That crew was extremely reckless, so much so that the iowa turned her guns at that destroyer and told it to make port, the marines stormed that ship at harbor and the entire crew were arrested for suspected attempted assassination. Never mentioned the depth charge they accidentally dropped that made the wgole formation think a uboat was upon them... Or that they were firing the torps because the officers were on a adrenaline high as the aa demonstration got their spirits up, but the person over the torpedoes didnt remove something from tge third torpedo in tube 3 and it launched, they were trying to train the crew to launch torpedoes using the iowa as an unwitting target... Thats what happened.

That "face" is actually a U shaped Lifting lug welded to the very front of the nose assembly (kind of like a lifting shackle) used to attach wire rope to pull the torpedo forward when loading. Their use of shadow around the contours does make it appear to have a mouth and nose. Weird!

You should look up just how many ships accidentally rammed into each other in WWII. Japan had multiple ramming incidents and at least four of them resulted in one of the ships sinking soon after.
Look into the William D. Porter's history and you'll see it was just one cockup after another. It tried to assassinate the president twice, the first time being a live depth charge that rolled across the deck. Then, it blew up an officer's garden while it was docked at port, and when it finally saw battle, it shot down a kamikaze airplane only for it to explode underwater and sink the ship. Luckily there was no casualties.
