what is the patho reason why the QRS complex is wide

Can't PVCs have retrograde P waves? If I see wide complexes with retrograde P waves, how can I be sure those are PVCs as opposed to Junctional beats with aberrant conduction to the ventricles??

Can you please verify why a patient with an Idioventricular rhythm with a rate of 35 would NOT require treatment?? Not many people perfuse well with a pulse of 35.
She didn’t write the name of the rythm
