I once used it on a poster ad that needed some help, and I hand painted a DOF matt in PS ( Lots of foliage!) for processing in AE and it really tied the composition together.
This tutorial is Awesome! I've been trying to mimic the Tilt Shift look, like the new Love, Death and Robots episode Night of the Mini Dead....this tutorial is the closest I got. Thanks so much and I love to see you take on real Tilt Shift effect. Thanks man!

I'm not sure there's a "best" way. From a 3d application it's automatic with a depth map. In AE if you're using 3d layers you can just enable depth of field on the 3d camera. If you need to take it with a custom depth map, then I would try doing it in as nondestructive of a way as possible. Like solid layers with mask paths. This way you can easily adjust the paths and feather out specific areas.

Using RunwayML, you can use AI to create an automatic depth map calculated from an image, or even a video- and it works pretty dang well.
