposite side
What's considered a high dose by MAPS in this vid, can anyone tell me please? I can't see any reference to actual amounts? Is it a Terence McKenna style 5g heroic dose... or a Kilindi Lyi serious psychonaut 25g+ dried high dose? :)
Ive heard that its 4g's dried psilocybe cubensis which is equivalent to 20-25mg of pure psilocybin, is the standard dose to bring about a "mystical experience " which has been the aspect of the experience that has been associated with healing.
25g dry?! Geeze Louise! How many tablespoons is that? 15 or so? I would think I'd vomit it all! I've taken 5g before and vomited, my best dose was 3.3g lemon tekked
5 grams really ain’t that high, people are just scared to see what’s behind that veil
I cannot help but wonder if your results are slightly tainted by only using people who fit the perfect criteria. Had you done the same with MDMA, I cant help but feel you would never of helped a single PTSD sufferer.
No, I am not slating MAPS and their work. I am just pointing out that not everyone in the world fits their regime for volunteers. Sure, I can understand their fears of "if things went wrong" with a volunteer, but sooner or later, they will need to do studies to see whether people who have suffered psychotic episodes or attempted suicide and self harm etc react in a negative or positive or indifferent manner to psychedelics. Who knows, MAPS may even be teetering on the very edge of a potential medicine for these people who currently are immediately ousted from the cut of volunteers...
These studies are done for more then 50 years dude. Ebsco for instance can satisfie your need. And in those papers they are stating specificity of patient is needed. Not every therapy fits to everyone, as you stating if i am not mistaken.
Keep in mind that this field is very precarious right now and if they have an incident it could negatively impact the entire field of study and stop 1000's of people from getting future treatment and help.
There is a place and setting for people with higher risk and this unfortunately is not it.
What if you don’t like the music?
It should’ve been changed.
The clinical approach seems a bit excessive; I've been going out to the woods to trip alone since i was a teenager, as per the ancient rite. I wonder if you have to have the guides there in the room or the music (ugh) another persons consciousness and artificial sound is such a distraction even if just a subtle knowing in the recesses of the experience, that it can be a real barrier to a break-through. If they haven't taken the trip themselves I would argue they're not qualified. Outdoors you dont have to fumble around in a bathroom or fool around with a change in lighting and toilet stuff which can be a big obstacle and break up the experience. If you're rolling around on the forest floor you just get up, hold on to a tree and go. What do they think high dose means?
Cory, what society are you from? You mentioned this is your ancient rite, I would be very interested to know?
Cory C. In my experience set and setting is specific to the individual. I myself prefer my own home alone, warm and where I know my surroundings. It provides a sense of safety security and grounding I find necessary to do self work.
I have severe chronic cluster headaches. I'm at my wit's end. How do I get to be part of a study?
UA-cam maybe the wrong place to ask. Call your psycaoetrist and ask him to sign you up, if not you go to a bether more understanding psycaoetrist and ask. Idk where you live but here in Norway we just pick our own shrooms in the fall time. If not you just grow your own.
I would wish cluster headaches on my worst enemy. I'm so Srry 4 you bro.
I recommend you buy your self a 5g with shrooms from your local hippe or buy it of the internet. Take the whole 5g and you won't have another seizure in months, find your own correct way to take it, it's no manual.. Everyone is defferent from each other
Remember most cluster headache are gut related. Intermediate fasting and keto diet that focus more on vegan life style.
Modern day Shamanism except they aren't sharing in the medicine and experience
47:00 merkabic science. someone please contact me to discuss. mind blown
At that dose Psilo has its own agenda I wouldn’t say it dangerous but once u get on that ride you can’t get off until it over 😂
I’d appreciate your comment
I bought some psilocybin mushrooms from an online supplier and it’s affordable and discreetly delivered

Extremely boring to listen to as it was describing looking after patents and no experiences, good work though would love to try it.
Is that slim shady in their core team picture
I'm telling you. There literally a way to transfer into the next realm. You have to be going in peace or you might experience hell. It opens your third eye. Jesus can protect you!
😎
Whenever you feel in a situation of " lost and no reference" try remember about ❤God❤.
Ask how is God, who is God? And let me know later😊❤ thanks
anyone have a playlist for a 'high dose psilocybin session' i could play? im planning a solo 5mg trip next week
Trip Tamine How did it go?
it was incredible, thanks for asking. to be honest i can't put much of it in to words but i am going to do a trip report on it. I was at one point flying in a circle on top of what i can only refer to as a place that seemed to be Egypt, with pyramids and sand and i had this overwhelming feeling that i was 'in charge' of looking after this place and the responsibility was SO SO overwhelming that i began to cry. it was an insane trip most of it probably won't make sense if i try to explain it
Wow, sounds intense. Would be interested in reading that trip report. Glad you had an incredible time.
***** man it was amazing. i was worried about doing it alone too as i don't like sitters, and to be honest i don't know anyone that would come and sit sober watching me trip. but at the end once i took the mushrooms i realised there was nothing to fear and worry about. have u taken shrooms before? Try a small dose on your own, around 1.8 to 2 grams, get a feel for it and then wait a couple weeks and go for the big dose, thats what i did and had an amazing experience both times
I'm so sorry to hear that :( i can't even imagine how hard life must have been. your on the right path though, i have seen amazing results from people with problems like you.
I hope you find the light at the end of the tunnel, its there trust me xx
Did I miss somewhere where it said how high the dose was?
The Johns Hopkins standard is five grams.
30mg/70kg body weight
30mg on 70 kg. It's "about" 3 Grams of cubensis in potensi
This is the same MAPS that enabled rapists and abusers as therapists by the way.
😘
@Michael Ross 🌸
12:05 Well then, you rule out 75% of your test subjects. Speaks so highly of what this was supposed to be about: helping people.
First stem Korach 2013
Go into nature,or the beach,or bushwalk may better than to lay back down on couch, maybe, maybe not ,more than likely, though.
trip on your own to heal. trip sitters are just voyeuristic ego pumpers.
Hello back hole :)
This guy lost credibilaty to me as soon as he said they did not include test subject with current depression, depression is a biproduct of feeling emotions intensely, (empathy, many people see through the veil of society.this can be painful.long dark night of the soul.psilocybin helped me work through my existential anxiety
Objectively, some poor souls, you might say would have definitely been better off in their lives without ever having taken these substances.
