- Shoulder Clinical Examination - Medical School Clinical Skills - Dr Gill
Personally, I find the shoulder examination the most complex examination possibly as there are so many variations and special tests. Some of which overlap and some will relate specifically to a patients presentation.
Often in a medical school syllabus, only select special tests will be used. In this shoulder exam demonstration, we include the Hawkins-Kennedy Test looking for impingement. This is dovetailed with examination for bicipital tendonitis as this is another possible cause of impingement type symptoms.
This shoulder upper limb exam follows the standard "Look, Feel, Move" orthopaedic exam approach, and overall order as set out in MacLeods Clinical Examination
Watch further orthopaedic examinations for your OSCE revision:
The Spine Examination:
• Spine Examination - Ba...
Knee Examination
• Knee Examination - Ho...
Hip Joint Clinical Examination
• Hip Joint Clinical Exa...
________
Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However during OSCE assessments. Different medical schools, nursing colleges, and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.
The examination demonstrated here is derived from Macleods Clinical Examination - a recognized standard textbook for clinical skills.

If it's not a sleep study, we got problems.
I’d probably fangirl all over him lol
@@coseleygirl shameless
We’ve had one patient commented about having seen a video first.
Me: oh that’s cool
Them: yes, meant I knew what you meant when you asked some questions.
Then the consultation carried on as normal thankfully 😊
@Alt Account "Oh I didn't realise you were here for diarrhoea!"
The way this man can do asmr whilst actually doing a normal exam amazes me
That’s because he’s not “doing ASMR”. It’s an accident people get ASMR from his voice.
he has a naturally very relaxing voice without trying
It's unintentional asmr, which is significantly better than intentional asmr videos for sure
The way you can say "whilst" in a sentence
@@kookieshoe4669 This. In my opinion, people from the British Isles tend to have more pleasing voices, than people from the States.
I am impressed with the respect Dr Gill shows the person he is examining. He always asks permission before starting the exam and before touching the patient.
consent is a huge thing
Fantastic..hi from Italy Dr.Gill😘
@@DrJamesGill watching other videos after yours is sometimes a little jarring.
Tbh, that shouldn't be impressive.
That's standard procedure...
If Pat didn’t get all those bad head injuries playing Hockey, we’d probably never have this community. Thanks Pat, sandwich breath and Dr Gill. The three horsemen.
So glad these masks finally solve the issue of sandwich breath.
I still don’t understand this 🤷♂️
@@DrJamesGill Examination of Pat LaFontaine by Dr. James Kelly.
They make you more red in the face though
Real tight, like you got soap in them.
FIGHT FIGHT FIGHT FIGHT FIGHT!
what a legend
🤨
@@DrJamesGill i mean, you are :3
@@DrJamesGill your one of the best
My girlfriend is a medicine student and i am an engineering student. She finds your material incredibly helpful and i find your videos a very stimulating. We both agree that your contents are a well-made source of information about medici and the human body!
Thank you! That’s a great comment. As we (eventually) move to the orthopaedics bit, we’ll probably touch on a little more surgical engineering 😊
@@DrJamesGillthank you so much for your kind reply! I definetely look forward to it!
After all the funny comments I would like to get a little more serious. I was in a detox in november after I relapsed with opiats. Your videos and your calm attitude and voice helped me a lot while I was going through a hard withdrawal. Today I feel a lot better and with every day I get a little bit back of my body. So thank you for spending a little bit of comfort through some of the hardest days of my life.
All power to you. Thank you for sharing your story. Keep on moving forwards 😊
If you need help, please reach out 😊
Same here, the nights were brutal. These videos helped me too. Blessings to you.
@@lindar5413 It is hell indeed and there is no easy way or a shortcut, but it can be done.
Stay strong dude
@@enbykeithThanks! I am staying strong. I'm clean for 6 month now and it feels good. Right now I am in the 9th week of a 6 month rehab. It is a complete new start for me. It feels good to fight and starting to win the battle against the demon of addiction.
Kinda glad he's getting attention from ASMR enthusiasts because it seems like these videos just get more and more relaxing
“Push push push push” is the new “fight fight fight fight”
Fight fight fight fight cant be replaced its iconic
@@mateovidakovic778 think of it as one passing the baton on to the other . No replacements needed. Fight fight fight fight’s legacy shall persist through countless eons.
Tatakae tatakae
I came here to say the same thing
Watching people talk to each other while wearing masks has a real Power Rangers vibe to it.
🤣 never thought of that!
Totally! LOL
haha power rangers is the new norm
I came on here to say this too 🤣 been watching so much PR with my kids lately
Brooooo you’ve articulated the feeling I’ve had for the last year!
Dr Gill is just one of those chaps in which you’ll do whatever he requests during an examination.
“I’ll just pop this paint brush in your hand, okay now raise it up...and back down again. Fantastic, continue doing that along the entirety of the wall with this magnolia paint”
🤣🤣
I trained to be a medic. And since discovering your videos (old and new) I have been able to add to, or adapt my knowledge and skill set and have watched your "bedside manner" and interaction with patients. Excellent.
Admittedly, the sterling quality ASMR has also sent me to dreamland a few times too.
thank you, from a medic that is a huge compliment!
Any suggestions as to how we could improve them?
I would reckon it is very difficult to improve on perfection. I enjoy the realism of your examinations and videos. Look forward to seeing many more in future.
Cheekily, I do look forward to hopefully seeing perhaps a peripheral vascular, examination, cranial nerve or diabetic lower limb/foot examination some day.
Can you imagine being examined by the doctor Gill? Something just incredible. And this goes with all the respect in the world.
All these comments are so respectfully wholesome. It makes me smile seeing so many people hyped out of there minds for a simple shoulder examination, which is pretty wild when you think about it
People really engage with learning, even if it isn’t their field.
I’m a medic, but still enjoy Cosmos as an example 😊
Cosmos 😍🔭
@@DrJamesGill Oh I LOVE Cosmos! I was so sad when they removed it from Netflix 😥
Now I have nowhere to watch it without paying for another streaming service
If I'm the guy he's examining, I'm afraid I'll just fall down asleep because of his voice.
Im here since dr gill has 3k subs because i saw a comment saying that dr gill has his own channel. Thanks god for that comment 🙏
👍
Friendliness is so underrated
I think I get more excited with his new videos (for the ASMR) than actual students. Helping people anyway, Doc. Thanks and hugs from Brazil!
Glad you like them!
@@DrJamesGill I even did the arm moviments together to check my range :)
Hahahaha. Well, it’s good to check I suppose 😁
I always think with these kinds of examinations that it would be way more helpful if the person being examined actually had something wrong with them so we could learn what signs to look out for and how it's diagnosed in real life. For example doing a test like this on someone with scoliosis or some kind of shoulder pain.
That has issues with patient confidentiality
Yeah me too. Since I have all kinds of pain in my shoulders and back
Put a bit a paper. I just felt the need to say that since I didnt see it in the comments after scrolling for a while. So there it is.
I'm training as a nurse and it's fascinating to see all the different jobs different staff do 😁 Great informative video, Dr Gill! Thank you :-)
I can't tell you how helpful these videos have been for me to learn how to have a good bedside manner. While I like to consider myself an empathetic person who enjoys my time with patients, I also have probable Asperger's and a good "flow" doesn't always come naturally to me. Watching your interactions continues to help me develop that. Cardiology is my area of research and study right now, so I can't wait for the cardio Deep Dive! You're doing good work, Dr. Gill, thank you!
Thanks for the comment me. With a similar Aspie brain, I literally LEARNT it.
With my best friend at medical school, we LITERALLY “questionsmithed” working out how best to phrase questions and tones of voice.
Hard work, and something I’ve always been jealous that some people can do naturally
Gill's whispering is celestial!
I love this sort of content. I may be rather young to start thinking of becoming any form of medical professional but this sort of content is very interesting, sometimes even finding myself following the motions. Much appreciated!
Love the examinations videos! Looking forward for more ❤️ it’s very relaxing but also useful to learn. Thank you x
Him: 1991

As someone who turned 30 last year (1990) - im glad you guys thinks that's young

This is so cool! I'm applying to med school this year and your channel is such a lovely resource, I feel like it's providing some much needed insight! I love how soothing you are and how encouraging you are even with simple tasks like moving an arm, it creates a very safe space and a safe dynamic. I hope I can be like you when I grow up
Good luck!! Where are you applying?
@@DrJamesGill Hull York Medical School is the main one!
I know it isnt at all what this was intended for but man this guy can really do some amazing ASMR, awesome work sir!
I absolutely adore how many times I've spotted dinosaurs on your outfits (ties, pockets, etc.). It adds even more enjoyment to the videos and makes you even cooler (I didn't even know that was possible)! Hope you're keeping safe and well :)
Glad you like them! It's my own way of trying to generate smiles with the patients (and me)
@@DrJamesGill Well, you can add me to the list of people that it makes smile! :)

Dr gill thank you so much , i sleep everyday cuz of ur clinical examination tbh its kinda good :)
Dr. Gill has a master degree in unintentional ASMR!
Please keep using this patient for future videos ❤️❤️❤️❤️❤️
I loved the triceratops pockets! As always, wonderful video. Thank you!
A friend made the scrubs at the start of the pandemic when we had trouble sourcing them. I absolutely love them
Holy shit. A new video to fall asleep to. And i mean that in a good way.

The man, the myth, the legend of ASMR.... Dr.Gill
Wow, what a body. I’m here to fall asleep Dr Gill not to thirst over patients 😂
Behave!
I know right! Lol

I was anxiously looking for the release of this video
I love Dr Gill, and I find the patient who appears in various videos very handsome. Hopefully we will see it in more videos.
This doctors voice is phenomenal....I wish he could just talk to me all day, everyday, for the rest of my life
His voice could send the anyone to sleep ✌so soothing and calm❤ legend
Your videos are so calming they are like asmr but better I fall asleep so quick
Back in my days I translated in Italian an university book about this exam 😊😊
Wow!! I’ve always been amazed by people who have that language skill!
I’d like to move to Europe, but don’t feel it would be safe for me to try medicine in another language
His voice literally makes me neck tingle for some reason :D

T-Pose Dr. Gill: 4:11
Hopefully this gets covered in more detail, in one of your a-z videos. I've had a lot of shoulder problems in the past, most recently only a few weeks ago
We will - eventually, but it isn’t a topic I’m keen to approach as it’s flipping difficult.
Although perhaps means o should get to it sooner
Thanks dr gill ♥️
Dr Gill is the consent KING
Good video. Will be great to see you explain what you are testing and what would abnormal look like.
Trouble with masks is your brain wants to believe the audio and video isn't synced but they are :p
It makes my job fractionally easier for the same reason!! 🤣🤣
Power Rangers.
why this video is so relaxing and satisfying
I just love you so much !
SO POLITE !
Awesome as always.
The best doc in the world i wish i can visit you
Excellent video, my congrats!

Educational asmr is the best! Thanks Dr Gill you help me drift off to sleep most nights.....😊 hoping my subconscious is retaining some info aswell lol
it's nearly 3am and i can't sleep so i'm watching your videos :)
These videos really help anxiety. Thank you
Hopefully they are useful for knowledge as well
We should do a petition to put comments back on for the asmr vid pat lafontaine
I'd love to see them! I only discovered the video after they had disabled the comments unfortunately
David Rogerson is a snack though.
Chances are he read your comment. Lol.
Only problem with this video is that’s it’s not long enough!

Great content Doctor
The only bad thing about this video is that it's only 6minutes long :( Dr. Gill is a genius! It's really cold here and my body was very tense, but after this video I finally managed to relax my body.. but now I want to go back to bed and sleep lol
Great video as always! Just a suggestion; perhaps in one of your videos you could get one of your assistants to ask a question at the end of the examination? It may make the scenario look a bit more realistic. Just a thought 🙂.
YOU'RE A SAINT !
if there could be a league of ' dr. Gill type doctors ' that people could find , maybe I wouldn't hate going to the doctor ! are there others like you ( especially in New York ) ? and if so , how do I find one ?
Estaba esperando por esto
After all these videos, I think I’d like to see one where you show concern when they can’t perform a function on one of the tests and how you’d proceed with the rest of the exam.
The examination would remain the same. You are doing a complete work up. There are a few special tests, such as checking for ascites in the abdo patient that you only really do if you suspect it
I've missed these types of videos
Are we not gonna talk about the scratches on his back lmao
great job as always :)
Thanks for information
If I wasn't all in for a new Dr Gill video I was ABSOLUTELY all in when I saw those dinosaur scrubs.
Great Dr.! Would love to see a video about cardiac auscultation and chest x-ray !!

Just fractured my humeral head the other day. Have a feeling I'll be experiencing this exam quite a few times in the next year!
Oh dear. Those are nasty injuries.
Hope you get better soon!
If the pain relief sufficient currently?
@@DrJamesGill Thank you! Turned out to be comminuted. Pain was a solid 9/10 on entry, and is now a manageable 4-5 with painkillers, though can spasm to a 9 every once in a while. Surgery tomorrow afternoon. Hope the doc knows how to make easy work of it!
That’s good. Bone pain is nasty. Shattered my elbow a few years ago.
You’ll need to focus to diet when you get out, a lot of calories and minerals to help to repair
