i can say for sure that you don't get a dark level if you're fast enough, but im unsure on restless dead.

In the options you can set it to always be a certain one or to randomise between the three

Last year or so a glitch was discovered where if you use the camera on Yama in his Eggplant form, the sprite gets messed up and he turns into a camel-mummy monster. I kept the robot with me in case I discovered a camera on the way!

They make you get stuck on walls, This could result in death or otherwise avoidable damage.

only if you also go to the mothership afterwards. the maoi will always be in the level after the mothership if you visit both special levels in the ice caves

in Spelunky HD, you can turn the boss in Hell into an eggplant and instantly kill them. It's very impractical and is just a challenge run

What does "max eggplant" mean?

On any category where I need to deal with a hired hand, I don't want the crysknife because I could accidentally kill them with it.
"Max" means the extra challenge of going through both the worm and the mothership.
