I had my gastric bypass 22 years ago. I had one of the first lapriscop surgeries in my area. San Francisco CA. I went from 238 to 135. My PCOS went dormant and i was able to bring 2 pregnancys to full turm.
@j2i8h Hi, yes, the pregnancy weight gain was not an issue. I had my daughter 2 years after gastric bypass. I gained 23 pounds with her pregnancy, and 2 days after delivering her, i had lost 20 pounds from the delivery. I only had 3 pounds remaining. With my son, he was born 6 years after my bypass, and i gained 25 pounds with his pregnancy and also dropped 20 a couple of days after birth. I regained 43 pounds of the 104 that I had lost 40%...13 years after gastric bypass when I turned 40 and had a car accident that left me with a brain injury. I was not able to exercise for 3 years. It affected my pituitary, and I also entered primerose, which makes it harder to take weight off.
I had bypass 2 months ago because history of acid reflux and I have Barrett esophagus. I’m losing weight, sleep apnea is gone, no more high blood pressure, cholesterol is normal and I have not had any migraines in almost 2 months. Feeling great
@soniahunt7913 Hi there! I have been looking into gastric sleeve and a big worry I had is not being able to have nsaids as I get migraines and aspirin doesn't touch them. Have you had issues with migraines in the past and how has your journey affected them?
@@ashley47448 honestly I used to get really bad migraines and the only thing that helped me was excedrin however now that I lost over 50 lbs I’ve had less migraines and my surgeon referred me to a migraine specialist and they gave me a different type is medication that I am able to take in case I get one. I had the gastric bypass surgery 5/20/2021. I was dx with uncontrolled diabetes type 2, HTN, sleep apnea, neuropathy, asthma-my highest A1C was 16. My highest weight gain 302. I was sent home with no medication post-op. I am now 195lbs my A1C is 4.8. This was the best decision ever ❤️

Agreed! I am 43 and type 2 diabetic. I weigh around 213 and am 5’3”. I was diagnosed last year but it’s possible since my diagnosis of being gestation while pregnant in 2016 that I could have been pre diabetic. Be that as it may, my doctor who is helping manage with metformin never brought this procedure up and I have been mentally and emotionally drained. I have two kids and want to be around for them. For whatever reason, I had a tremendous amount of fluid on me while pregnant because I don’t over eat but I have slowly gained so much weight around my stomach and waste-line. I found out today that Kaiser insurance doesn’t pay for this surgery and I need to get this done because I most definitely want to reverse my diabetes. I have even looked into going out of the country to get it because I’m able to pay out of pocket and cost is more feasible. That’s a last resort though. Thank you again, I think the bypass is a better option. I just don’t want to look sick with my weight loss.

Anyhow, my situation s exactly the same. 43, 2 kids, 5'1, gestational diabetes/pre diabetic, hypertension and pretty much everything you said. I go see my doctor on the 6th. I prayed about and had a long conversation with God and my family. I want live my life and not be affected by my weight and the other issues that come along with it.
I had sleeve surgery in 2017, and lost all my weight. Unfortunately, today I suffer with severe gerd/acid reflux and a little weight gain. I was thinking about revision surgery. I was told that the mini bypass would just make my gerd worse. I am so so confused. My worry is that the full bypass will make me lose too much weight. Can you please help me with understanding it better?

Very informative! I’m currently at 365lbs and will be having the Gastric Sleeve done in 3 weeks. Why would you do a sleeve?? I am 325 lbs and am going for the bypass on Monday.. My doctor told me if you need to lose more than 100 lbs it's pointless to do the sleeve. The sleeve is for patients needing to lose up to 80 lbs 🤔

I was in moderate pain unpon waking up soon after but was given pain killers and IV during my 24 hour hospital stay. I had zero pain after 24 hours, but I did feel weird in the stomach. For the first 2 and a half weeks, my abdomen was "bubbly" (for a lack of a better word) which I assume was just the healing process. Diet wise, I will transition into soft foods tomorrow, but I literally eat 1/8 of a teaspoon before feeling satiated. I've grown extremely sensitive to smells. I smell things others don't, and I'm a bit grossed out by food sometimes, though the grossness has faded lol. What surprised me is how difficult it is to drink water; even taking small sips has been difficult for me. Water feels VERY heavy from the moment I swallow it. For this reason, I was dehydrated a few days ago and thus needed IV 3 times in a span of a week-nothing serious tbh-just lots of fatigue because of the dehydration. Drinking as I walked helps tremendously. It is very important to follow the post-OP diet strictly to the T and that you walk a minimum of 30 minutes/day to ensure proper healing. I also strongly suggest learning how to eat intuitively and learn the "healthy" way of eating (i.e. nickel sized bites, putting your spoon down between bites, learning your bodily cues, etc.). I started taking a multivitamin, iron, and vitamin D immediately following the surgery to prevent any deficiencies. Finally, before the surgery, I was 227lbs. 20 days later, I weigh 198lbs. My family says they see the difference in my body already.

I had the gastric sleeve done in November 2020 and lost 195 pounds in a year and a half and have stayed consistent since then. I went from 370 to 175 pounds and as a 6’ tall guy my weight is right where it should be for my height. I was 28 years old when I had it done so that might have helped some too. I chose the sleeve because it was less invasive compare to completely rerouting everything but I also didn’t want to deal with the dumping syndrome and other stuff that can come from the bypass. I’m happy with the outcome and would recommend bariatric surgery to people who are serious about following through with what comes after. If you’re not going to be serious about it because you think all your weight is going to magically disappear without doing anything then you won’t be successful with it and be part of the statistics of people who didn’t lose much weight. Good luck to everyone who’s thinking of or has had it done. The transformation can be amazing if you have the mindset to it.

That’s the one I want to have in singing the consent tomorrow but I hear some people saying they threw up a lot or reflux and I’m wondering if is the surgery itself or just bc they probably eat wrong stuff than fell sick ? What do you think ?

@@BLACKGIRLBROKEN I’ve had no major complications. Thrown up a few times but always my own fault when I eat too fast. Constipation and now I’m losing some hair but fortunately I had thick hair to start with. 5.5 months and down 75 lb.

Love my surgeon Dr. Pitcher, I am a current patient. I had the gastric bypass surgery on 06/14 & just about 3 mos post op. to date & I've lost about 65lbs! His wonderful & his reassuring support before & after surgery is amazing. He takes his time & answers any questions or concerns you may have and his team is just as encouraging & supportive. It's not an easy surgery & I am still adjusting to my new life style & I have had to make a new relationship w/food but I have no regrets. This is by far the BEST decision I've ever made for myself. He said Since I’m a type 2 diabetic it would be better for me I have a host of other comorbidities I’m medium obese and I thought the bypass was for ppl that was severely obese but science is learning more and more with the studies of these surgeries new info is learned I think this is the best video comparison on the surgeries. @@estheradaaja2023 Hi. After 4 months I am getting used to the new pouch so I am feeling better. Unfortunately my surgeon and his staff were not very thorough in explaining the food limitations and I was overeating and making bad food choices like fried foods. My surgeon also failed to tell me about post bariatric surgery Telogen Effluvium (hair loss). In a period of a month I have lost an enormous amount of hair. I believe everyone goes through that. Regrettably not all surgeons are as thorough as Dr. Pilcher, so be careful of your surgeon choice and ask many questions. The nature of my problems were excruciating pain after eating, dehydration (never thirsty) and severe constipation. I have learned a lot from Dr. Pilcher's videos and thanks to his explanations I am now aware of many things that I should have been told from the start.

I must say even with my surgeons suggestion coming from a lap band that id be happier and more successful with the bypass I was still too scared of the Bypass. As soon as he said its the same to the body on a physilogical way- I said why take the chance of this reflux with the sleeve, when that is the precise reason why im removing the band!! Gastric bypass is for me far superior. I had sleeve ten years ago and got the revision to RNY November 2021 . Never been happier , losing more and feel so much better. I had wanted the DS for revision, the surgeon did not recommend it for me , I believe due to my age, and I didn’t have a huge amount to lose , I suggest people listen to their surgeons advice, they do know what is best ! RNY rocks ! Thanks for your videos educating people.
Gastric Bypass is what I am leaning to as well, but I read you can't eat red meet afterwards, what, never ever? Is it no longer digestible? I don't eat it a lot now, but occasionally I really crave a steak, & I couldn't give that up for ever.
@@user-vu7rv1xf1l Thats totally false about red meat. At the beginning its hard to digest, and not recommended as time goes on you can have a little, meaning all dense meats will fill you up. Chichen being very dry.. will be the hardest one in its plain form to swallow.. go for it and you can eat almost anything down the road. Just account for it in your plan

Being from an European country in which English is not the mother language I learned English in school for 5 years and had some practice talking it in Asia but on a very low level I"m very happy with the English dr. Pilcher uses. Searching for months for information to make my decission for gastric bypass OR gastric sleeve this video gave me the most clear and far going information I found in months reading, and looking video"s, and talking with people who had a gastric surgery. And, important for me, in a kind of English I understand. So now I know much more then I knew before, and even in English, which is my third language. My healthcare team made me go to 10 weeks of orientation for my desicion, you should ask your healthcare if they offer that, it doenst comit you to the surgery, it makes it so you can make an informed choice.
Pcos having 35 year old mom of 2 here to say that I'd go with the sleeve. My sister in law almost passed after her bypass surgery, it's more risky. 4 family members, 2 friends and now myself have had the sleeve and after 8 years of the person with the longest surgery I know the sleeve people have been amazing. From weight in the 400s to the 200s.
This is absolutely the best video on gastric bypass and sleeve that I’ve seen to date you explain it fully. I just had my surgery six days ago and I feel really good for just having surgery and I’m very optimistic at my outcome. I did have a moment work. I asked myself did I do the right thing. Only because the family member seems to be against it. But this video has completely added important information needed for others if they did not understand the reasoning behind the surgery rather than just a copout or laziness. I had the sleeve 9 years out and I developed a substantial hiatal hernia. I am now scheduled to have RNY surgery in 12 days. I did have a 40 pound regain on my sleeve. I am hoping this surgery results in substantial weight loss and feeling much better with no more reflux or nausea. Your video helped put my fears to rest. Thank you.
I am almost 2wks post op from sleeve surgery. I only want to lose 100pds max. No pre-existing conditions/meds. After surgery, I haven't had any cravings for sweets or soda which were both my biggest vices. I have already slowly started back to regular foods bcuz I despise shakes. So far no issues other than nasausea (which im ok with as long as I can enjoy a little food sometimes). I'm hoping that I do well long term with this procedure and see results.

I had the gastric bypass surgery in November 2019 at the age of 68, and so far so good: NO complications whatsoever; NO pain, NO diarrhea, NO blood clots ……NOTHING AT ALL! And, I don’t have diabetes or any other major health issues, i,e. anemia, blood disorders, heart disease , asthma, etc. etc. I am a “health nut” so eating healthy is priority number one for me, and I am a vegetarian per se; I have been pretty healthy all my life. This gastric bypass surgery has been the greatest decision that I have ever made. My acid reflux IS definitely gone for good; my bowel movements are normal, my back pain is gone, and incontinence is also gone…..and I am enjoying my life to the fullest AFTER losing 95 pounds, when I was 235 and I went down to 140. My ideal weight right now because of my age and height. What a relief that is, a huge relief. 😊 NEVER, EVER smoked nor did drugs; I NEVER, EVER drank alcohol NOT EVEN socially, and I still DO NOT now. I don’t care for it nor I like it and that it is perfectly fine with me. The only issue that I have is that I developed HYPOGLYCEMIA (Low blood sugar) after this surgery more or less after 2 years of the gastric bypass surgery done which is a side effect of the gastric bypass surgery according to my endocrinologist. Never before I had blood sugar problems/issues…..NEVER! As a matter of facts, I fainted inside a Mall when my blood sugar dropped as low as 50, losing consciousness and when I woke up, I was in a hospital and stayed there for 5 days. They were monitoring me and trying to find out what was wrong with me having done ALL kind of tests including a head to toes MRI, and all tests were negative. So, I took it upon myself to go to an endocrinologist who, not only sent me to a lab to check the blood sugar levels in my blood, but he sent me for an “in-depth” blood sugar testing done in a hospital, and it came back positive for HYPOGLYCEMIA. It is a nuisance having to check my blood sugar on a daily basis but it could have been worse, so I am not complaining. After a trial period dealing with low blood sugar only by diet, my endocrinologist decided to prescribed “ACARBOSE” to take this pills before any meals I eat, and I am more than okay. I feel terrific. I would rather have hypoglycemia than diabetes, that’s for sure. Hell No, I do not want diabetes at all. It doesn’t run in both of my parents’ families so I am very FORTUNATE not having blood sugar issues or I should say HYPERGLYCEMIA (High blood sugar or diabetes). PS: Dr. Pilche, if you respond to patients like me, can you explain the reasons why I developed hypoglycemia after the gastric bypass surgery??? It is really appreciated. Thank you in advance for my request.
What most people don’t tell you is that the sleeve cures a lot of things or has a high chance of curing things like diabetes, sleep apnea and other things related to obesity. My doctor took me off of insulin that day if the surgery. Of course you have to do the work though too. Meaning no more dumb snacks and sweets like junk food. My sugar levels have been great since surgery and I haven’t even went back to eating junk and I’m 3 years post op and haven’t messed up either
I have a friend in Ministry who lost over 100 pounds with the sleeve. Is all about food maintenance and simple exercise such as walking. People are responding excellently with the sleeve.

Lucky to have had Dr. Pilcher perform my surgery and after care. He is definitely well trained, informed, and highly skilled. Do your research -- not all surgeons in this space are reputable. I don't think you can find anyone better than John Pilcher to do your surgery. After that -- it's on you. Respect the gift, maintain the weight loss and improved health!
This video was so helpful and also calming. I was leaning towards the sleeve because I'd heard horror stories about the bypass and heard that a sleeve is "less invasive". After watching this, I'm definitely going to get the bypass as I already get acid reflux. Although it is minor, I don't want to make it worse and like the fact that a bypass will actually help with it. Although I won't be able to use NAIDs, I don't see this as being a huge issue for me as I only use them during my period and I have other ways I could manage that pain instead. I got gastric bypass in Mexico in April 2021 and I have lost 115lbs, hitting my weight loss goal already, But the doctor said that bypass had malabsorption for the rest of my life. He recommended it due to my thyroid being completely destroyed in malpractice. I'm glad I did it but this information would have made it so much easier for my anxiety!
Thanks for watching Sylvya. Just a cautionary note - in much of Latin America, a different version of the gastric bypass is done that DOES create some reduced absorption of nutrients and vitamins. Best of luck on your journey!

My Dr told me he wouldn’t preform the bypass. Due to age. He also stated that I would Surpass others that had the surgery the same day. 3 years later He was RIGHT. He told me you put in the work despite your disadvantages. Because I didn’t have the same opportunities as the others. I didn’t have to go through all the hoops. But he stated that I needed to go on UA-cam and gather all the information that I will need. And I did just that I worked out before and after surgery. I had already changed my outlook on food. I’m beyond thankful for him and his team!
I am thinking of having surgery for yrs but I am scared with complications. I am glad I found this video. It helps me a lot with understanding. I am 265lbs 56 yrs old with diabetes 2,sleep apnea, and allergy asthma. I am walking with my dogs nearly everyday as my exercise , I don’t want to go to the gym because of shame. I am not a big eater, no matter what diet I do I’m not losing any weight. I was told I’m gaining weight due to my asthma medications. I was also diagnosed with panic disorder. Dr. Pilcher, I came across your videos recently after considering GB and after talking to a friend who had the surgery as well as her Mother who have no regrets having it done. I really appreciate the information on this video to give me some idea of what it is about and things to consider. Today I am going to my primary doctor to see if they would give me a referral to see a surgeon in order to know if I am a good candidate for the surgery. I have many medical issues that the surgery maybe beneficial for me besides just want to be smaller and able to move better. Thank you for making it not seem as scary as people or the internet make it out be but certainly life changing if commited to the process. Your calm demeanor and kind spirit is very reassuring if I decide this is the right path for me. The liquid diet at first seems overwhelming but to know many people have been sucessful with the help of a GB seems worth it. OH MY GOD! I'm so happy I watched this in its entirety, it has EVERYTHING I wanted to know answered. Initially I was looking into lap band, then wanted vsg (it's trendy now), now I'm leaving leaning towards byspass. I am 31, and importantly it's reversible if needed. I wanted lap band because I can lose it when I want but it doesn't work apparently and is high maintenance.
What an incredible explanation🙌 my surgery happened 22 years ago and my hunger increased after 10 years and i gradually regained 40 of 103 pounds. The restriction of my pouch size has maintained 60% of my loss permanent.

Loved the explanation. Sadly, insurance does not cover either of the surgeries. My hubby did have the sleeve because of some extreme hernias that had to be taken care of and it was self pay. He’s lost 140 in 8 months, so tremendous results. Hard to do self pay for a second surgery.
What state are you in? My surgery is 100% covered by my insurance. I’m in Texas. I wonder if you could travel out of state and get it covered or maybe consider changing your insurance because it’s definitely covered by insurance most places.
Most insurance covers it. Sounds like you need to maybe look into it and if yours won’t pay, then make changes at open enrollment time. Call the other insurances available to you and find out if they cover it.
i had the gastric sleeve and whilst i lost 80lbs and have maintained for now 2 years, i think for someone for my original size (320) a bypass would of been a better idea. However, if i eat proteins mostly i still have very good restriction so i know if i get my head right i can get there :D but very good explanation
After going through so many videos I have to say this one has all the information I was looking for,very informative.thank you so much for taking the time,now the difficult part is to find a surgeon that’s qualified.
I’m one of the females for which the sleeve did not produce results. I felt hunger even in the few days right after surgery. It also gave me reflux. I’ve been self treating with Nexium daily which keeps it mostly at bay. I had not thought to have an endoscopy to check on things though. If I ever consider a revision to bypass, I would probably come to your practice as I live in the Austin area.
Endoscopy is what your doctor should recommend after a sleeve. They should recommend that afterwards to see if the pouch has stretched since the surgery to see if that’s why you eating more
Friend of mine had the sleeve and hers as well never had good results. She ended up having infections and life threatening complications afterwards. She ended up had to get the bypass once she was stable enough and is much better health wise now.
I’m so happy I did this surgery & would do it again if I need to, thank you for explaining the difference between both surgery…. I had the gastric bypass done 3 yrs ago went from 224lbs to 135lbs but now I’m at 179lbs. A little worried about that but will see my surgeon & Dietitian soon…

Thank you for the info. I'm 68 years old and 6+ years out from Gastric Bypass due to acid reflux. No problems , yes some dumping usually due to what I did and still have my restriction,, 108 pounds lost and through the years 20 weight gain, but about 90 pounds seems to be my set point.

I have similar issues and also considered the sleeve and mimi gastric bypass. However my surgeon advised me to go for the full bypass since with a mini there’s only one connection and something about the bile juices collection with a mini wouldn’t work with my hyperacidity issues.

I was leaning toward the sleeve thinking the Bypass was for much larger patients. But because of my acid reflux, I am now leaning toward the Bypass. I do however want to know ... how does the surgeon determine how much stomach to remove? Is it determined by the size of the patient?
That's a great question to ask your surgeon. In my personal experience, it still makes the most sense to make a tiny gastric couch for smaller patients as well as for larger patients. The smaller patients lose an *appropriate* amount of weight and not an excessive amount.

I have the Lapland and did well for a couple of years. By ten years, I'd regained all the original weight. At 15 year mark, tried ozempic and gained even more weight as one of a subset of people where it didn't work. At 70, just after giving up on ozempic, severe joint and muscle pain began, so I have been newly diagnosed with fybromyalgia and have started Duloxitene. I'm going to ask the surgeon for the bypass or switch. I'm so tired of fighting my body. You'd think at 70, the appetite would naturally decrease. Sigh!😢 Thanks for the interesting breakdown. I will research the switch as well out of desperation. I don't smoke or drink alcohol so they are good points. I also do swimming exercises twice a week.

I had my gastric bypass 22 years ago. I had one of the first lapriscop surgeries in my area. San Francisco CA. I went from 238 to 135. My PCOS went dormant and i was able to bring 2 pregnancys to full turm.

@j2i8h Hi, yes, the pregnancy weight gain was not an issue. I had my daughter 2 years after gastric bypass. I gained 23 pounds with her pregnancy, and 2 days after delivering her, i had lost 20 pounds from the delivery. I only had 3 pounds remaining. With my son, he was born 6 years after my bypass, and i gained 25 pounds with his pregnancy and also dropped 20 a couple of days after birth. I regained 43 pounds of the 104 that I had lost 40%...13 years after gastric bypass when I turned 40 and had a car accident that left me with a brain injury. I was not able to exercise for 3 years. It affected my pituitary, and I also entered primerose, which makes it harder to take weight off.

@@ashley47448 honestly I used to get really bad migraines and the only thing that helped me was excedrin however now that I lost over 50 lbs I’ve had less migraines and my surgeon referred me to a migraine specialist and they gave me a different type is medication that I am able to take in case I get one. Hope that helps

I had the gastric bypass surgery 5/20/2021. I was dx with uncontrolled diabetes type 2, HTN, sleep apnea, neuropathy, asthma-my highest A1C was 16. My highest weight gain 302. I was sent home with no medication post-op. I am now 195lbs my A1C is 4.8. This was the best decision ever ❤️

Agreed! I am 43 and type 2 diabetic. I weigh around 213 and am 5’3”. I was diagnosed last year but it’s possible since my diagnosis of being gestation while pregnant in 2016 that I could have been pre diabetic. Be that as it may, my doctor who is helping manage with metformin never brought this procedure up and I have been mentally and emotionally drained. I have two kids and want to be around for them. For whatever reason, I had a tremendous amount of fluid on me while pregnant because I don’t over eat but I have slowly gained so much weight around my stomach and waste-line. I found out today that Kaiser insurance doesn’t pay for this surgery and I need to get this done because I most definitely want to reverse my diabetes. I have even looked into going out of the country to get it because I’m able to pay out of pocket and cost is more feasible. That’s a last resort though. Thank you again, I think the bypass is a better option. I just don’t want to look sick with my weight loss.

I go see my doctor on the 6th. I prayed about and had a long conversation with God and my family. I want live my life and not be affected by my weight and the other issues that come along with it.
I had sleeve surgery in 2017, and lost all my weight. Unfortunately, today I suffer with severe gerd/acid reflux and a little weight gain. I was thinking about revision surgery. I was told that the mini bypass would just make my gerd worse. I am so so confused. My worry is that the full bypass will make me lose too much weight. Can you please help me with understanding it better?

@@Iamnatalierose I ended up getting a laparoscopic gastric sleeve completed in late November (3 weeks ago exactly). The surgery took less than an hour. I was in moderate pain unpon waking up soon after but was given pain killers and IV during my 24 hour hospital stay. I had zero pain after 24 hours, but I did feel weird in the stomach. For the first 2 and a half weeks, my abdomen was "bubbly" (for a lack of a better word) which I assume was just the healing process. Diet wise, I will transition into soft foods tomorrow, but I literally eat 1/8 of a teaspoon before feeling satiated.
I've grown extremely sensitive to smells. I smell things others don't, and I'm a bit grossed out by food sometimes, though the grossness has faded lol.
What surprised me is how difficult it is to drink water; even taking small sips has been difficult for me. Water feels VERY heavy from the moment I swallow it. For this reason, I was dehydrated a few days ago and thus needed IV 3 times in a span of a week-nothing serious tbh-just lots of fatigue because of the dehydration. Drinking as I walked helps tremendously. It is very important to follow the post-OP diet strictly to the T and that you walk a minimum of 30 minutes/day to ensure proper healing. I also strongly suggest learning how to eat intuitively and learn the "healthy" way of eating (i.e. nickel sized bites, putting your spoon down between bites, learning your bodily cues, etc.).
I started taking a multivitamin, iron, and vitamin D immediately following the surgery to prevent any deficiencies. Finally, before the surgery, I was 227lbs. 20 days later, I weigh 198lbs. My family says they see the difference in my body already.

I had the gastric sleeve done in November 2020 and lost 195 pounds in a year and a half and have stayed consistent since then. I went from 370 to 175 pounds and as a 6’ tall guy my weight is right where it should be for my height. I was 28 years old when I had it done so that might have helped some too. I chose the sleeve because it was less invasive compare to completely rerouting everything but I also didn’t want to deal with the dumping syndrome and other stuff that can come from the bypass. I’m happy with the outcome and would recommend bariatric surgery to people who are serious about following through with what comes after. If you’re not going to be serious about it because you think all your weight is going to magically disappear without doing anything then you won’t be successful with it and be part of the statistics of people who didn’t lose much weight. Good luck to everyone who’s thinking of or has had it done. The transformation can be amazing if you have the mindset to it.

That’s the one I want to have in singing the consent tomorrow but I hear some people saying they threw up a lot or reflux and I’m wondering if is the surgery itself or just bc they probably eat wrong stuff than fell sick ? What do you think ?

@@BLACKGIRLBROKEN I’ve had no major complications. Thrown up a few times but always my own fault when I eat too fast. Constipation and now I’m losing some hair but fortunately I had thick hair to start with. 5.5 months and down 75 lb.
My goal is 40-50lbs max. You Iost it in 12 weeks. I'm wondering if I'm a good candidate for wls with only a 40-50 lb weight loss goal.

I had mine in 2016 ( by pass ) my weight was 290 now it is 180 , muscular 14% of body fat , the secret is diet, exercising and keep vitamins on level . ❤

I’m 55 and I was shocked when my Bariatric surgeon said bypass. He said Since I’m a type 2 diabetic it would be better for me I have a host of other comorbidities I’m medium obese and I thought the bypass was for ppl that was severely obese but science is learning more and more with the studies of these surgeries new info is learned I think this is the best video comparison on the surgeries. @@estheradaaja2023 Hi. After 4 months I am getting used to the new pouch so I am feeling better. Unfortunately my surgeon and his staff were not very thorough in explaining the food limitations and I was overeating and making bad food choices like fried foods. My surgeon also failed to tell me about post bariatric surgery Telogen Effluvium (hair loss). In a period of a month I have lost an enormous amount of hair. I believe everyone goes through that. Regrettably not all surgeons are as thorough as Dr. Pilcher, so be careful of your surgeon choice and ask many questions. The nature of my problems were excruciating pain after eating, dehydration (never thirsty) and severe constipation. I have learned a lot from Dr. Pilcher's videos and thanks to his explanations I am now aware of many things that I should have been told from the start.

I must say even with my surgeons suggestion coming from a lap band that id be happier and more successful with the bypass I was still too scared of the Bypass. As soon as he said its the same to the body on a physilogical way- I said why take the chance of this reflux with the sleeve, when that is the precise reason why im removing the band!! Gastric bypass is for me far superior. I had sleeve ten years ago and got the revision to RNY November 2021 . Never been happier , losing more and feel so much better. I had wanted the DS for revision, the surgeon did not recommend it for me , I believe due to my age, and I didn’t have a huge amount to lose , I suggest people listen to their surgeons advice, they do know what is best ! RNY rocks !

Gastric Bypass is what I am leaning to as well, but I read you can't eat red meet afterwards, what, never ever? Is it no longer digestible? I don't eat it a lot now, but occasionally I really crave a steak, & I couldn't give that up for ever.
@@user-vu7rv1xf1l Thats totally false about red meat. At the beginning its hard to digest, and not recommended as time goes on you can have a little, meaning all dense meats will fill you up. Chichen being very dry.. will be the hardest one in its plain form to swallow.. go for it and you can eat almost anything down the road. Just account for it in your plan

Being from an European country in which English is not the mother language I learned English in school for 5 years and had some practice talking it in Asia but on a very low level I"m very happy with the English dr. Pilcher uses. Searching for months for information to make my decission for gastric bypass OR gastric sleeve this video gave me the most clear and far going information I found in months reading, and looking video"s, and talking with people who had a gastric surgery. And, important for me, in a kind of English I understand. So now I know much more then I knew before, and even in English, which is my third language. For someone with PCOS , anemia and reflux is gastric bypass a better option. My weight is 120 kg and 165 cm height. Thank you for explaining everything so clear. I've been struggling with the decision of having the surgery for a while and I think I'm convinced now.
My healthcare team made me go to 10 weeks of orientation for my desicion, you should ask your healthcare if they offer that, it doenst comit you to the surgery, it makes it so you can make an informed choice.
Pcos having 35 year old mom of 2 here to say that I'd go with the sleeve. My sister in law almost passed after her bypass surgery, it's more risky. 4 family members, 2 friends and now myself have had the sleeve and after 8 years of the person with the longest surgery I know the sleeve people have been amazing. From weight in the 400s to the 200s.
This is absolutely the best video on gastric bypass and sleeve that I’ve seen to date you explain it fully. I just had my surgery six days ago and I feel really good for just having surgery and I’m very optimistic at my outcome. I did have a moment work. I asked myself did I do the right thing. Only because the family member seems to be against it. But this video has completely added important information needed for others if they did not understand the reasoning behind the surgery rather than just a copout or laziness. I had the sleeve 9 years out and I developed a substantial hiatal hernia. I am now scheduled to have RNY surgery in 12 days. I did have a 40 pound regain on my sleeve. I am hoping this surgery results in substantial weight loss and feeling much better with no more reflux or nausea. Your video helped put my fears to rest. Thank you.

I am almost 2wks post op from sleeve surgery. I only want to lose 100pds max. No pre-existing conditions/meds. After surgery, I haven't had any cravings for sweets or soda which were both my biggest vices. I have already slowly started back to regular foods bcuz I despise shakes. So far no issues other than nasausea (which im ok with as long as I can enjoy a little food sometimes). I'm hoping that I do well long term with this procedure and see results.
I'm also addicted to sugar. I'm confused as to which one I should get. Has your sugar cravings been under control since the surgery??

I had the gastric bypass surgery in November 2019 at the age of 68, and so far so good: NO complications whatsoever; NO pain, NO diarrhea, NO blood clots ……NOTHING AT ALL! And, I don’t have diabetes or any other major health issues, i,e. anemia, blood disorders, heart disease , asthma, etc. etc. I am a “health nut” so eating healthy is priority number one for me, and I am a vegetarian per se; I have been pretty healthy all my life. This gastric bypass surgery has been the greatest decision that I have ever made. My acid reflux IS definitely gone for good; my bowel movements are normal, my back pain is gone, and incontinence is also gone…..and I am enjoying my life to the fullest AFTER losing 95 pounds, when I was 235 and I went down to 140. My ideal weight right now because of my age and height. What a relief that is, a huge relief. 😊
NEVER, EVER smoked nor did drugs; I NEVER, EVER drank alcohol NOT EVEN socially, and I still DO NOT now. I don’t care for it nor I like it and that it is perfectly fine with me.
The only issue that I have is that I developed HYPOGLYCEMIA (Low blood sugar) after this surgery more or less after 2 years of the gastric bypass surgery done which is a side effect of the gastric bypass surgery according to my endocrinologist. Never before I had blood sugar problems/issues…..NEVER! As a matter of facts, I fainted inside a Mall when my blood sugar dropped as low as 50, losing consciousness and when I woke up, I was in a hospital and stayed there for 5 days. They were monitoring me and trying to find out what was wrong with me having done ALL kind of tests including a head to toes MRI, and all tests were negative. So, I took it upon myself to go to an endocrinologist who, not only sent me to a lab to check the blood sugar levels in my blood, but he sent me for an “in-depth” blood sugar testing done in a hospital, and it came back positive for HYPOGLYCEMIA. It is a nuisance having to check my blood sugar on a daily basis but it could have been worse, so I am not complaining. After a trial period dealing with low blood sugar only by diet, my endocrinologist decided to prescribed “ACARBOSE” to take this pills before any meals I eat, and I am more than okay. I feel terrific.
I would rather have hypoglycemia than diabetes, that’s for sure. Hell No, I do not want diabetes at all. It doesn’t run in both of my parents’ families so I am very FORTUNATE not having blood sugar issues or I should say HYPERGLYCEMIA (High blood sugar or diabetes).
PS: Dr. Pilche, if you respond to patients like me, can you explain the reasons why I developed hypoglycemia after the gastric bypass surgery??? It is really appreciated. Thank you in advance for my request.

What most people don’t tell you is that the sleeve cures a lot of things or has a high chance of curing things like diabetes, sleep apnea and other things related to obesity. My doctor took me off of insulin that day if the surgery. Of course you have to do the work though too. Meaning no more dumb snacks and sweets like junk food. My sugar levels have been great since surgery and I haven’t even went back to eating junk and I’m 3 years post op and haven’t messed up either

I have a friend in Ministry who lost over 100 pounds with the sleeve. Is all about food maintenance and simple exercise such as walking. People are responding excellently with the sleeve.

This video was so helpful and also calming. I was leaning towards the sleeve because I'd heard horror stories about the bypass and heard that a sleeve is "less invasive". After watching this, I'm definitely going to get the bypass as I already get acid reflux. Although it is minor, I don't want to make it worse and like the fact that a bypass will actually help with it. Although I won't be able to use NAIDs, I don't see this as being a huge issue for me as I only use them during my period and I have other ways I could manage that pain instead. I wish I had seen this before my surgery! I got gastric bypass in Mexico in April 2021 and I have lost 115lbs, hitting my weight loss goal already, But the doctor said that bypass had malabsorption for the rest of my life. He recommended it due to my thyroid being completely destroyed in malpractice. I'm glad I did it but this information would have made it so much easier for my anxiety!

My Dr told me he wouldn’t preform the bypass. Due to age. He also stated that I would Surpass others that had the surgery the same day. 3 years later He was RIGHT. He told me you put in the work despite your disadvantages. Because I didn’t have the same opportunities as the others. I didn’t have to go through all the hoops. But he stated that I needed to go on UA-cam and gather all the information that I will need. And I did just that
I worked out before and after surgery. I had already changed my outlook on food. I’m beyond thankful for him and his team!

I am thinking of having surgery for yrs but I am scared with complications. I am glad I found this video. It helps me a lot with understanding. I am 265lbs 56 yrs old with diabetes 2,sleep apnea, and allergy asthma. I am walking with my dogs nearly everyday as my exercise , I don’t want to go to the gym because of shame. I am not a big eater, no matter what diet I do I’m not losing any weight. I was told I’m gaining weight due to my asthma medications. I was also diagnosed with panic disorder. Im so sick of being sick. I used to be 120-145lbs until 2000. I gained pounds all these yrs and I’m lack of confidence. Thank you for your video it helps a lot.

Dr. Pilcher, I came across your videos recently after considering GB and after talking to a friend who had the surgery as well as her Mother who have no regrets having it done. I really appreciate the information on this video to give me some idea of what it is about and things to consider. Today I am going to my primary doctor to see if they would give me a referral to see a surgeon in order to know if I am a good candidate for the surgery. I have many medical issues that the surgery maybe beneficial for me besides just want to be smaller and able to move better. Thank you for making it not seem as scary as people or the internet make it out be but certainly life changing if commited to the process. Your calm demeanor and kind spirit is very reassuring if I decide this is the right path for me. The liquid diet at first seems overwhelming but to know many people have been sucessful with the help of a GB seems worth it. OH MY GOD! I'm so happy I watched this in its entirety, it has EVERYTHING I wanted to know answered. Initially I was looking into lap band, then wanted vsg (it's trendy now), now I'm leaving leaning towards byspass. I am 31, and importantly it's reversible if needed. I wanted lap band because I can lose it when I want but it doesn't work apparently and is high maintenance.

What an incredible explanation🙌 my surgery happened 22 years ago and my hunger increased after 10 years and i gradually regained 40 of 103 pounds. The restriction of my pouch size has maintained 60% of my loss permanent.

Loved the explanation. Sadly, insurance does not cover either of the surgeries. My hubby did have the sleeve because of some extreme hernias that had to be taken care of and it was self pay. He’s lost 140 in 8 months, so tremendous results. Hard to do self pay for a second surgery.

What state are you in? My surgery is 100% covered by my insurance. I’m in Texas. I wonder if you could travel out of state and get it covered or maybe consider changing your insurance because it’s definitely covered by insurance most places.
Most insurance covers it. Sounds like you need to maybe look into it and if yours won’t pay, then make changes at open enrollment time. Call the other insurances available to you and find out if they cover it.

i had the gastric sleeve and whilst i lost 80lbs and have maintained for now 2 years, i think for someone for my original size (320) a bypass would of been a better idea. However, if i eat proteins mostly i still have very good restriction so i know if i get my head right i can get there :D but very good explanation

I’m one of the females for which the sleeve did not produce results. I felt hunger even in the few days right after surgery. It also gave me reflux. I’ve been self treating with Nexium daily which keeps it mostly at bay. I had not thought to have an endoscopy to check on things though.
If I ever consider a revision to bypass, I would probably come to your practice as I live in the Austin area.

Endoscopy is what your doctor should recommend after a sleeve. They should recommend that afterwards to see if the pouch has stretched since the surgery to see if that’s why you eating more
Friend of mine had the sleeve and hers as well never had good results. She ended up having infections and life threatening complications afterwards. She ended up had to get the bypass once she was stable enough and is much better health wise now.
Anecdotal information. My gastric reflux was caused by coffee. Maybe you have an issue similar to that. Hoping you get a simple solution!

Thank you so much for your detailed explanation of both of these procedures. I've had a lapband but I'm going for a revision next month. My surgeon did advise bypass but he was going to do an endoscopy first before deciding on the final surgery (sleeve vs bypass). Your video has helped tremendously in helping me make an informed decision. God bless you for all the work that you do.

I am too having revision from the lap and and preparing for the sleeve or bypass. I had a discussion with my surgeon and I don’t have to decide yet, but doing my research to decide which one is right for me.

Thank you for the info. I'm 68 years old and 6+ years out from Gastric Bypass due to acid reflux. No problems , yes some dumping usually due to what I did and still have my restriction,, 108 pounds lost and through the years 20 weight gain, but about 90 pounds seems to be my set point.

This is a great video, I work in the OR and scrub for these procedures on a regular basis. I can really tell that you care about your patients and them being educated about these procedures . Well done you seem like you would be a surgeon I would love to scrub with!

I have similar issues and also considered the sleeve and mimi gastric bypass. However my surgeon
advised me to go for the full bypass since with a mini there’s only one connection and something about the bile juices collection with a mini wouldn’t work with my hyperacidity issues.

Thank you so much for this video. You have no idea how helpful this information has been in helping me decide which weight-loss procedure to get for myself. I am 5'0 and 185lbs. I was leaning toward the sleeve thinking the Bypass was for much larger patients. But because of my acid reflux, I am now leaning toward the Bypass. I do however want to know ... how does the surgeon determine how much stomach to remove? Is it determined by the size of the patient?
That's a great question to ask your surgeon. In my personal experience, it still makes the most sense to make a tiny gastric couch for smaller patients as well as for larger patients. The smaller patients lose an *appropriate* amount of weight and not an excessive amount.
@@DoctorJPhello, my acid reflux is not severe & I feel safer doing sleeve my doctor said it’s my choice but do you recommend sleeve or bypass? I heard some people will experience bad acid reflux after w bypass as well & more risky?
I have the Lapland and did well for a couple of years. By ten years, I'd regained all the original weight. At 15 year mark, tried ozempic and gained even more weight as one of a subset of people where it didn't work.
At 70, just after giving up on ozempic, severe joint and muscle pain began, so I have been newly diagnosed with fybromyalgia and have started Duloxitene.
I'm going to ask the surgeon for the bypass or switch. I'm so tired of fighting my body.
You'd think at 70, the appetite would naturally decrease. Sigh!😢

@@DoctorJPjust saw your 7 day liver shrinkage diet and responded to that. I saw the surgeon yesterday and will have the bypass in 7 days so will follow your plan since my doctor had no preoperative demands. I want to make it as easy as possible.
