  • I never full understood or appreciated this cause of diarrhea until I started GI fellowship. It is one of the most satisfying GI conditions to treat!

    Thank you for this. As you were speaking I was vigorously crapping into the toilet, a pile of wet, runny dookie in the toilet, while the toilet water was splashing up. Me screaming while my family wondered “wth is she doing” this video helped me so much to understand what was happening in my bowels. Thank you.

    I went from racing to the bathroom 10-20 times a day then taking the pill I became horribly constipated. It was gastric bypass that naturally stopped the chronic diarrhea and finally get normal stools with a regular routine. Still yellow despite what I eat. Always need to watch out for carbs and greases.

    Ate something that irritated my small intestine and ended up with bile not being absorbed. Bile acid diarrhea is horrible very painful. But being very careful with diet and staying away from fats for about 2 weeks let everything heal . I slowly increased fats bact to normal. All is well. Offending food item is now off my diet list. If it burns burns burns that's bile. Stop the fats and see your doc.

    This makes me wonder if theres a medical reason why i cant eat onions, i doubt its an allergy as i CAN eat them, if i power through it (because i generally just dislike onions) but usually like 30 ir 40 min later i get stomach cramps and makes me want to go throw them up. I say i doubt its an allergic reaction as i get zero swelling, i have touched them and have eaten them in other means, but for whatever reason raw onion usually makes me throw up

    Yep, after I had my gallbladder out, despite a low fat diet, I always had diarrhea. It took a year to figure out what it was!! They called it post-cholecystectomy dumping syndrome

    Very interesting! I subscribed to your channel just before I was diagnosed with Crohn's Disease, so while I enjoy your skits, these kinds of videos are helpful to know, even if they don't directly apply to me (yet). It's good to know what questions to ask my own gastroenterologist.

      @@tgbluewolf I was diagnosed this past October, but had previously-unexplained anemia and vitamins B12 and D deficiency since 2014 or so. I'm on Inflectra infusions, have been since December, and while my numbers (and life!) are better, I'm not in remission yet.

      Greetings, I was diagnosed with Crohn's in 1992/1993 area...it was a lengthy process. I take Questran to monitor my diahrea, but that's pretty much it for now. It took years to get that point.

    Primary BAM/BAD is estimated to be 3 times more common than Crohn’s disease. Further, it’s estimated up to 1/3 of patients diagnosed with IBS-D actually have BAM.
    Type 1 BAM is related terminal ileal resection or terminal ileal disease (eg. Crohn’s)
    Type 2 BAM is idiopathic. Although a lot of research is pointing to a deficiency in FGF19 in 70% of the these patients.
    Type 3 BAM Is related to gallbladder removal, pelvic radiation…
    Labcorp now offers a blood test. 7AC4 blood test. Mayo Clinic Laboratory offers the more accurate 48hr stool test. Canada and many countries in Europe offer the SeCHAT scan.

      Wait... What crappy place are you at that this wasn't explained to you as a possible side effect?
      When did you get it removed? If it hasn't been long you might get lucky and your body adapts after a few weeks making it stop (it atleast did for me :'))

      @@cexilady3333 ouf, well I did some research beforehand anyway and found some story's of life-long diarrhea. I really hope you find a cure, cause after such a long time I doupt it'll change on it own.

      Me too. This was like I just found a piece of a puzzle. I was told a could react to fatty foods after the gallbladder operation. But there's no difference between fatty and non fatty foods for me... Its all the same, running to the toilet minutes after every meal, whatever I eat.

    My sister and I have digestive issues. I've managed to figure out mine but my sister's is still a mystery. We often find out about different conditions, see if the symptoms fit, and take it to her doc. This type of video really helps.
    She's tried the only GI specialist in network in her area and it wasn't a good visit.
    I'm not suggesting anyone use YT as a replacement for a doc but doctors are human and can't know/remember everything. I just really appreciate this type of content, especially for people who cannot afford a ton of visits and tests. It can cut down on diagnostic time

      Same here. Welchol will do you wonders. Just try to figure out the dose. I take 3 in the morning.

      No gall bladder means a lifetime of issues. Colonoscopy,scopes on esophagus, intestinal blockage,chronic diarreah,chronic constipation. Hemorrhoids. Causing rectum tears. Those are fun.

    I went to the ER for blood in my diarrhea after having diarrhea for months and losing weight dramatically without a diet change, and they just said it's anxiety and now I don't go to the doctor anymore 🤣

    Few questions:
    Could a scope confirm this? I'd imagine the colon would be irritated and inflamed if the acid is going down there.
    Related, does having this condition also increase the chances of colorectal cancer?

      Technically no. You don't eat anything the day before your colonoscopy so the irritation /inflammation would be long gone.

      No, scope can just exclude other stuff. You need a SeHCAT test to confirm.
      I had 2 scopes bevore my SeHCAT. All normal in blood and scopes but SeHCAT positive.

    Cholestyramine is awful! lol in my experience anyway.
    None of the bile acid binders ever worked well enough for me to beat just plain ole loperamide at helping my diarrhea, even though mine is 100% from bile acid not being reabsorbed.
    I had my terminal ileum removed approximately 30cm and then an additional 12cm (years later when my ostomy was reversed) following a stricture that eventually caused a complete bowel obstruction.
    Love your informative GI videos! I wish this stuff was around 20 years ago when I was a newly diagnosed young teenager going through the worst of my Crohn's

    Since it wasn't mentioned in the video, the bile sequestrant the doctor is referring to is called Colesevelam. The powder form I find disgusting, but there is a pill form (Welchol brand name). Relatively cheap and does wonders. If you have diarrehea after removing your gallbladder this might solve your problem. No doctor ever told me about it though, I had to figure it out myself!

      You can buy capsules empty in mass quantities for fairly cheap too if you can stand the powder a 00 or 000 capsule or two should hold enough for a dose I'd imagine if you really can't stand the powders

    Another cause of BAM is SIBO when bacteria create biofilm in the terminal ileum which prevents reabsorption of bile

    This is actually one of the reasons I am trying to keep my gallbladder as long as I can. That and I was a surgical tech for years and I know how badly wrong that surgery can go. I take that stone breaker supplement when I have a flare up and it definitely works

    Please respond to me !!! I have orange diarrhea and it's after i eat for me i have binge eating too because i never feel full or satisfied ever i eat all day and my stomach is empty why is that I'm gaining weight too a lot of it yes i had gallstones so i had to take some bile i also took a lot of laxatives because before i can constaption i also took other things for blood sugar and vomiting Please help me 😭🙏😭

    My dad had this when his gallbladder was failing. It was scary he couldn't eat anything and he lost a bunch of weight. He didn't know it was his gallbladder, though, because it didn't have the typical gallbladder pain. Took like two months before we convinced him that it was his gallbladder and he needed to go to the doctor.

      @@tgbluewolf I was diagnosed 2019, and attempted to manage it with a mesalamine formulation called Pentasa, and by altering my diet. I had limited success, and started on an immunosuppressant called remicade in 2021. Ever since I've felt MUCH better! I have energy again, and feel happier, all because my intestinal pain is virtually non-existent.

    But we need bile to absorb nutrients or our metabolism is wrecked ,the binder takes it all along with bile.

    I underwent a resection in May 2019 to remove my damaged ileum. I had chronic diarrhea for about 4 years. Last week, I was put on a pill for bile acid malabsorption, and I saw a very dramatic, positive improvement over night!

    I had my gallbladder out and this absolutely makes complete sense. I also have problems with the excess bile irritating my stomach.

      @@belgadog99 no I don’t throw up but I get horribly nauseated, that will last for hours, and I’ll have pain where I used to have a gall bladder. When I get “bouts” it leaves me so exhausted and drained. I’ve had an endoscopy which didn’t reveal much besides the bile irritation. Probably too much info. there 🤣

      @@suzy_____ I cry a lot. Honestly. I take prescription strength Pepcid every night and I take Amitriptyline to help with the IBS. If I can rest, I do as much as I can. No idea what else to do. Then it passes for a little while. We are traveling to places this summer that have better food practices and I am really curious to see how I will feel eating that food. I hope better because I am so terrified of messing the trip up with my stupid stomach issues.

    Very interesting! I have a severe and difficult to treat case of chrons disease and have had to have my gallbladder removed due to the organ dying (fun stuff, non-vital organs shut down and cease to function when you are severely sick for a long period of time). I will have to keep this information in mind.

    can Bile Acid Diarrhea cause backpain, my gaubladder was removed about 12 or 13 yrs. ago.and i just developed Bile Acid yellow oily Diarrhea about 2 mos. ago.and now i have sudden backpain for about 2 weeks.

    I'm an anesthesiologist and your videos keep giving me great insights into other parts of medicine I haven't encountered since med school. My Mom recently had a cholecystectomy and had been having chronic diarrhea since. Thid cause never occured to me but I'll have her bring it up with her GI. Thanks doc Schmidt!

    Thank you! Cant find a family doctor here in Canada...but if i wait for 3 hours outside a walk in clinic or go to an emergency room and the doctor isnt deflated or angry...they may take my suggestion and i could get a prescription

    Ya this was something no one warned me about when I had my gallbladder removed .
    Luckily (?) I have a lot if other chronic conditions for which half the medication cause constipation. So that + no gallbladder now just makes everything “regular”.

I had many tests for stomach pain after MANY tests & some procedures, they found bile in my empty stomach after me *smelling* Subway making bread on the way to my endoscope- causing me burning and pain all my life, when smelling food, Aniexty, gum after brushing my teeth, having shower, many other things. Since I've always had it all my life, I always thought these were hunger "pains" & couldn't imagine how people could NOT eat. I carry food with me constantly, usually gives my stomach monster something to consume.
    Stomach ulcer meds didn't help, NO H. PYLORI either & PPIs made this worse for me (although I realize people can have many conditions at once.) I wish I would have listened to my GI doctor when she asked if it could be Aniexty. She was right, also have IBS. It took me getting an Endoscope in a hospital, walking by Subway on an empty stomach. The bread smelled wonderful, yes waiting for my Endoscope was painful on my empty stomach but GI Doctor found my Bile Reflux in stomach, stomach polyps. (No colon polyps.)
    Although I've had stomach pain all my life, this was all found in my 30s after I had gallbladder removal, hiatal hernia repair and also found out I had slow motility esophagus & swallowing.
    Hint to others.. ask to pay for digital pill test for Acid Reflux- Not the WIRE in your throat for 24hrs - pain every time I swallowed.

    Did some digging and found a scholarly paper that recommended fiber supplements three times a day. Works for now. Cheaper than bile supplements. But, like he said, talk to your doc. Don’t get medical advice from YT. Or any socials.

    Ive been dealing with this for 5 years since having my gallbladder out. Binders work but ONLY when Im one them but that still doesnt rule out a sudden urge to poop on myself. So I cant really go ANYWHERE.

    Could bile acid binder stop the pancreas stones? Can you even get stones from pancreas without gallbladder? Definitely seeing gallbladder stones. Not kidney stones.

    This actually makes me angry!! After getting my gallbladder removed I have had daily issues since 2020!!! I could have skipped the colonoscopy and genetic testing for Crohns by just trying a bile binder?? For the love of pete

    Dr… you “nail it” that was my problem I sufre sor more that 40 years and finally try wit a (very young and beautiful) doctor, she prescribe Cholestyramine in powder packs of 4th. and y had enoje one week of Greg recovery!!!! No more colics!!!

    I've had this. Not comfy. Showed up after the chemo killed my gall-bladder. Eventually, it went away because my system just needed to adjust to not having a gall-bladder anymore. Also, I have to be sure that I distribute my fat intake throughout the day pretty evenly. Too much fat at once or too long without it and it comes back. Fortunately, I don't need to avoid fat. I can still have pizza and fried chicken, and french fries... just not all at once.

    I had my gallbladder removed two years ago and thankfully have never experienced bile acid diarrhea, in fact I have opposite of it and frequently have to take softeners, probiotics and laxatives. I just wish I could have normal movements.

    I'm from pakistan. Here is my story
    I have GERD issue since 18 years old and i had used antacid for it. And i was fine after that.
    Then i age 30 i had gallbladder attacked due to blocked by the stones. Then after surgery my gallbladder is removed.
    I don't have GERD issue now due to diarrhea! Now my question is that after 6 months i have problem like whenever i eat right after 10 to 20 minutes my stomach cramps started and i will rush to bathroom and there is diarrhea.
    Please suggest me what is the issue? Is it lack of bile or lack of stomoch acid or what?? I am so worried and there is no good doctor here to answer my question. Please help

    After being removed 50 cm small bowel, started me diarrhea, today after 2 years living with medicine questran 4g, I was 26 years old when it happened, this destroyed my life,it suck!

    Dr. Schmidt, this seepage of excess bile into the v intestines can also be caused by a loss of muscle function and slacked elasticity of the pylori valve (pyloris.).I have suffered from this for decades. It is uncomfortable/painful as hell. The worst.

    I was undiagnosed with entamoeba histolytica for 2 years and trichuris for 1. Due to gastroenterologists and PCPs not running and serial ova/parasite testing. Do you test your patients for parasites? Spoken as an American, DVM, who saw a tropical medicine MD that reported >70% of Americans have GI parasites that are undiagnosed or misdiagnosed as “IBS”. Do you test for parasites? Do you get taught about them at all as a human doc, because millions are being undiagnosed and it’s such a simple problem to treat depending on the organism. Please spread awareness and please, as someone who suffered and also who diagnoses parasites daily, please know they exist and people need basic tests run too.

    Can this be caused by autonomic nervous system malfunctions, too? Cause I spray acid if I am spooked awake, triggering my nervous system to go haywire.
    Taking Propranolol, Ajovi, and Ritalin has reduced the chances of it happening by a ton, it used to be pretty much every morning regardless of how I was awoken.
    Ritalin has been the absolute most helpful.
    It's also not uncommon for me to have elevated bile levels in my bi-yearly blood and urine tests. And to have massive light yellow logs that shave off 5lbs of my weight- that usually coincides with my ketones being extremely high.
    I'm not asking for a diagnosis, only theory on the possibility, food for thought. I could always google it if I wanted to self diagnose (or just ask my doctor next week)

    Hint to others.. ask to pay for digital pill test for Acid Reflux- Not the WIRE in your throat for 24hrs - pain every time I swallowed you'd be amazed how many times a min people naturally swallow, even if they aren't eating or drinking. I will also ask what happens for each tests. Getting tubes up my noses with no pain killer or being put to sleep was not explained to me. Neither was the option to swallow a digital pill. I would have paid $$$ to avoid that 24hr wire.. it is so painful, I wouldn't recommend it to depressed people, it could be tempting to be life ending, it is so painful in throat & ears. Nothing will take away the searing pain. Fortunately, as soon as it was removed, I was glad to be alive and was ready to eat.

    I was just talking to a friend about this... maybe. The worst type of diarrhea. The yellow poostorm. Very liquid, very rank and hard to clean. And it does resemble bile. Had not occurred to me before.

    Can u do a clip on irritable bowel syndrome. It a sudden onset,often instantaneously, it happens with liquid or regular diet. Using immodium to help control diarrhea, it is helping, however even without eating or small amount one lives with abdominal cramping,pain and frequent stools ,to thebpoint u just stay home. Can u do a clip on it? Thx

    Thank you for this. I've actually been told very little about what it's like to live without a gallbladder, I've just been finding it out the hard way. I also find that there's very little out there that I can digest in the form of media that will enlighten me as to how I can change the things that really suck about it. Thank you so much please make more lol. Particularly, and I'm sure that you won't actually have the capacity to do this or desire. But I'm pretty sure that it's affecting my skin health. I've had full body folliculitis ever since I got it taken out. Pretty sure that's due to the fact that I can't absorb fat soluble vitamins like vitamin A. Among other things. But I feel like that has a big part to do with it. Anyway thanks again

    OK, but if it is secondary bile acid diarrhea meaning the C4 is not elevated and FGF19 then there is no overproduction and normal amounts are not absorbing. So doesn't that mean if you take a binder you will absorb even less? You won't get diarrhea, but don't you need that bile to absorb your fats or other things?

    Bile acid diarrhea is terrible. I had it briefly a few times but only when I ate deep fried Kentucky fried chicken omg water came right out my butt onto floor. Luckily I was at home alone. It's the bad oils I can't tolerate anymore especially canola oil, seed oils. I have no problem with ghee butter olive oil tallow meat fat. My liver and gallbladder are not working good for a few years from stress and toxic chemical American food

    This is happening to me now. Never had this issue in my life until I started eating carnivore. Every meal causes it for several hours. Last week I went for 3 days and became dehydrated.
