  • Beyond its role in treating opioid dependence, methadone has unique benefits that make it a viable option for chronic pain management. But with these come multiple pitfalls, for which the risks of methadone therapy may outweigh its benefits for some patients. Levorphanol shares all of the benefits but almost none of the risks associated with methadone. Pharmacists Kominek and Brooks discuss its suitability as a therapeutic option.

    Methadone saved my life. I wouldn't be alive without it. It gives me hope to go on.

      Do u go to pain clinic? I'm on 80 mg methadone and have been on methadone a minimal of 60 mg for 32 years ..first year 20 mg ...2nd year 40 mg 3rd year 60 mg...went as high as 180 mg came down to 120 mg the last 5 years they have forced me down to 80 were 120 mg controlled my pain...methadone has allowed me to live a decent life with all my pain issues...now I have to use advil & tylenol to help, which is bad for u...I dont understand this one size fits all stuff...my clinic has been saying I'm on way to high of dose...but I wont let them take any more ..they say govt, says 30 mg a day ! where did they get that from...

      @@mreloo I am sorry your having to go through this. I live in Scotland. And they have no problems giving methadone out to ppl who have to be prescribed methadone.. In Scotland they start ppl on 40 ml , they make £1000 everytime a new person has to go and be dispensed at the chemist. Dundee Scotland has the HIGHEST drug related deaths. And it's due to ppl on methadone using streeet valium ( because the drug centre aren't so quick to dispense diazapam. It's so strange how this works.methadone helps you live a pain free life so that's a different thing. To e.g a heroin addict for 12 years and then on methadone for a further 10 years. I have no talking support or any worker seeing me. I am at the point where I want out of there twisted system. I was showing the polarity of it. I hope you have managed to drive again. And maybe I will finally get off my 40 ml.....I just might , if I'm lucky be able to feel like Im actually proud of myself. ( Sad that the only thing I'm proud of is how far I've come. I wish you well.

    I wish the small minded people in my town would watch this video. Everyone thinks that since I’m on methadone I’m just a junkie. I feel like I’m my opinion it saved my life and continues to every day

    Wow!! This was an awesome informative, and dsfinitive perspective on both of these medications.

    I'm 52, married, with Major Chronic pain issues.
    16 herniated discs in my spine alone; HELLO?
    So, yeah I have a necessary dependency for medication. I have been pondering making a switch to methadone, as I am really not comfortable with an Pump Implant which has been suggested. My number 1B concerning is abdominal distention, and constipation. Big concerns there. So,... Thanks, and btw, I really liked the jazz background music.

    Another downside of Methadone is if you need painkillers in the future for anything legitimate, chances are good the doctors or dentists who treat you won’t treat your pain and say something like…“Just take Advil or more of your Methadone” (yes really) or give you the lowest prescription possible that might as well be nothing. As soon as they hear or read Methadone everything you tell them is just “drug seeking” and will be ignored because the Dr.’s only priority from this point is to cover their backside.

    Methadone is by far the best chronic pain med,naturally long lasting and extremely cheap.Only problem is if your opioid naive (no tolerance)and you take a moderate to slightly high dose you will DIE.But for the tolerant long term patient it's a miracle drug,when every other narcotic pain med has eventually failed over a long period patient 10yrs + methadone is probably your best option but remember once your prescribed it's a lifetime drug for most,I know from being a chronic pain patient since 2007,and always start small and go slow especially if taken with a benzodiazepine,be careful and with a little common sense and you'll be fine taking both.

      30mg a day,with the new cdc guidelines thats the max because it's equal to 90mg of morphine unless you go to a addict maintenance clinic where there is no limit.Theres people there on up to 200mg,and that's twisted and backwards the addict are the ones that should be limited.

      @@ronaldclayton824 no 30 is not the max. Due to spinal surgery that didn't fix the issue (no fault of the neurosurgeon), cervical pain, severe arthritis, I was on 40mg for 8 yrs, 10mg 4×/day. I would have had zero quality of life without it. The only thing which bothered me about it was that I am one of the 5% who has daily soaking - & I mean SOAKING - sweats on it. I could no longer tolerate those awful sweats. Now in transition to another med for chronic pain. The do-gooders & bureaucrats are going to be mighty sorry for the limitations they are putting on chronic pain meds should they one day need help themselves.

    I'm thinking of asking my doctor about levorphanol since he seems pretty against methadone these days citing that it's harder to prescribe in this climate. My concern is it won't be covered and is easily dozens of times the cost of methadone on an equianalgesic basis and its availability is probably extremely scarce so I'd wonder if it'd be accessible monthly without unnecessary delays. Levorphanol, having almost entirely better properties to methadone, really does need to gain clinical relevance. I think it genuinely helps to employ an opioid that naturally functions as a long acting agent as opposed to tampering a short acting agent to be long acting through technology. I feel like with the later there's just more potential ways that the patient may not get the best effect. Then again it's pretty much just buprenorphine, methadone and levorphanol for the opioid molecules that naturally have long durations of action.

    Methadone got me out the horrible addiction I had to pain pills. I'm very grateful for that because I didn't think I would ever be able to get off of them. I started at a clinic 3 weeks ago, they have me at 60 mgs a day and I don't want my dose to go any higher than that I feel fine most of the time except for anxiety and erectile dysfunction which started right after I began treatment I'm doing very well. I'm 46 years old so the Ed thing is disturbing. Glad I'm not married anymore

    Levorphanol is a very rare opioid and it's very important to note its SNRI properties. It's also the chemical racemate of dextrorphan, the active hallucinogenic metabolite of dextromethorphan, and is analogous to butorphanol.

    I suffer from nerve condition in my feet called tarsal tunnel syndrome. It’s chronic, and it’s hell. Norco stopped working so my pain doctor tried other drugs: tramadol, Percocet, morhpine, Dilaudid, and Nucenta - none did a damn thing. Also this year I had my appendix removed and was put on a Dilaudid drip at the hospital - did nothing. Finally my doctor prescribed Levorphanol, but insurance won’t approve it. I’ve been fighting for months. I hope I can get it and that it works. I’m tired of hurting.

      You should try to lower your tolerance. If a dilaudid drip didn't help you dude, you may have a MAJOR tolerance issue. I do too trust me
      The hospitals don't know wtf to do with chronic pain patients like us. Sucks so bad.

      I’m so glad I do not have to worry about insurance here in Australia as health care as good as the best of the best in the US is universal health care any medications I need are no more then $20 a month

    I have a question. I started methadone yesterday, as a short term detox plan from 1 year IM heroin addiction. Before going into methadone, I already tried cold turkey 3 times, but failed at around 40 hours mark, scored, but I'm sure my tolerance dropped quite a bit. So my starting dose is 10ml/day methadone, it holds withdrawal at bay 80%, my aim is to quit totally, not for maintenance. How do you suggest I should go about this plan? What dosage reduction and timeline should I follow? Thank you.

      I tapered from 110mg to 1 mg then off it,took 3 months, eat right and work out hard and as you get dosage down you feel stronger. I had a bad headache for 4 days when I stopped but it was worth it.

I went from 180mg to now 19mg as my place just got the liquid and allows 1mg titration instead of 5mg jumps like before which was brutal.. right now I been on for 14 years and really sick of everything about it but terrified of WD.. I have pain though and I’m reading now Methadone is linked to nerve damage and other things which I have and now wondering if it’s permanent or will go away if the methadone goes away.. if it dosent go away or need pain meds to deal with it.. I’ve been lowering my dose over the past 3 years but Kinda feel as if I’m always in a slight state of WD … I go once a month so it makes it difficult and the 5mg jumps would take me weeks to stabilize .. how fast did you decrease between drops in dose and how was the WD off 1mg? Any advice

      @@pastypatsy7515 I had a week on 1mg then nothing but had a bad headache for 4 days, everybody is different I suppose but 5mg a month decrease should be comfortable till you hit 20mgs then decrease as you see fit say 2mg decrease per week.

I feel even a 1mg decrease .. but I’m torn on what to do .. no one wants to feel like trash let alone make themselves feel like trash.. i have legitimate pain, a lot of nerve pain and im reading now they say long term mdone use causes nerve damage .. so I have no idea if this is permanent and if so then I need to be on it? Ugh .. your only symptom was a headache? No depression or sweating and diarrhea, chills, restless legs, can’t sleep, or eat?

      @@RainHavok1111 bullet to the head. That's about where i am as well. Doctors are terrified to write anything anymore. I have a 12 yr pain management history with full compliance, and i can't find a dr to write meds because i don't have anything they could also do surgery on.

    Well their saying no dependence due to nmda antagonism...but everyone I Kno taking/took it says the withdrawals are worse than Heroin. Contradicting as hell.

      She did not say dependence, she said tolerance. It is theorized that NMDA antagonism makes it so tolerance doesn't develop so assuredly and therefore patients can remain on a stable dosage for much longer without needing to increase it to create the same baseline analgesia.

      ampswithfans. Well your body will absorb 44-99% of the methadone if its liquid. So that will help with long term pain management.

      Methadone is one of the best long term pain management medications out there for reasons like long half life, and the effects on certain receptors. The body has a hard time building a tolerance to it.

    I have bad Arthritis and take 7 perks a day and my pain is still over whelming my dr is like everyone else saying it’s not a long term solution I’ve been on them since I was 16 he say methdone might be a good option I think he only wants me to take that stuff because then I’m not his problem anymore I’d be a methdone dr patient really need some help deciding I’m doing lots of research on what’s safest only bad thing I really get from perks is tired the next day / id call it groggy

      Jesse Ferguson i think yu would benefit from methadone vs taking perc's. Your body will absorb 44-99% of the methadone so it will last longer is one pro for switching.

      the perks have other additives right? your better off with straight oxycodone 30mg's for breakthrough pain, and methadone for 24/7 relief. you can cut the 30's in half if the whole one is to much at one time. I do.

    Okay I'm feeling a little old at the moment because I feel like I'm watching the Olsen twins here it looks like they're trying to talk as if there's some type of doctors or maybe nurse practitioners really didn't introduce themselves on any kind of level they just started to talk but I definitely know I have a son that to way older than the two of them so maybe they're still in medical school I don't know they seem to know about what they're talkin about to a point I guess I will hear it out to the end, this video doesn't look like it has a very good turnout perhaps I'll move along I've been on methadone for 15 years start off at 40 mg until I found my comfortable dose well over 10 years ago 70 I have never surpassed that and don't plan on it so with that being said I am 50 years of age nearly and been around longer than these two people so if you have any questions you're more than welcome to message me back best of luck! I made a conscious decision to get off of pain meds because methadone is a clearer and more safe way to combat pain if you are looking for a high you're not going to get it with methadone!

    Saying methadone is treatment for addiction is like me giving my babies addicting drugs for the flu. Even though we know with a week or so of physical pain and discomfort discomfort. Some love and being there for them would work way better.

      everybody's different in a 20-year addict is not going to agree with you so you may know someone but I can honestly bet that you have never walked a mile in these people shoes or being a quarter of a mile of mine

      @@davidjones2048 your right, I'm not a 20 year addict, I wouldn't have ever made it that long but addiction is addiction. It ruins us in many ways. And it tries to kill us. Some people can use for years and years. But I believe no matter how long it take you or myself I think we all end up with a extremely similar situation. But I understand everyone is different and what works for me may not work for me.

      @@savannah3698 Savannah if you go back and look I'm not the one who made the comment to you , as addiction has consumed a large part of my life I wouldn't want to cut or down anyone at this point in my life , understanding addiction as a disease took some time for me instead of seeing it as just being a weak person I hope people understand that the medication is used to assist addiction not to keep somebody high and it improves our relationships and our abilities to work and be decent people in society without hurting innocent people , so m.a.t programs can be very beneficial for some if they use it as a tool or a maintenance so we can maintain our lives in our relationships , a lot of people will just pop up and say things that they don't understand at this point but I truly hope that you or anyone else on here issuccessful and doesn't go through the horrible rigors of opioid withdrawal so good luck to you and everyone else seems to me that this man here was trying really hard to do what was right in his program sometimes being stereotyped as an addictby the healthcare professionals are supposed to be helping us looking down their nose at is is not helpful to our sobriety I truly hope that he got his problem taking care of cuz trust me it took a lot of work to get that many take and a lot of clean screens
