    If everyone had a science professor that was able to elucidate complex concepts as well as Dr. Bikman, I can guarantee you that we would have a lot more people going into STEM professions. Having listened to hundreds of presentations from low carb physicians or researchers, none do a better job at painting a comprehensive picture of insulin resistance better than Dr. Bikman. He is a credit to academia. Thank you for having him on your show.

    After 9 months in ketosis I was getting very concerned with my fasting glucose 101 to 112. This video has provided some great insight into what is probably going on. Thank you

      Ok, good. Me too. Fasted 33 hours, glucose went from 100 to 91. Ate a steak, six hour later it was 107. But that was at 4 AM.

      Was "glucose sparing" the term he used? If I understand correctly, it's not particularly concerning, because insulin levels and insulin resistance are low.
      I think Dave Feldman has come to the same conclusion regarding slightly-higher fasting BG in low-carb adherents.

      Saxgirl Hornboy after 18-20 fast noticed my glucose levels went up . Usually 85-95. Fast it goes up to 110-115 . I run a CKDProtocol weekly.

      I'll give you my "whole picture."
      I think there are four pillars to biohacking that will deliver superior health.
      Fasting -and Exercise (catabolic)
      Eating and Sleeping (anabolic)
      The rest is quibbling about what and how much.

    One of the things I have noticed is if you Google testing Insulin or something similar almost every link will be about measuring Glucose levels vs actual Insulin levels. Heck even my last doctors office visit a few months ago I asked for a HOMA-IR test. He'd never heard of it and their in house lab(Quest) didn't seem to know what it was either. I was floored. I was able to get a C-Peptide done(I think). Still trying to interpret the results myself.
    Also something I learned "Metabolic Syndrome" used to be called "Insulin Resistant Syndrome" among other names.

      Catherine Croft in her PhD thesis found that a 2-hour postprandial insulin assay was a better measure of IR (compared to the. Kraft OGTT or HOMA-IR). So maybe we only need to get our doctors to write orders for insulin tests.....

      My understanding is for a HOMA-IR you just need a fasting insulin and a fasting glucose taken at the same time. You plug the results into a calculator to get your score. There is no specific HOMA-IR test that a doctor can order, which would explain why they don't know about it. Here is a link that may help. www.thebloodcode.com/calculators/

      Next time I'll ask for a plan old Insulin test or whatever it's called.

    Had it, barely had symptoms. I attribute that to how good my metabolic state is right now, thanks to discovering this healthy way of eating four years ago. Had loss of smell, reading and hearing from other people how it took between 1-3 weeks to recover it, for me after 3 days it was completely back.

    I am one of those who are long term keto adapted for years, but insulin floats in the low 3-4 range and typically have 12hr fasting glucose around 100. I am also doing the Levels cgm and interesting to see daily/weeks view vs point in time finger prick glucose readings. I hope they will extend or FDA finally make these available to general public affordably.

    Thank you so much for sharing this knowledge. I sincerely believe this has saved my life as I’ve been so sick of constantly exhausted for years, despite T2D normal blood glucose and A1c on keto and even carnivore. Recently experimenting with fasting (12 noon/ 6pm) and very low carb feeding window. Amazing results from first week: 11lb weightloss after complete stall for over a year, palpitations and dawn effect (which continued despite normal glucose) quickly reduced, nocturia from 3+ x /night is now once around 4-5am !! Energy levels slowly improving, mood very stable now. And so on ... Now hopefully TIAs and memory loss/cognition issues may improve with time. Thanks again.

    I recently discovered that I was highly insulin resistant. I have since adopted a Keto diet. I found that when my insulin was high an old neck injury became extremely painful and I believe this was due to the inflammatory properties of insulin in the body. Things are much better now.

      Sai Sumanth I wanted to get a HOMA IR test here in NZ. The lab had never heard of it. So I tried to fashion my own doing blood tests for Glucose & Insulin levels pre-food & 1,2&3 hour post-prandially. Even this request was messed up, tho paid for..... I give up, I know I am very Insulin resistant due to PCOS. And I try my best to desl with it with diet. Started years ago with low fat, low cal. On to Low Carb, Keto & now. Carnivore. Plus Interrmitant fasting daily. I add 24 - 70 hrs fasting when I can. Due to very high Insulin levels despite my diet, hunger usually wins....😐

      @@yvonnewagner9833 Multiply glucose by insulin, then divide by 405 and you get your HOMA-IR. 1.0 and below is insulin sensitive.

    True 100% I am suffering for almost 6 years from Hypothyroidism, lack of TSH, now.. one of the intelligent Turkish doctors has discovered, diagnosed my body problem which is lack of Insulin resistance unfortunately. I am very sick but finally. I knew the exact problem. I was suffering all the time until I got severe sickness and My ESR has been increased unfortunately. Amazing book and doctor. Dr. Ben. Keep it up.

    Interesting around 1:03 and 1:04, I had rock bottom insulin levels when I was carnivore and when I ate carbs, I couldn't handle carbs and sugar just shot up so much if I checked it with a glucose meter. ANd now I know I was very inflexible with handling glucose. Unfortunatley during covid lockdown I kept loading it and wasn't clearing it. Got chubby quick again. But back to keto carnivore and fasting to get my insulin back down.

    Evidently Bikman and Naiman agree exactly on fat and protein...
    1:1 by gram (not calorie) as typically appears in fresh whole animal product.
    I think the reason protein became so contentious has a number of elements...
    1. Vegetarian and vegan ideology; particularly that (excess) protein causes cancer, diabetes or kidney disease.
    2. The fitness industry; particularly as they emphasized protein and carbs and downplayed fat. (thus causing an unnatural association between protein and carb which is insulinogenic/anabolic and with potential for metabolic issues).
    3. Longevity proponents who are obsessed with M-tor (connected, perhaps, to vegan ideology).
    Yeah. We need to get away from the above that has been unduly influencing scientific rigor with agenda or ulterior motive.

    I see the South Beach diet now has a Keto South Beach new book. My old school cardiologist liked the South Beach diet , Dr agetston, inventor of the CT scan and author admits he didn't quite get it right on original book. Let's hope his support of keto convinces more doctors

    I have been Keto for a year and I glucose spare all the time. My glucose never goes above 120 but it is also quite stabile after a meal. It can take a long time to come down from a muted peak. I am eating more protein so I would like to better understand the glucagon factor.

    I am in remission from prediabetes but still have mild IR given my HOMA IR. Still working on that but worry that I would still suffer with COVID. At goal weight with good muscle mass, but will be low carb and IF for life.

    Dr. Bikman didn't really answer the question about blocking aromatase in men. I was most interested in that question. I am obese and working on increasing my testosterone. But my estradiol is now up to 68. I read that 20-30 is optimum for men and anything over 60 is getting into a territory with risks of heart disease and stroke. So, I started an aromatase blocker this week to try and bring it down. Once I get my weight under control, I'll stop. Hopefully all hormones will stabilize at that time.

    I think everyone needs to clarify that they are referring to the problem of treating glucose with medicines rather than diet. I don’t believe there is a such thing as treating glucose with diet where it doesn’t also resolve insulin.

    Low insulin causes severe electrolyte dumping. I started adding carbs before bed to avoid heart palps and Charlie horses. Also, low carb lifestyle for the last two years made me hard to sleep.

      I sometimes take magnesium glycinate before bed and that takes care of the heart palps and muscle spasms in my calves. Magnesium also relaxes me enough that I sleep pretty well also.

    If I am doing OMAD (keto or carnivore) what is the best strategy of timing the meal and workout? And could it be goal specific (e.g. gains, autophagy etc.) or type of workout specific (strength or cardio)?

    Relying on a piece of fabric as your first line of defense without a back up plan to COVID-19 is one hell of a leap of faith. Improving one’s health will pay the greatest dividends to reduce the severity of the flu.

      Both are important. Wear the fabric to prevent the spread but optimise your core immune system so that your body doesn't fail when fighting the infection if it happens.

    57:03 fat helps protein work better, "when you stack protein and fat together it was more anabolic at the muscle then protein alone". Can someone please explain to me if he is saying "by eating fat and protean and low carb it helps make muscle building better, so for example a body builder eating only protein and fats = greater gains then protein an carbs? I think that's what he meant and want to get that confirmed.

    I have been wondering about carbs in liver. It is in the form of glycogen. Do we have digestive enzymes that can let us even access this? Is it available as a nutrient? When is a carb not a carb? When you can't use it.

      You don't need enzymes to break down glycogen. It just needs a bit more time than glucose to be metabolized. Yes, glycogen is a nutrient. We can always use carbs and sugar to run our body. But we should not use it all the time.

    Men NEED more aromatase when then are eating low carb or fasting because your liver will eliminate estrogen very quickly and you need some estrogen to have a healthy heart, brain and libido.       ​@@davidplyler8173 I always laugh when people ask for a link to basic physiology. Get some books and start reading. Science does not work by 'studies' they are essentially just statistics. It works through experiments and direct observation. You are going to have to read if you want to know what you are talking about, or reread if you already learned about the endocrine system and have forgotten.

    Given that fat is pro-inflammatory abdominal liposuction could be apart of treatment. I had lipo of the abdomen (it would not go away no matter how much I worked out and dieted) and now I notice my whole body is getting even leaner (aka, just less swollen)! I used to be so confused about my body because I ate well and worked out but I could look at bread and swell up like a balloon!
    This stuff is complicated! But I keep trying!

      That’s great Suzie Q, but as long as you realise that even now if you eat bread etc’ you will end up back where you were pre-lipo.
      Secondly Lipo itself cannot remove dangerous deeper visceral fat..... Only adherence to dietary/exercise principles can do that.

      Suzie Q Cool, whatever works for you then do 🎉 Unfortunately simple endeavours don’t work for for me as they did 20-30 year’s ago. I need much more extreme measures. I had abdo lipo when I was 40 yrs old & kept to a Low Carb diet - not enough. I put back what was removed + more 🤗 Just be very aware always is my advice & long may it last 😁👍😎

    Crikey - there is intelligent life on Earth after all ! Who knew ? I really wish that people like Steve were intimately involved in healthcare policy here in the UK 🇬🇧. We have had the NHS ( National Sickness Service ) here for 80 years - result a very decrepit population that did very badly regarding Covid . Obviously the NHS has done great things and given a lot of poor people access to healthcare , but overall it has done poorly with regard to creating a robust healthy nation . Prevention equals early detection at the most . It would be great if doctors here could screen for insulin resistance but testing for blood insulin levels is one test that NHS doctors cannot request on the NHS . At the present time the ' cure ' for the obesity epidemic here is ' eat less , move more ' ( wrong on so many levels ) ' , or ' fat shaming ' ( 😱) .

      It depends on what type of carbs are being consumed and also consider the pathophysiology factors involved. Understanding the biophysics will allow one to get a better look at the bigger picture. Biochemistry just scratches the surface when it comes to function.

      @@daves1107 Also we ignore the gut microbiome for the most part. A lot of cultures also consume high carb and low fat, and vice versa but now we know that these groups of people have different microbiome. A microbiome that is healthy is more important than just macronutrients albeit, we need to understand that the quality of the macros are important. Highly processed macros like distilled products like sugar, or msg wreck havoc on the gut microbiome, and we now know that the gut microbiome is so closely related with the immune system as well as regulating body fat mass. Plus, every individual is different, even if they belong to a group of carb eaters like the Okinawans or fat eaters like the French. We need to go towards tailoring diets according to the individual but that's the most difficult part.

      Problem is leadership and the media. Like conventional medicine, the ones with the loudest microphones are pushing for the magic pill shortcut. Big pharma and big food make large donations and pay media companies a lot of money for ads and to make sure their bottom line (profits) does not get impacted. Statins for example also increase risk of having complications or dying from covid. And yet, the studies about statins and covid do not get any attention
