799 LS heads Mystery

I found a pair of LS 799 heads on a junk yard, but it came with a little surprise. I been thinking about how was this even possible, but I can't get my head around it. Do you know how that happened? How does that engine had got a valve failure and destroyed the piston but the chamber on the head still intact?
