      can this affect you for 8 years? I have documentation that says "a MRI showed tendinitis/tenosynovitis involving the flexor hallux longus. It could represent possible partial thickness intrasubstance tear vs tendinitis or teniopathy"

      From the center of the calf to right under the heel where it feels like a marble, all the way to the ball of the foot, but not into the toe. Also when I stretch that leg in split leg hamstring stretch, I can see the tendon protruding more than anything else in the foot.

    Only problem with the foot drawing is you have the FHL shows as running along the side of the foot and toe...it runs along the bottom of the toe and between the sesamoids!

    I think this is what I have. I danced ballet for years and it’s catching up with me. It started off with posterior tendinitis in the inside of my ankle, 3 years later, it spread to my big toe, then the bottom of my foot.

    Thanks for this! Especially the drawings and graphics, I finally feel I can explain to someone what I am feeling. I had a L5-S1 disc rupture which caused severe nerve damage in my left foot causing numbness along the back and outer edge of the foot and last two toes. This caused about a year worth of limping, drop foot, and lots of PT to correct. However, since then I’ve had tons of foot spasms locking my big toe and general pain along the side. It makes a lot of sense to me that I spent a year walking on the inside big toe of my foot to make up for pain and numbness on the outside. I think this has caused lasting effects on the abductor hallucis.

    OMG! finally the explanation i've been looking for. Its been about 5 weeks since the first time my toe locked and the pain was so bad i almost cried. I can now assume it was from Muay Thai. pushing off my left foot / toe. I went to the doctor and they had no idea. and sent me for xrays lol.
    How do I know if its os trigonum syndrom? Because I can manually bend my toe with no pain.. when i push my heel up and point my foot and manually force my heel up with my hand, I feel a pain above my ankle.
    also I have a question.. ( i hope you can answer) the top of my foot seems to have a bit of pain between the bones that connect to my big toe and the one beside it.. feels like a bruise. And my pain seems to be way worse in mornings.. and then almost no pain at night after lots of stretching

    Nice video. I had pain in the feet. With some swiming, and with time i felt pain only after hours ot walking (not always). But i used some wrong shoes and it came back, and after that i did some working and it became really bad. My ankle is swollen, and i feel pain immediately when i am standing. The doctor said i have FHL tenonditis. I have some questions. Firstly, i have supination in the foot, and the doctor suggested to make the right soles. But if you have supination or over pronation is the FHL tenond over used?

    yooo i have ben having feet and knee problems for a long time and last night i was doing some exercise were i had my feet up agains tthe wall and found this exact spot on my foot HURTING SO BAD when id bend my toes glad i found this so fast

    This video explains exactly what I'm dealing with. My orthopaedic doctor just broadly explained to me that I just overworked my foot (10 weeks post ankle surgery). But I'm the type that writes everything in a journal and jot down exactly where I'm feeling the pain. So I'd like to know what part of the anatomy got affected so I know exactly what not to do and what to do. Thank you so much for the education! This all makes sense

    started having pain almost 10 years ago. Had lots of I juries due because of ice skating This year I went to my Dr because I cold t stand the pain. She did an X-ray and the ortho prescribes shoe inserts. I know I need more than that. I’m so much in pain in my ankle.

    I have FHL pain in my mid foot! I had pretty severe fhl tendinitis in the area near the sesamoids about 5 years ago (diagnosed with MRI) and after three years away from all activity could do some moderate activity for a year or so, until it started hurting again, but in the midfoot below the ankle joint (2 years ago). So thank you for pointing out that it is possible to have pain there!
    Currently trying my best to let it heal with rest / heat / collagen supplements / insoles, but it's a sloooow game
    Would love to know some exercises / eccentric strength to work on to supplement!

    Please help. I have ALL the symptoms of this. but i had my foot ran over and ever since, it's been like a year, i have had huge issues with my foot. is it possible i did something to my flexor hallucis longus? does not make any sense to me at all. would really appreciate any input as i cannot afford to go to the doctor right now.
    edit, forgot to mention. the pain is mostly in my top big toe joint, feels like a deep ache but it sometimes seems to move around my foot and up to my calf and my calf feels tight. and it sometimes tingles or tingles very easy

    Help please! What do you think aboutr arch support bandage with pad in arch? I have problem: tendenitendonitis of the flexor hallucis localised in arch... There is like a "pea", which becomes smaller if you massage it, but a few minutes after the massage it reappears, it hurts if you press it with something hard like a pencil...

    hi, I have pain in my big toe for 4 days its looks like Gout first, then i see this video Flexor Hallucis Longus PAIN [FHL Tendonitis BEST Treatment 2021!], and I this that could be it. can really bend my big toe it's too painful. it happened all so fast . it's swollen and very painful but if I st stretch it its seems I feel better. and I feel burning back and front of the ankle. I work where I stay the whole day and few hours down on my knees. do you have any idea what this, thanks so much

    I have huge pain in the ball of my foot, shoots all the way up this tendon area. I had a hairline fracture on one of my sesamoids which is still causing this pain.

    I had the top of my foot ran over directly in the middle. Had a bit of bruising and pain after it happened but that was it. It's now been 2 years since it has happened and now my big toe is very tender and hurts a lot, especially at night. It seems like it's stuck flexing upwards a little bit and i can't bend it at all. I've been doing a lot of research and I'm thinking it is checkrein deformity possibly. It seems to me that it's the big tendon on top of my foot that hurts, the one that runs into the big toe, because it's stuck bending my big toe upwards. I can't put any weight on it either it hurts like a mofo. The pain is right at the top, where the toenail is but sometimes my calf feels achey and some days my ankle will hurt a little as well, but mainly my big toe.

    I wonder if it's really my FHL leg muscle that hurts whenever I walk more than a few yards or if it's actually the tendons attached to the bone? I don't experience any pain at all in my big toe or foot, only the back of my leg.

    Hi,
    Can you tell the difference between Flexor Hallucis Longus and Flexor Digitorum Longus? I checked some pictures of the anatomy and to me it seems like the Hallucis Longus should be on the outside at the calf and not on the inside of the calf as you showed it. I got diagnosed with a problem at the Hallucis Longus and I do have the pain on the outside and was just curious when I saw the video if I got something wrong..

    Hello doctor, I would like to ask. I have this vibrating-like feeling under my right foot on its left side (inner arch part), upon palpating the area, I can feel that it pulsates upon vibration, one time I walked it hurt like having something pinched inside (I don't know if it's cramps or something is really pinched)- it was painful that I had to stop walking. I've been having this for 2 days now. I tried massaging the area, it does help to be relieved for a while then the vibrations goes back. I mostly feel the vibrations when lying down or sitting. Stomping my feet makes it painful as well like that prickling feeling. No numbness nor burning sensation.
    I searched it up online, and said I may have pallesthesia and it can be a cause of compressed nerve or blocked artery. What could this really be? Want to do home remedies for it, what can I do?

    My theory for how i caused mine: i do leg press with more than 400 pounds. I'm short so i need to push off with my toes to get it off the rack. In the moment I push off i can feel soreness and slight pain in my feet. I think doing this constantly is what caused the pain behind my inner ankle.
    It's been around 3 weeks and it get reaggravated everytime i walk. Hopefully wearing those special shoes or braces can help

    trail and road runner here. wanted to check something with you guys. any help would be much appreciated.
    anyways, weird thing happening in my case. i have this weird “pain” just beyond my ankle where both flexor digitorum and hallucis meet. basically on the bone… it actually hurts when i press that spot with my fingers, just like a bruise. but that’s not all - when i feel the most discomfort is by pressing my foot on the ground with its left or right edge. that’s when it starts to hurt, when the foot stabilization muscles activate. all the discomfort happens in the backfoot area, gravitating to that beforementioned spot beyond the ankle. i can’t seem to pin point the affected muscle, but i’d say it’s hallucis. i can run however, but i can also still feel that the problem persists.. is it ok to just lower the running volume and include more foot strengthening exercises or should i stop running completely until the discomfort is gone?

    Mine is at my arch n around the bone inner most of the time. After standing on the foot the outer foot bone around the bone. It hurts. I hurt it at work in June why is it taking so long to heal up?

    I am a housekeeper for multiple hotels.. tourist destination. Very high, narrow arch; wide toe box, unstable ankle; likely underpronation/supination. 5 feet tall, 18.5 inch calves.. I also work out and stretch but calves are super tight probably related to foot/calf/achilles anatomy. Treating this issue with FHL. Trying to avoid it in the future also. Your shoe link doesn't recommend anything for my type foot and gait. Any recommendations?

    Just did an mri and I have full thickness cartilage loss and moderate to severe first metatarsal joint space narrowing. Large subchondral fracture of the first metatarsal head 0.9 x 1.6 cm

    Folks, read Ebonie Rio, Jill Cook, Craig Purdham and Keith Baar on tendonitis. They are the best sources of information. It doesn't have to be this complicated. Phase 1 - Isometrics, Phase 2 - Isotonics, Phase 3 - Load Storage , then phase 4 exercises which are energy storage and release . Walking is so tough cause it is a stage 3 or 4 exercise where as rehabbing to the point of pain free walking requires minimizing walking until you've built up your phase 1 and 2 strength. Good luck.

    Hey Doc. I got diagnosed with tenosynovitis of the FHL at the knot of Henry (mri showed fluid right at the knot). I have had it for 7-months now. I don’t experience pain per say… but there is discomfort at the bottom of foot while I’m out hiking / running (mostly when going uphill)…more discomfort when hiking than running during the push off phase. Have you seen this condition in the same spot? I have zero pain when at rest….but the condition is limiting my endurance outings… does this usually take several months to recover from? Do you have any recommendation for how to get rid of that fluid that's built up deep in the foot? I have taken serious leaps in my foot-calf strength/active flexibility. Diet is low inflammatory. But MRI still shows fluid and I still feel it. Thanks for reading!

      sorry didnt proofread very well - apples. You may need to slowly reduce your intake of these foods to improve inflammation, but very slowly reduce one item at a time.

    Can this pain follow ORIF surgery while healing? I’m 4 months post op - I’m coming along quite nicely!! Thanks for your videos, they’ve helped me. I’m trying to get my strength back in bad foot to raise to my toes. I cannot do it on that foot alone at all without support. Wondering if the weakness to do this is caused from this, or my Achilles, or my calf muscles. I know I’m still healing, but curious what muscles or tendons are used in toe raises. Gotta get back to full strength here!! 💪😊🦶

      @@ryant9418 exercise, exercise, exercise. I called it my ‘flat tire’. Couldn’t raise up on my foot at all. I did toe raises with both feet. I could tell my weak foot was taking a bit of a free ride though. Kept working on toe raises and eventually did it one foot at a time. Kept working on it, and it got stronger and stronger in time. Also try heel drops off of steps to stretch your calf muscles. Then raise up and down on the step. I believe it’s the weakness in the calf that causes the issue. And being non weight bearing for 8 weeks is what weakened the muscle. Time to build it back up. Work in squats too. Squat as low as you can, and try to improve your range there.     I’ve been dealing with this FHL tendinitis for 5 years now. I’ve tried everything mentioned in this video. Walking boot, custom orthotics, Hoka shoes, compression sleeves, physical therapy (marble toe pickups, towel crunches, calf raises). I dont know what to do anymore, please help!

      In the exact same boat now. Hope you found a solution! Acupuncture and dry needling in my lower calf muscles seem to be helping

    Is it possible for a person with severe flat feet (had reconstructive surgery in left foot 3 years ago and right arch is also terrible) to strengthen foot muscles to a point where the arch is fixed or minimalist shoes are possible? Or can the deformity be so severe you're doomed to maximalist shoes, expensive orthotics and weak feet for life? In other words, can you really only fix feet with minor to medium issues?

      Basically minimalism shoes force you to take shorter steps and work your foot muscles more. Personally I'm a much bigger fan of more supportive shoes and find people are much happier with them and a good orthotic. I speak from personal experience also as I have a permanent left leg injury that is almost fully resolved while in a good shoe and orthotic.

    What about abductor hallucis? Pain is focused on inner foot and midway between toes and heel. On feet for long shifts.

      Same here.. how’s it going now??.. I think I have tendinitis in the abductor hallucis tendon (bunion bone to mid-arch).. tendon sticks out, is tight, thick and painful which then pulls on the abductor hallucis where I have pain, tightness and cramping..
      I think I’m finally working it out now but it’s been bothering me since last August!!!..

    The back of my foot near my achilles gets inflamed when I go deep into Plantar flexion. Im thinking its this tendon but it doesnt bother me when I pull my toe? What else could it be

    I have the os trigonum syndrome for about 3 years now, it started when i rolled my ankles while running down the stairs, I heard a lot of popping like bubble wrap sounds and alot of pain in my inside ankle after it happened. It has been 3 years and I still have it. The only annoying part is when I roll my big toe down it locks it all the way to my heel, and I need to play with it a little to snap it back to place, does not affect me when running or walking. Any tips on how to get rid of it?

      @@fingerstyleacoustic6214 whenever I roll my big toe down and it locks, I just lift my foot/toes as high as I can and it snaps back for me easy 90% of the time.

    Hi, I hope somebody can help me. So Im having problem from my right flexor hallucis longus tendon ever since and whenever I run my FHLT does not work/activate from my right foot/calf, so most of the time my right heels and right leg quad muscles absorbs my body weight instead of the whole leg and calf muscle. During my run I believe the the flexor digitorum brevis lifts most of my weight as well from right foot/leg but my left leg and foot is perfectly fine. another good example I have is Im having difficulties tip toeing using my right foot but it does not hurt tho.What actions to take please. I really want to rebalance my body, hope to hear from anyone.

    I broke my fibula and tibia and since then my big toe forces down. I pull my foot toward me the great toe forces down.

    I HAD SURGERY TO REMOVE MY OS TRIGONUM BECAUSE MINE WAS 3 TIMES LARGER THAN THOSE THAT HAVE THIS ACCESSORY BONE.. THIS BONE FULLY RUPTURED MY FHL.. SO I HAD TO HAVE SURGERY TO TAKE THE BONE OUT, AND THEN THE FHL FROM THE CALF PULLED DOWN AND INFUSED INTO MY FOOT.. CURRENTLY REHABBING BUT WAS ONLY GIVEN SHEETS OF PAPER SHOWING WORKOUTS.. ANY TIPS ON STRETCHING AND HELPING WITH THE SCAR TISSUE?
    Also still cant bend my big toe 👈😢

      @Rebecca Gonzales well I didnt just have to Os trigonum removed, I also got my FHL attached to my calcaneus or foot.. the os trigonum fully ruptured my FHL so I could and still cant bend my big toe.. I had surgery on oct.14th 2021, it is now January 2022 and I am back training but not at full speed just yet.. the hard part is being patient , allow it to heal, stretch, and the swelling is the major issue..

    I dislocated my big toe in a car accident and fractured my 1st metatarsal, MRI tomorrow, one month after the injury I can only move my big down downwards a little and have some strange sensations and pain around the toe.
    I had a large amount of bruising at the back of my calf in the area that the FHL tendon seems to go through from your markings. Have been in a boot since and walking without it seems to be getting easier over time, although I bear as little weight on that side of the foot and even got an orthopedic insert for my shoe to take pressure off the area. I have used RICE for the first 4 weeks, the foot is no longer swollen, should I continue with icing at this point?
    On my other foot, I can clearly see my EHL tendon attaching to my big toe from the top of my foot, on the injured foot, I have no such visible tendon line.
    What injuries am I potentially dealing with and what kind of prognosis should I expect? The Dr. did mention the possibility of an operation, but I know the MRI will confirm the next step.
