I dont think shes real. We know V can cause hallucinations, we know Homelander has all of the compound V from what A Train said. I think Hughie injected it but imagined he didnt

Her and Hughie both communicate and interact with doctors together, which doesn’t happen in scenes where Butcher is hallucinating.

Maybe this show has just made me paranoid but I'm almost certain Hughie's mom will later be revealed as an antagonist, she's already earned plenty of ire by being an astoundingly shite mom and I think that's the shows way of prepping us for the reveal. I really doubt she used the V on his dad out of the kindness in her heart.
