I'm glad you changed the base for the engine it looked messy with all the leads showing the only thing I'd change would be USB charging for the batteries still excellent little project

What component is actually doing the loud clacking noise?

That is the cam actuating on the small roller bearings on the valve rockers, the cam has a agressive pitch causing the clacking noise.

Is the piston water cooled. Is that what the metal hose on the bottom is for and the top one feeds back to the water holder at the back?

Yes the piston is water cooled. It has a small water pump for the coolant

Lots of room for improvement that flywheel would be on my lathe immediately and some work on the wiring layout and coolant lines.
