    I have some questions
    Do you buy the 1.5% NaOCl or do you make it yourself?
    Is it ok if we close the perforation with GIC ? I am an associate and my boss says it’s too expensive to buy MTA :/

      Sorry for the late reply. It only took a year!! LOL!
      You can make the 1.5%yourself by diluting the NaClO with water. GIC is not a good substitute as it’s inflammatory. Your Boss should understand the importance of using the right materials.

    thank you for a great video!!!! quick Q... if there is a perf would you recommend to first obturate (even of there is bleeding) and then fixing the perf with MTA or even the putty?? I am asking because I am worried about the MTA or whatever material we choose to go into the canal and cleaning it wont be easy

      That's why we now developed the BC Liner, which is dual cure material that goes over the Putty after perforation repair and can be bonded and allows for the proper sealing of the perforation immediately. You can then proceed with the root canal. Here's the video. ua-cam.com/video/xhFtg-lil_Y/v-deo.html. If BC liner is not available where you are, then place some BC Putty in the hole knowing that most of it will wash out over the course of the procedure, but it will control the bleeding and you can then repair a second time at the end.

    may I get your opinion on warm vertical vs cold lateral condensation? pros and cons etc.?

      This is a long answer. Maybe a video would be best to answer. But basically, they both do the same thing when using conventional sealers; you can get away from using either one if you're using some of the modern sealers (bioceramics).

    Hello Dr Allen ,Please I wanna ask you about negative pressure inducing pain during insertion of gutta-percha cone while the patient is not under local anesthesia ?? Or Reasons of pain during insertion of gutta-percha cone not under LA ?? Pain is during insertion and removal of the cone , ...cone is to the full working lenght ,not exceeding not under .All procedures of cleaning,shaping and irrigation is applied ?? Thanks 😊

      Mina Moheb sorry for the late reply but I made a video a while ago about pain during obturation. It’s not negative pressure. It’s actually positive pressure but the reason for pain is loss of Pikos’s anesthesia at the last phase of treatment. The difference is between tissue and pulp as anesthesia. Cheers!

    The bioceramics are highly soluble although easy to work with.MTA is still the material of choice.

    So no rinsing with sterile water or anything like tht after removing the NaOCl pellet? I'd be afraid to leave the bleach in the PDL tissue without removing it. Perhaps some NaOCl does reach the periapical tissues during standard irrigation of canals, but I always believed none of it would actually extrude beyond the Apex when using a proper side vented syringe. Maybe a small amount is OK on the tissue.

      NaClO is buffered within 3min of contact with dentin or calcified tissues. It's not a big concern. A bigger concern is if your saline is clean enough. I think anywhere from 1% to 5% NaClO will be safe enough when simply placed under pressure with a cotton pellet. Cheers!

    Sir please suggest fast setting putty for perforation .
    Can we use MTA as putty material in perforation?

      Yes, Putty FS is the best option for perforation repair. And yes, MTA can be used too.
