Local recurrence after Total thyroidectomy better be treated with US guided RFA or PEI.?
All said and done, the connotation of “Guru” is testimony to the fact that the affable lady belongs to our part of the world, the subcontinent.

The complications of total Thyroidectomy may be no more than partial in the hands of experienced surgeons but such experience is available only at a few high volume centers. Most hospitals where surgeons perform thyroidectomy no more than 5-10 procedures yearly can’t be considered high volume centers. The potential complications of TT ie RLN injury and hypo parathyroidism and its affect on the quality of life will offset any advantage it may have. Besides the need for lifelong thyroxine and potential harm as a result of RIA May further confound the problem.

The debate “ less is more or the more the better“ continues because of the bravado of the celebrated surgeons from the centers of excellence who proclaim that the morbidity of total thyroidectomy in their hands is no more than hemi thyroidectomy done elsewhere.
