On the first attack on the rifle squad in tile 14, did you forget the cross fire bonus? You rolled five dice: one for rifle G, one for rifle A, and four for the machine gun B, minus one for trench. Shouldn't it be one more for crossfire? Or am i missing something? 14:45

Yes, correct. In fact I rolled 7 dice because the SGT fired too, but I should have added another for Crossfire. Good spot! Thanks.
