One thing is that is wrong is that Jaime loses his hand in ASoS, but he has a fakeout death in ACoK too, when Catelyn confronts Jaime in Riverrun's dungeons, the chapter ends with Jaime antagonising Cat, and then her calling for Brienne to give her a sword.
I'd argue Stoneheart is way more of a fake out, or at least surprise, than the Ned LF fake out. Both ends with a blade on their throat Robert Strong as well, the revalation of Qyburns work, a fate worse than death, is some good grrm shit. Similar I'd put Theon becoming Reek higher. When he's captured and tortured and then the world thinks he's dead you're like, well he's a giant dick, but then books later when you read that Reek chapter and slowly figure out it's Theon and what the torture did to him... Okay wait, all the pronunciation guides I’m seeing are saying it’s GRAY-ning (like Walter Koening is KAY-ning). So, not the German ö, but the English pronunciation of the German ö sound
We also just categorically decided to exclude all of the characters that actually die. Not to say the Beric/Stoneheart/Mountain reveals aren’t awesome, just that they were all originally categorically excluded

Beric dies and is revived within the same chapter, Cat is confirmed dead at the same time she's confirmed undead (if you don't count wolf dreams), and Gregor isn't actually confirmed dead or Robert Strong at all

I dont remember if it was a canon or semi canon source about Rorge and Biter, but Rorge seizing an orphan off the streets and viciously training him to be a pit dog is genuinely evil, and Biter being essentially an animal kind of absolves him morally...
