How can I obtain a sample of a civil statement of claim in Ontario? In Small Claims Court there is a plaintiff’s claim 7A that you can fill but in civil court should I use the form 14A to start with or just follow your instructions to e-filing ? I have requested for Default Judgement online since the defendant didn’t serve there defence in 20 days. While filing for 11B online, no where it mentioned that I need to upload the form in PDF form. I have paid the fee and current status is submitted. What should I do? Can I email them my form ?

Great Video. If you are the moving party filing an urgent motion 14b with affidavit 14a (family). Do you have to wait until the court "approves" it before serving the other party notice?

Thank you for your question Poonam! An excellent resource is Steps to Justice. They say: "After you get your divorce order, you can apply for a Form 36B: Certificate of Divorce. This certificate confirms the date that you and your partner legally divorced. This is usually 31 days after the date of the divorce order. You need a Certificate of Divorce if you want to get married again. To get a Certificate of Divorce, you must go to the same court where you applied for your divorce and take a copy of your divorce order. You have to pay a court fee of $24. You can pay by cash, cheque, or money order made payable to the Minister of Finance." @@markiss9048 I did as a matter of fact .. once your filings are prepared and affidavits commissioned .. all documents must be scanned to a file and the file sent to the Clerk at your local courthouse.. they file documents ... seal and send you a copy of status which will now have your new court file number attached :) .. :) hope that helps .. there is a directory for the email addies online for your jurisdiction ..
