I think you should do another video on how a PM can avoid these causes of failure. (If you did that, if you gave us a "freebee," I think it would ultimately help the sales of your software.

Someone asked today what is a project specification - I normally don’t have C anything to do with documentation on my projects - I write the design and give it to junior developers to write up. But would be great to know the different documents at different points in a project and what point they are created / used Thanks.

I say most project fail, because many companies way to confident about theirs requirements, requirements change and to figure it out what is important and not important is hard, this way you need to have a external consultants to make sure that you do the right stuff, consultans would work like early failure prediction system, but companies say we dont have money for that, well they earned the failure yeah ha!
