The temporomandibular joint (TMJ) connects the skull's temporal bone to the jaw's condyle, aided by an articulating disc for smooth movement and pressure distribution. Temporomandibular disorder (TMD) encompasses various conditions causing pain and dysfunction in this region. TMD's key features include orofacial pain, restricted jaw movement, joint noises, and common symptoms like headaches, ear issues, muscle tenderness, and masseter hypertrophy.
TMD affects all genders and ages, with a higher prevalence in women aged 20-40. Risk factors include jaw injury, bruxism (teeth grinding), arthritis, and psychosocial stress, although it can also be idiopathic.
TMD can be categorized into four groups: muscle-only issues due to overuse or myalgia, TMJ hypermobility from loose ligaments, inflammation (primary or secondary), and TMJ disc derangement, which includes various subtypes like anatomical deviations, disc displacement, and degeneration.
Diagnosis involves a medical and dental history, physical examination, and imaging techniques like MRI or CT scans, sometimes requiring specialist referrals for an accurate diagnosis.
Treatment options depend on severity and causes:
Self-care interventions: Resting the jaw, hot/cold packs, soft diet, and stress management.
Conservative treatments: Over-the-counter NSAIDs, muscle relaxants, mouthguards, splints, and physical therapy.
Advanced interventions: Injecting medications into trigger points and, in rare cases, surgical management.
