Thank you so much. I am 24 hr after surgery, I had the follow up and noticed the ND right away and the doctor said just to give it time but didn't explain what is was or that it was normal. I was very much worried. Also noticed shaking of vision and also it had not been mentioned as a normal occurrence. Thank you for alleviating my worries.
I had my left eye done 24 hours ago, and the arc became apparent just an hour ago. I was ready to call my doctor, as I did not have it at our 1-day post-op appointment. I had cataract surgery two days ago. I noticed this a slight flashing or flickering sensation. So naturally I thought I broke my lens. You've reassured me. I will wait to talk to my doctor at my one week check up. Thank you.
Great video. Very informative. I had a cataract surgery in September 2022 and October 2022. Three months later I still see these crescents on the 2 outer side of my eyes. My ophthalmologist will see me in February 2023. I am glad that I have this information now, it's a relief. Many thanks.
I had my eyes done in 2022, and the right eye still has the crescent shadow sometimes, I was a bit freaked out by it and need to see my Ophthalmologist for a checkup anyway, but now I know how to talk about it.

Thank you for this video. Very informative. Patients should be informed of this pre and post op. I was not. It was definitely scary and very depressing when I noticed it late evening, the day after surgery. The Post op exam showed nothing wrong, so I had no clue why I started experiencing this. And this information is not readily available anywhere else on the internet. So I wondered what was happening, Friday, Saturday and today (Sunday). I’ll call my Optomologist/Cataract surgeon in the morning.
same exact experience as me. It's been only 5 days since my surgery; the post op check up was great, only began noticing the edge of lens that night. She had told me the other side effects but did not mention this one.
My experienced surgeon not only failed to inform me of this possible side effect, he did not acknowledge the symptom, and got short with me ("you just had suregry yesterday!") for discussing the symptoms (also having flickering and shimmering). In the after visit notes following day, did not note Dysphotopia, instead "halos" which is not what I described. I'm a researcher, and I am reassured by knowledge, and the arrogance of not providing full information and the ego of his response makes this much harder. I feel like he thinks I'm a complainer now and I still have to do another eye, so I need to button up.

I had my left eye done in February 2020 - panoptix iol. I notice ND only when sitting at my desk for work which has a window 90 degrees to the left of my sitting position. It is bothersome but not enough to make me unhappy with the implant. But I'm glad to have the name! I kept wondering what was going on because I never heard this issue mentioned in research prior to surgery. On a side note, it's amazing to watch all those tools poking into the eye and it still works afterward. Cataract surgery is amazing. I was seriously going blind prior to the surgery.
Winner of a video, I have been researching "how do you cure eye floaters naturally?" for a while now, and I think this has helped. This is exactly what is happening to me. My optometrist didn’t have an explanation for the arc. Did an exam and said everything looked fine. My ophthalmologist won’t even see me unless my optometrist refers me. I thought it might be a retinal detachment but he said retina was fine and had no explanation for the arc . Thank you for this video. I am 5 months out of surgery , have a Toric lens and didn’t really notice it till 3 weeks ago. I guess I just live with it

Finally ! No one, not my eye doctor, not the surgeon told me of this. When I complained about my new permanent swim goggles all I was told was I'd get use to it. My reply to them was if I had a car that pulled to the right and I took it to a mechanic and he said don't worry you'll get used to it, that would be the last visit to that garage. Great video Dr. Wong, thanks for posting. I underwent cataract surgery 3 days ago, and I have been experiencing all 4 of the post-operative symptoms you have described. I wish that my surgeon had explained these 4 common symptoms before the surgery like you do. I've been spending hours online trying to figure out what went wrong. Do I have a detached retina? Is the lens unstable? Was it the wrong diameter? Etc. This video was very reassuring.
@@robnation2475 It has been 7 months and I still have the peripheral shadow. Also the flickering is still around. And if in a dim room, or even in total darkness, looking to the side and back rapidly shows a brief streak of light on the periphery. They say it can take up to a full year to “go away” so in 5 months we’ll find out. Note: I opted for a monofocal lens in order to have the least side effects possible, didn’t work. Finally, I had none of these issues prior to cataract surgery.
@@generic_official Thanks for that info,...sounds pretty frustrating. Probably makes a guy wonder if they got the correct physical measurements? Or is it just a matter of time to adjust? Yesterday mine calmed down but today flickering is back along with the peek-a-boo shadow line. Seems to be less obvious when I wear some glasses with the lens removed because the frame seems to "disguise" the shadow for me. Different levels of lighting seem to affect the flickering the most for me. Another multifocal IOL patient had better results after introducing artificial tears. I picked up some "Refresh Tears" yesterday and I had my best day after that. Still not sure what number of other factors may have made the difference though. Either way my eyesight is better than what I was dealing with prior to surgery and I put 5 of my 7 pairs of glasses in a drawer so far. (so I've got that going for me ...which is nice)
I agree,, I wish may surgeon had told me about the blue color that would plague me for months, and that damn shadow that scared the hell out of me. I thought the lens had come detached. I had to go online to find out what it was. I wish Docotors would stop treating their patients like idiots!!!

I had cataract surgery 4 weeks ago.... I still have the experience which you described. I was told just to ignore it. Every day I wake to this annoying shadow I am scheduled to see my doctor in 3 months. I wish your practice was in New York....
I always considered that temporal arc to be a refractive effect of the temporal corneal incision and it tends to dissipate as the incision becomes more transparent with time. And I use exclusively the B and L LI61AO lens myself. If the dysphotopsia was edge related I would expect it to be a complete circle, not restricted to an arc that correlates to the exact placement of the incision. That is my speculation.
I would like to speculate further by mentioning that perhaps the arc is complete. Understanding due to the fact that the length of the exposed portion of the sclera from corner to corner of the eye is greater than measurement from upper to lower lids that the upper and lower lids as well as the bridge of the nose might act in a similar fashion as to placing one's hand adjacent to the side of the head 1:20 thereby concealing the disphotopsia which is typically only visible on the unblocked side. BTW "I like the way you talk" (spoken in the voice of and quoted from Billy Bob Thornton as Karl Childers - Sling Blade (1996)).
@@orbsphere- The next patient who mentions it to me I’ll try a little trick. If I can get them to see the arc in the clinic, I’ll pull the lids back to see if they report the arc extends superiorly and inferiorly. In this way I shall test your speculation.
This was very helpful Dr.Wong. I had right side cataract surgery last Thursday, June 15, 2023. The cloudiness has gone but the crescent shape is still there when I look to the left. I have experienced the “flickering “ but seems to be subsiding. After listening to your video I feel better. I have a two week checkup on the 29th and my POD one checkup was good. Hopefully it will subside bc it is a little concerning. I’m going to call my surgeons office just for reassurance. Thank you
I had surgery on the left side and the shadowy crescent has been present since the day after the surgery. My first post-operative exam didn't show any issues. Has yours gone away?

Thank you for your amazing explanation. Ive had the surgery a month ago and im experiencing similar symptoms in my left eye. However, I only experience the shadow when I look at things at a close distance ( approximately 1 meter). I also experience a sort of quiver when I'm focused on one thing and then I look at something else.
Had it, noticed it immediately, and it became less obvious to me over the next two or three weeks. Piece of cake. When I saw my ophthalmologist next, I mentioned what I described as an archic artefact at the extreme left of my vision, and added that I feel it must be the result of the geometry of the IOL. I didn't get the last word out but she laughed and said, 'Whoa, you're too smart for me!' The lens was the J&J Toric II ZCU 225.
I got my cataract surgery done. My vision has magnified 20 fold. I think they used incorrect lense. I tried to tell the doctor but he dismissed my concern and said they can use a "lesser powerful" lense in my next eye. Why can't they just replace the incorrect lense with the right one? I feel so angry that I trusted the doctor's staff (the doctor did not take my eye test, his staff did so no fault of the doc!). If anyone can advise how I can go about getting this corrected without negative ramifications to self. Thank you

Hi, one week ago I had Refractive Lens Exchange multifocal and although the light flickering is gradually decreasing I can still see what looks like dark arcs in me extreme peripheral vision, i bit like looking through binoculars. Very early days I know but hopefully this will improve over time.
Thanks for your informative videos and explaining more about the procedures and artifacts that unfortunately many surgeons don't discuss with patients. Do you have a video about the flicker or quiver that doesn't go away after 6 weeks? Seems to be associated with brighter light in peripheral vision. Also, perhaps a discussion of positive dysphotopsia, hazy vision and starbursts around lights (toric monovision single piece IOL). Thanks!
Noting that the incredible Dr. Wong can hold a flimsy flexible acrylic IOL model at arms length while talking to the camera…and that IOL barely moves…especially notable is the fine motor control of the little dangling armatures. 💪🏼👍🏼
3 months post Vivity lens cataract surgery I do not see clearly. have lost my near vision. I am an artist, doctor said this lens "would solve all my problems." only see about 60% distance of what my glasses or contacts provided. 4 weeks out I could not see out of left eye total blurr, (doctor said I could see, until he looked in my eye and realized I did have a problem, then asked if I had stolen numbing drops and been using them!! I am not kidding! ) ended up 2 months with Cornea Specialist. This has been a disaster. I would be happy to go back and live with a little glare in my one eye if only I could. dry eyes, floaters so bad it looks like space trash, pain in right eye, my eyes actually twitch a very strange feeling like a strobe light, the list goes on. my advice don't do it. I did the research, interviewed 3 doctors, 1 twice, was positive and excited to have the surgery. surgery was a breeze. thought it was going to be good. so wrong. it has caused a huge handicap with my career.
I wish you were closer to where I live. I have had similar problems with my left eye after cataract surgery.. My right eye was done first with no complications and excellent results. Both are standard Bosch and Lomb LI61SE’s. Each done a few weeks apart. The left eye had complications. Trouble with bleeding during the procedure and placement of IOL. Recovery was never decent. Created cloudy effect a couple of days later. Performed a YAG on left eye about a month later. Took care of the cloudy effect. But, have a small rotating star effect when looking at tungsten lights. Especially, at night. Also not as clear as the right eye. Went in for a second procedure using same YAG machine to get rid of some floaters about a month later. Nothing has changed almost a year later from the initial surgery. I personally think I need a IOL replacement. But, am told that it is too risky. I also at times see the arc on the outside edge at times. Making me think it’s not placed correctly. A year later, star effect that rotates, not as clear vision, (like a smudge on the IOL), and arc at times. My right eye is perfect. So, that is my comparison. Thanks for any insight doctor. Thank you for this video. I just had a multifocal lens fitted two days ago and I have been worried about a dark crescent and slight flicker at the very edge of my peripheral vision. The worry was that was I experiencing the curtain effect that people talk about when you have a detached retina. I didn't think so, as I was already noticing that light may be the issue and had tried the hand to the side of my face technique myself. Watching your video I was like yes! This is what I am experiencing too! I have already lost most haloing and can now read my phone without glasses for the first time in a few years, so I am adapting very quickly, but my joy was rather quashed by my initial worry. I am now not as worried. Thank you again for making my day better.
Did your shadowy (flickering) crescent go away? I did the surgery a week ago and have the same symptoms as you described. My post-op exam showed no issues.
@shivarahimi7597 Hi. The flicker went away pretty quickly, none at all now. The dark crescent has become barely noticeable now, I only see it if I look to my extreme left (my right eye is he one with the new lens), but I’m used to it so it's not an issue. The only thing I do still have is the halo effect on bright lights and a general 'glow' around white text on a dark background on my phone. All trade-offs have been worth it, is it perfect? No. Is it life changing? Absolutely! I would also add that my vision is less sharp in low light, but that was/is an issue I had before surgery anyway (both eyes). It does seem that my natural eye is sharper than my robo eye at distance, my robo eye is better close up, that's not difficult though, the left eye has awful near vision. Good luck with your recovery. :)

@@JetmanUK I got the monofocal lens since at the moment I don't have cataract in the other eye. I would have gone with the multifocal if I were doing the operation for both eyes. I am 50 and hopefully it may be years before my other eye would require the surgery. Having gone with the monofocal lens, I don't have the halos and glares right now., But who knows maybe in the future it will be different. But right now my two bothersome symptoms are flickering and the shadowy arc.
My left eye had uvitis, and floaters and dipolpia and cataract, I also have a freckle in the right eye I'm thinking of going for Cataract surgery. I live in NJ. I seen Dr. FOSTER in Mass, who sent me to Ocular immunologist who was trained by him Dr. David Chu. Do you think cataract surgery would be complicated? Just wanting to get some points of view.
I see some kind of shiny yellow semi-circle to the left if the light source is on the right, hope you understand what I mean. But when I move my eye to the right or move somewhere else it’s sort of disappears and then appears and like a northern light perhaps. I wonder if my lens got shifted in my eye? I didn’t practically move since operation, all precautions to the extend: it’s been 38 hours now, the eye is still dilated for some reason and I have no focus on anything really. Generally it seems like someone put a cloudy contact lens in me and it irritates me why it’s not clear. Yesterday I had a show from Christmas tinsel-like light dispenser from a light in a mirror.
3 weeks after monofocal plus my vision on that eye alone is a little double visioned which i believe will go away. My neg dysphotopsia seems to have subsided after the 3 week time. But hope for vision becomes sharper.
I have had nothing but issues since I had my cataract surgery. I had a normal lens put in my left eye and a toric lens for astigmatism put in my right. I have the symptom of negative dysphasia and described it as loss of my peripheral vision to my doctor, but she never mentioned this issue. I did the trick you said, and it went away. Why didn't she tell me about this? My first surgery was done on April 29th, and my second surgery was on May 12th. My right eye I have decent distance vision but poor up close and my left eye is the exact opposite. In my last f/u appt, my doctor told me I was a "Puzzle," and she didn't know why my vision was imbalanced. She put me in a - 0.75 contact for my left eye which is an improvement but still not great. The imbalance in my vision is very disturbing and interferes with my life overall. She told me to return in 6 weeks for a recheck, but I need a second opinion. Can you tell me your opinion? had surgey with Vivity last wednesday luckily i didnt get the shadow. had terrible light streaks at night for 3 days. I still have slight blurriness at distance, vision from 3-15 feet is very sharp
This question doesn't have to do with ND but I'm 23 days past my very bad brunescent cataract surgery on my right eye and just yesterday I've notice a dark shadow on the upper outside of my eye. Nothing bad and it comes and goes and not very often so I can live with it I guess. More importantly my vision is still VERY BLURRY in that eye. I'm so sad. Is there any way it will get better? Is there any time that a bad lens is put in that could be causing this blurryness. In past visits the surgeon said nerve looks good, no macular stuff goin on and she did the YAG procedure to get rid of scar tissue. I go back to see her on Tuesday, Sept. 3rd for a prescription.

I experienced the ND and “jiggling” same day post-op - I was afraid it was due to the lens dislocating because I did a short gentle jog to the elevator from my apartment. I forgot I wasn’t supposed to so vigorous or strenuous exercise.
Hi Julianna. I just had a Panoptix lens put in this morning and have the jiggling (and out-of-focus vision) I fully expect these things to go away after time, how has your experience been 1 month later?
Next time read about the do's and don'ts of this type of surgery, like not lifting anything heavy or bending over. Your doctor should have given you a list of of what not to do before or after surgery. Also no alcohol or smoking.
3 days ago(19/05/2022) my father's cataract operation has done. Today he is saying that he is seeing a black disk only. Please give me some suggestions why is this happening and what to do?

Is any one experiencing as one person called it "moveable fog with eye movement? I had left eye done mid April 2024 & Right eye 2 week later. Vision is good, but having this fog type sympton in both eyes. Does it go away?
I do not like the default surgery Drs put forward for cataracts. Several reasons: 1. Sucking out the bad lenses never addresses the reason we got the cataracts. 2. Placing two pieces of plastic one into each eye, In My Head, so the plastic can leach out, sounds like a BAD idea. The way medicine in practiced in the US is so tunnel vision/compartmentalized with no forward thinking on what the downside/consequences are of sucking out this oxidized lens and putting Plastic into my body! There ought to be an eye drop that arrests the development of cataracts or dissolves them. Cataract eye drops DO exist for dogs! Reason none for humans? Drs charges U $3200 to $6000 Per Eye to put in these pieces of plastic. No eye dr wants that nice revenue stream interrupted. One of the Ophthalmologists I called to ask some questions of in my area said she does 11 surgeries a day. Do the math--this woman is swimming in money. Post-surgical procedures are common.
Has anyone experienced seeing a "butterfly wing" (surrounded by a halo) looking reflections from streetlights and headlights when driving at night? (after getting surgery)

It’s been 2 months and I still having the flickering in my left eye with the crescent shadow and it’s scareing me to death, doctor now diagnosed me with PCO and can’t get the laser done for another 2 months, vision is blurry in the left eye and coudy in my right, I have a toric lens set for distance and am so terrified I’ll never see right again 😩
I had cataract surgery in right eye only 2 1/2 months ago and about 2 weeks after my surgery I woke up to crescent light shadows on both sides, left and right eye.When I close my left eye the shadow disappears on right side. When I close right eye the shadow disappears on left side. Very odd. When I wiggle my nose I see both shadows move so the shadows appear to be a reflection of my nose and face. A few days before my shadows appeared I was diagnosed with mild macular edema and have been taking 2 types of eye drops to calm the edema. I wonder if the drops could cause the shadows or is this ND. My Cataract surgeon does not believe me as she said ND always happens right after cataract surgery. Thanks for your response in advance. Any ideas if this is ND? Both shadows are light and I can see through the shadows.

What would you do if secondary ROC failed, i.e light still bypasses the edge of the lens rather than going through it and a dysphotopsia remains? leaving vignetting, with edge of lens ghosting.

Can you help me understand why I keep seeing flutters of light, black line and a few of these flutters of light I could actually feel hit my eye especially one of them I felt that my eye had just been shocked I notice that these occurrences happen more on cloudy days and in the presence of table lamps, these flutters of light most white and some like a color prism. I had cataract surgery on left eye Jan. 3, 2024 and the right eye on Jan 17th. Also my vision has went blurry especially the right eye and the doctor I go to is going to do a yag procedure the end of this coming april because of the reoccurence of cataracts. Thank You Barbara
I guess I'm lucky. I have not noticed ND. My dominant eye changed to the IOL (vivity) after the first surgery. My doctor is doing mini mono so it will be interesting if the dominant eye reverses after the next surgery.
Will shadow go away by the time pass ? I see shadow when I read something for near or far distance. I had Toric Tecnis Eyhance for Astigmatism implanted in my both eye and its been 3 weeks now and shadow still not going away, will it go away by the time pass ?
hello I had surgery a month ago with an icl evo phakic intraocular lens and from the day after the operation that shadow that you describe appeared When you look closely, you notice that when you look at the horizon... Is it possible that this type of lens gives me this negative dysphotopsia?
I have fully recovered from being totally blind for a year from root canal infections, after they pulled all my teeth, please pay close attention to if problems are from bad root canals the patient will not feel any pain and my not notice symptoms are from the teeth.. this almost killed me because nobody believed me and thought I was just drug seeking at 135 pounds now I have gained 40 pounds since I was sick and my 💯% vision back . I am now seeing iol edge glare / light flash after a capsulotomy just appeared after 3rd week. Weird that appears looking up or side to side. Is this different and how to address it. I am a big fan and thank you for your insightful videos. Because of you my mind was at ease as I received my Vivity lenses fyi the eye that had the capsulotomy is the one showing edge light. Dr. Wong, I was blind in my left eye for 2 weeks after cataract surgery. Dr said a piece of residue not removed was blocking pupil. Dr dilated eye and apparently the iris moved the residue, and I was able to see for a day. After a day the residue migrated back somewhat and obscured vision about 30% on the side closest to my nose. What should I do?
my doctor missed my weak zonucles during obervation and said she ajusted for it durning sergury im now seeing shadows to the right side should i these shadows move around and come and go its not constantly there based on the light of the room or the position of my head or do you think maybe its broken zonucles moving around makeing the lens move.
I had a right eye cataract operation on feb15 2020, and to date is may 4, i did the check for up to march 5, which i will have another check up on march 15, but due covid 19 we had a quarantine, now my eyes is some how ok, but sometimes not, why i am seeing a floaters, why sometimes if i look on my cp my vision suddenly become blurred..kjndly please help me to unxerstand
As a physician, it is also your responsibility to disclose potential complications, risks and recovery details associated with revision surgery after IOL has been installed some months past. People watch your video and think it's a simple procedure to go and fix dysphotopsia in a 20 minute procedure, when in fact any revision surgery can potentially harm inner layer of Cornea and lead to months of long tedious recovery that potentially requires DMEK to fix that problem. What you ought to start saying is, that if dysphotopsia is bothering you, consider that potential negative outcome of revision surgery can make your life hell if you can't see well due to damage to your endothelia cells. Fixing this problem is like breaking someone's shin bone with the intent of lengthening their height by a millimeter, and not telling them that the surgery can potentially damage your nerves which may impede your balance and ability to walk straight. So would that be worth it to you?
Great video Dr. Wong - I have had regular Cataract surgery both eyes in June 2022 & from the very onset complained as if my hair was falling over the corner of my eyes & was told it was called a ‘shadow’ which would go away! It is now over one year since my surgery and it is still there and I am so uncomfortable and feel as if I have been crying with heavy eyelids besides having to push back the imaginary hair coming across my forehead down to the corner of my eyes - is there help for me in this situation? Most appreciative of any suggestions from you.

I had cataract surgery 8 years ago and still have Negative Dysphotopia. It never goes away. I was given 3 choices. Take it out. Put a clear one on top of the existing one. Or another surgery. I chose none of them.
Wtf. I couldn’t live with it it feels like always looking into the eye instead out to the world. Also I can feel how this bring some tension into my neck.

Hi Dr Wing I hope you could advise me about my problem. I had cateract surgery last year March 2019 on my left eye and two weeks later I had the right eye done. Every thing went well but after 3 months one day after washing my hair my eyes started paining me, it got so bad I had to go to the emergency eye hospital. I couldn't see light there was this terrible pain. I was given eye drops to take for the next 6 weeks. As soon as I finish taking them the pain return 2 days later. Back to the hospital they did a scan my eyes was inflammed, more eye drops for another 2 months. Same again when the drops finish the pain came back. This went on now up to this day August 2020. I am worried as what if all these eye drops is going to have on my body, how long would it be taking these eye drops. I wish I did not had this surgery done I am worried about my sight. Can you advise me please. I need help they don't have any answers for me.
If you replace an SPA for an Li60 due to negative dysphotopsia, do you routinely place the Li60 in the sulcus? Or do you feel confident enough in to put it in the bag. I'm making this exchange this coming week and was planning to go for the sulcus, it's the first time I've used the Li60.

Thank you so much. I am 24 hr after surgery, I had the follow up and noticed the ND right away and the doctor said just to give it time but didn't explain what is was or that it was normal. Great video. Very informative. I had a cataract surgery in September 2022 and October 2022. Three months later I still see these crescents on the 2 outer side of my eyes. My ophthalmologist will see me in February 2023. I am glad that I have this information now, it's a relief. Many thanks.

I had my eyes done in 2022, and the right eye still has the crescent shadow sometimes, I was a bit freaked out by it and need to see my Ophthalmologist for a checkup anyway, but now I know how to talk about it.

Thank you for this video. Very informative. Patients should be informed of this pre and post op. I was not. It was definitely scary and very depressing when I noticed it late evening, the day after surgery. The Post op exam showed nothing wrong, so I had no clue why I started experiencing this. And this information is not readily available anywhere else on the internet. So I wondered what was happening, Friday, Saturday and today (Sunday). I’ll call my Optomologist/Cataract surgeon in the morning.
same exact experience as me. It's been only 5 days since my surgery; the post op check up was great, only began noticing the edge of lens that night. She had told me the other side effects but did not mention this one.
I really dislike my post surgery symptoms. For me it is like a movable fog when I move my eye. Very annoying.
My experienced surgeon not only failed to inform me of this possible side effect, he did not acknowledge the symptom, and got short with me ("you just had suregry yesterday!") for discussing the symptoms (also having flickering and shimmering). In the after visit notes following day, did not note Dysphotopia, instead "halos" which is not what I described. I'm a researcher, and I am reassured by knowledge, and the arrogance of not providing full information and the ego of his response makes this much harder. I feel like he thinks I'm a complainer now and I still have to do another eye, so I need to button up.

I had my left eye done in February 2020 - panoptix iol. I notice ND only when sitting at my desk for work which has a window 90 degrees to the left of my sitting position. It is bothersome but not enough to make me unhappy with the implant. But I'm glad to have the name! I kept wondering what was going on because I never heard this issue mentioned in research prior to surgery. On a side note, it's amazing to watch all those tools poking into the eye and it still works afterward. Cataract surgery is amazing. I was seriously going blind prior to the surgery.
Winner of a video, I have been researching "how do you cure eye floaters naturally?" for a while now, and I think this has helped. This is exactly what is happening to me. My optometrist didn’t have an explanation for the arc. Did an exam and said everything looked fine.
My ophthalmologist won’t even see me unless my optometrist refers me. I thought it might be a retinal detachment but he said retina was fine and had no explanation for the arc . Thank you for this video. I am 5 months out of surgery , have a Toric lens and didn’t really notice it till 3 weeks ago.

Finally ! No one, not my eye doctor, not the surgeon told me of this. When I complained about my new permanent swim goggles all I was told was I'd get use to it. My reply to them was if I had a car that pulled to the right and I took it to a mechanic and he said don't worry you'll get used to it, that would be the last visit to that garage. Great video Dr. Wong, thanks for posting. I underwent cataract surgery 3 days ago, and I have been experiencing all 4 of the post-operative symptoms you have described. I wish that my surgeon had explained these 4 common symptoms before the surgery like you do. I've been spending hours online trying to figure out what went wrong. Do I have a detached retina? Is the lens unstable? Was it the wrong diameter? Etc. This video was very reassuring.

@@robnation2475 It has been 7 months and I still have the peripheral shadow. Also the flickering is still around. And if in a dim room, or even in total darkness, looking to the side and back rapidly shows a brief streak of light on the periphery. They say it can take up to a full year to “go away” so in 5 months we’ll find out. Note: I opted for a monofocal lens in order to have the least side effects possible, didn’t work. Finally, I had none of these issues prior to cataract surgery.

Yesterday mine calmed down but today flickering is back along with the peek-a-boo shadow line.
Seems to be less obvious when I wear some glasses with the lens removed because the frame seems to "disguise" the shadow for me. Different levels of lighting seem to affect the flickering the most for me. Another multifocal IOL patient had better results after introducing artificial tears. I picked up some "Refresh Tears" yesterday and I had my best day after that. Still not sure what number of other factors may have made the difference though. Either way my eyesight is better than what I was dealing with prior to surgery and I put 5 of my 7 pairs of glasses in a drawer so far. (so I've got that going for me ...which is nice)
I agree,, I wish may surgeon had told me about the blue color that would plague me for months, and that damn shadow that scared the hell out of me. I thought the lens had come detached. I had to go online to find out what it was. I wish Docotors would stop treating their patients like idiots!!!

I had cataract surgery 4 weeks ago.... I still have the experience which you described. I was told just to ignore it. Every day I wake to this annoying shadow I am scheduled to see my doctor in 3 months. I wish your practice was in New York....
I always considered that temporal arc to be a refractive effect of the temporal corneal incision and it tends to dissipate as the incision becomes more transparent with time. And I use exclusively the B and L LI61AO lens myself. If the dysphotopsia was edge related I would expect it to be a complete circle, not restricted to an arc that correlates to the exact placement of the incision. That is my speculation.
I would like to speculate further by mentioning that perhaps the arc is complete. Understanding due to the fact that the length of the exposed portion of the sclera from corner to corner of the eye is greater than measurement from upper to lower lids that the upper and lower lids as well as the bridge of the nose might act in a similar fashion as to placing one's hand adjacent to the side of the head 1:20 thereby concealing the disphotopsia which is typically only visible on the unblocked side. BTW "I like the way you talk" (spoken in the voice of and quoted from Billy Bob Thornton as Karl Childers - Sling Blade (1996)).
@@orbsphere- The next patient who mentions it to me I’ll try a little trick. If I can get them to see the arc in the clinic, I’ll pull the lids back to see if they report the arc extends superiorly and inferiorly. In this way I shall test your speculation.
This was very helpful Dr.Wong. I had right side cataract surgery last Thursday, June 15, 2023. The cloudiness has gone but the crescent shape is still there when I look to the left. I have experienced the “flickering “ but seems to be subsiding. After listening to your video I feel better. I have a two week checkup on the 29th and my POD one checkup was good. Hopefully it will subside bc it is a little concerning. I’m going to call my surgeons office just for reassurance. Thank you
I had surgery on the left side and the shadowy crescent has been present since the day after the surgery. My first post-operative exam didn't show any issues. Has yours gone away?

Thank you for your amazing explanation. Ive had the surgery a month ago and im experiencing similar symptoms in my left eye. However, I only experience the shadow when I look at things at a close distance ( approximately 1 meter). I also experience a sort of quiver when I'm focused on one thing and then I look at something else.

Had it, noticed it immediately, and it became less obvious to me over the next two or three weeks. Piece of cake. When I saw my ophthalmologist next, I mentioned what I described as an archic artefact at the extreme left of my vision, and added that I feel it must be the result of the geometry of the IOL. I didn't get the last word out but she laughed and said, 'Whoa, you're too smart for me!' The lens was the J&J Toric II ZCU 225.

I got my cataract surgery done. My vision has magnified 20 fold. I think they used incorrect lense. I tried to tell the doctor but he dismissed my concern and said they can use a "lesser powerful" lense in my next eye. Why can't they just replace the incorrect lense with the right one? I feel so angry that I trusted the doctor's staff (the doctor did not take my eye test, his staff did so no fault of the doc!). Hi, one week ago I had Refractive Lens Exchange multifocal and although the light flickering is gradually decreasing I can still see what looks like dark arcs in me extreme peripheral vision, i bit like looking through binoculars. Very early days I know but hopefully this will improve over time.
Thanks for your informative videos and explaining more about the procedures and artifacts that unfortunately many surgeons don't discuss with patients. Do you have a video about the flicker or quiver that doesn't go away after 6 weeks? Seems to be associated with brighter light in peripheral vision. Also, perhaps a discussion of positive dysphotopsia, hazy vision and starbursts around lights (toric monovision single piece IOL). Thanks!
Yeah the flicker is greater (or more apparent/obvious) in brighter light for me as well. Only 2 days after surgery but hard to ignore.👀

Noting that the incredible Dr. Wong can hold a flimsy flexible acrylic IOL model at arms length while talking to the camera…and that IOL barely moves…especially notable is the fine motor control of the little dangling armatures. 💪🏼👍🏼

3 months post Vivity lens cataract surgery I do not see clearly. have lost my near vision. I am an artist, doctor said this lens "would solve all my problems." only see about 60% distance of what my glasses or contacts provided. 4 weeks out I could not see out of left eye total blurr, (doctor said I could see, until he looked in my eye and realized I did have a problem, then asked if I had stolen numbing drops and been using them!! I am not kidding! ) ended up 2 months with Cornea Specialist. This has been a disaster. I would be happy to go back and live with a little glare in my one eye if only I could. dry eyes, floaters so bad it looks like space trash, pain in right eye, my eyes actually twitch a very strange feeling like a strobe light, the list goes on. my advice don't do it. I did the research, interviewed 3 doctors, 1 twice, was positive and excited to have the surgery. surgery was a breeze. thought it was going to be good. so wrong. it has caused a huge handicap with my career.

I wish you were closer to where I live. I have had similar problems with my left eye after cataract surgery.. My right eye was done first with no complications and excellent results. Both are standard Bosch and Lomb LI61SE’s. Each done a few weeks apart. The left eye had complications. Trouble with bleeding during the procedure and placement of IOL. Recovery was never decent. Created cloudy effect a couple of days later. Performed a YAG on left eye about a month later. Took care of the cloudy effect. But, have a small rotating star effect when looking at tungsten lights. Especially, at night. Also not as clear as the right eye. Went in for a second procedure using same YAG machine to get rid of some floaters about a month later. Nothing has changed almost a year later from the initial surgery. I personally think I need a IOL replacement. But, am told that it is too risky. I also at times see the arc on the outside edge at times. Making me think it’s not placed correctly. A year later, star effect that rotates, not as clear vision, (like a smudge on the IOL), and arc at times. My right eye is perfect. So, that is my comparison. Thanks for any insight doctor. I watch your channel a lot.

I just had a multifocal lens fitted two days ago and I have been worried about a dark crescent and slight flicker at the very edge of my peripheral vision.
The worry was that was I experiencing the curtain effect that people talk about when you have a detached retina. I didn't think so, as I was already noticing that light may be the issue and had tried the hand to the side of my face technique myself.
Watching your video I was like yes! This is what I am experiencing too!
I have already lost most haloing and can now read my phone without glasses for the first time in a few years, so I am adapting very quickly, but my joy was rather quashed by my initial worry. I am now not as worried.

Did your shadowy (flickering) crescent go away? I did the surgery a week ago and have the same symptoms as you described. My post-op exam showed no issues.

The flicker went away pretty quickly, none at all now.
The dark crescent has become barely noticeable now, I only see it if I look to my extreme left (my right eye is he one with the new lens), but I’m used to it so it's not an issue.
The only thing I do still have is the halo effect on bright lights and a general 'glow' around white text on a dark background on my phone. All trade-offs have been worth it, is it perfect? No. Is it life changing? Absolutely!
I would also add that my vision is less sharp in low light, but that was/is an issue I had before surgery anyway (both eyes). It does seem that my natural eye is sharper than my robo eye at distance, my robo eye is better close up, that's not difficult though, the left eye has awful near vision.

@@JetmanUK I got the monofocal lens since at the moment I don't have cataract in the other eye. I would have gone with the multifocal if I were doing the operation for both eyes. I am 50 and hopefully it may be years before my other eye would require the surgery. Having gone with the monofocal lens, I don't have the halos and glares right now., But who knows maybe in the future it will be different. But right now my two bothersome symptoms are flickering and the shadowy arc.
My left eye had uvitis, and floaters and dipolpia and cataract, I also have a freckle in the right eye I'm thinking of going for Cataract surgery.

I see some kind of shiny yellow semi-circle to the left if the light source is on the right, hope you understand what I mean. But when I move my eye to the right or move somewhere else it’s sort of disappears and then appears and like a northern light perhaps. I wonder if my lens got shifted in my eye? I didn’t practically move since operation, all precautions to the extend: it’s been 38 hours now, the eye is still dilated for some reason and I have no focus on anything really. Generally it seems like someone put a cloudy contact lens in me and it irritates me why it’s not clear.

3 weeks after monofocal plus my vision on that eye alone is a little double visioned which i believe will go away. My neg dysphotopsia seems to have subsided after the 3 week time. But hope for vision becomes sharper.
I have had nothing but issues since I had my cataract surgery. I had a normal lens put in my left eye and a toric lens for astigmatism put in my right. I have the symptom of negative dysphasia and described it as loss of my peripheral vision to my doctor, but she never mentioned this issue. I did the trick you said, and it went away. Why didn't she tell me about this? My first surgery was done on April 29th, and my second surgery was on May 12th. My right eye I have decent distance vision but poor up close and my left eye is the exact opposite. In my last f/u appt, my doctor told me I was a "Puzzle," and she didn't know why my vision was imbalanced. She put me in a - 0.75 contact for my left eye which is an improvement but still not great. The imbalance in my vision is very disturbing and interferes with my life overall. She told me to return in 6 weeks for a recheck, but I need a second opinion. Can you tell me your opinion? had surgey with Vivity last wednesday luckily i didnt get the shadow. had terrible light streaks at night for 3 days. I still have slight blurriness at distance, vision from 3-15 feet is very sharp
This question doesn't have to do with ND but I'm 23 days past my very bad brunescent cataract surgery on my right eye and just yesterday I've notice a dark shadow on the upper outside of my eye. Nothing bad and it comes and goes and not very often so I can live with it I guess. More importantly my vision is still VERY BLURRY in that eye. I'm so sad. Is there any way it will get better? Is there any time that a bad lens is put in that could be causing this blurryness. In past visits the surgeon said nerve looks good, no macular stuff goin on and she did the YAG procedure to get rid of scar tissue. I go back to see her on Tuesday, Sept. 3rd for a prescription.

I experienced the ND and “jiggling” same day post-op - I was afraid it was due to the lens dislocating because I did a short gentle jog to the elevator from my apartment. Next time read about the do's and don'ts of this type of surgery, like not lifting anything heavy or bending over. Your doctor should have given you a list of of what not to do before or after surgery. Also no alcohol or smoking.

For me, the white light looks bluish after surgery. Maybe you can tell me why.
I do not like the default surgery Drs put forward for cataracts. Several reasons: 1. Sucking out the bad lenses never addresses the reason we got the cataracts. 2. Placing two pieces of plastic one into each eye, In My Head, so the plastic can leach out, sounds like a BAD idea. The way medicine in practiced in the US is so tunnel vision/compartmentalized with no forward thinking on what the downside/consequences are of sucking out this oxidized lens and putting Plastic into my body! There ought to be an eye drop that arrests the development of cataracts or dissolves them. Cataract eye drops DO exist for dogs! Reason none for humans? Drs charges U $3200 to $6000 Per Eye to put in these pieces of plastic. No eye dr wants that nice revenue stream interrupted. I have the crescent and eye flickering my surgeon wants to use a laser to cut a hole! Is this right or wrong?

It’s been 2 months and I still having the flickering in my left eye with the crescent shadow and it’s scareing me to death, doctor now diagnosed me with PCO and can’t get the laser done for another 2 months, vision is blurry in the left eye and coudy in my right, I have a toric lens set for distance and am so terrified I’ll never see right again 😩
I had cataract surgery in right eye only 2 1/2 months ago and about 2 weeks after my surgery I woke up to crescent light shadows on both sides, left and right eye.When I close my left eye the shadow disappears on right side. When I close right eye the shadow disappears on left side. Very odd. When I wiggle my nose I see both shadows move so the shadows appear to be a reflection of my nose and face. A few days before my shadows appeared I was diagnosed with mild macular edema and have been taking 2 types of eye drops to calm the edema. I wonder if the drops could cause the shadows or is this ND. My Cataract surgeon does not believe me as she said ND always happens right after cataract surgery. Thanks for your response in advance. Any ideas if this is ND? Both shadows are light and I can see through the shadows.
What about using brimonidine tartrate eye drops?

I too thought the arc a refractive effect of the temporal corneal incision and it would dissipate as the incision becomes more transparent with time. I don't think my doc uses B&L. Now I know that if this arc does not dissipate over time, I can tell my surgeon.
What would you do if secondary ROC failed, i.e light still bypasses the edge of the lens rather than going through it and a dysphotopsia remains? leaving vignetting, with edge of lens ghosting.

Can you help me understand why I keep seeing flutters of light, black line and a few of these flutters of light I could actually feel hit my eye especially one of them I felt that my eye had just been shocked I notice that these occurrences happen more on cloudy days and in the presence of table lamps, these flutters of light most white and some like a color prism. I had cataract surgery on left eye Jan. 3, 2024 and the right eye on Jan 17th. Also my vision has went blurry especially the right eye and the doctor I go to is going to do a yag procedure the end of this coming april because of the reoccurence of cataracts. Thank You Barbara
I guess I'm lucky. I have not noticed ND. My dominant eye changed to the IOL (vivity) after the first surgery. My doctor is doing mini mono so it will be interesting if the dominant eye reverses after the next surgery.

Will shadow go away by the time pass ? I see shadow when I read something for near or far distance. I had Toric Tecnis Eyhance for Astigmatism implanted in my both eye and its been 3 weeks now and shadow still not going away, will it go away by the time pass ?

When you look closely, you notice that when you look at the horizon... Is it possible that this type of lens gives me this negative dysphotopsia?
I have fully recovered from being totally blind for a year from root canal infections, after they pulled all my teeth, please pay close attention to if problems are from bad root canals the patient will not feel any pain and my not notice symptoms are from the teeth.. this almost killed me because nobody believed me and thought I was just drug seeking at 135 pounds now I have gained 40 pounds since I was sick and my 💯% vision back . I am now seeing iol edge glare / light flash after a capsulotomy just appeared after 3rd week. Weird that appears looking up or side to side. Is this different and how to address it. I am a big fan and thank you for your insightful videos. Because of you my mind was at ease as I received my Vivity lenses fyi the eye that had the capsulotomy is the one showing edge light. Dr. Wong, I was blind in my left eye for 2 weeks after cataract surgery. Dr said a piece of residue not removed was blocking pupil. Dr dilated eye and apparently the iris moved the residue, and I was able to see for a day. After a day the residue migrated back somewhat and obscured vision about 30% on the side closest to my nose. What should I do?
How many of my fellow cataract surgeons woud be comfortable doing this? Very impressive...

my doctor missed my weak zonucles during obervation and said she ajusted for it durning sergury im now seeing shadows to the right side should i these shadows move around and come and go its not constantly there based on the light of the room or the position of my head or do you think maybe its broken zonucles moving around makeing the lens move.

I had a right eye cataract operation on feb15 2020, and to date is may 4, i did the check for up to march 5, which i will have another check up on march 15, but due covid 19 we had a quarantine, now my eyes is some how ok, but sometimes not, why i am seeing a floaters, why sometimes if i look on my cp my vision suddenly become blurred..kjndly please help me to unxerstand
As a physician, it is also your responsibility to disclose potential complications, risks and recovery details associated with revision surgery after IOL has been installed some months past. People watch your video and think it's a simple procedure to go and fix dysphotopsia in a 20 minute procedure, when in fact any revision surgery can potentially harm inner layer of Cornea and lead to months of long tedious recovery that potentially requires DMEK to fix that problem. What you ought to start saying is, that if dysphotopsia is bothering you, consider that potential negative outcome of revision surgery can make your life hell if you can't see well due to damage to your endothelia cells. Fixing this problem is like breaking someone's shin bone with the intent of lengthening their height by a millimeter, and not telling them that the surgery can potentially damage your nerves which may impede your balance and ability to walk straight. So would that be worth it to you?
Great video Dr. Wong - I have had regular Cataract surgery both eyes in June 2022 & from the very onset complained as if my hair was falling over the corner of my eyes & was told it was called a ‘shadow’ which would go away! It is now over one year since my surgery and it is still there and I am so uncomfortable and feel as if I have been crying with heavy eyelids besides having to push back the imaginary hair coming across my forehead down to the corner of my eyes - is there help for me in this situation? Most appreciative of any suggestions from you.

I had cataract surgery 8 years ago and still have Negative Dysphotopia. It never goes away. I was given 3 choices. Take it out. Put a clear one on top of the existing one. Or another surgery. I chose none of them.
Wtf. I couldn’t live with it it feels like always looking into the eye instead out to the world. Also I can feel how this bring some tension into my neck.
Dr. Wong, I had my surgery with you almost 2 yrs ago. Is it normal for me to have the arc in bright light in the eye that has the pan optic lens? We redid a surgery I had previously because of the serious problem and you moved the lens forward. Everything was great but, I see the arc and it seems my vision in that eye is becoming poor

I hope you could advise me about my problem. I had cateract surgery last year March 2019 on my left eye and two weeks later I had the right eye done. Every thing went well but after 3 months one day after washing my hair my eyes started paining me, it got so bad I had to go to the emergency eye hospital. I couldn't see light there was this terrible pain. I was given eye drops to take for the next 6 weeks. As soon as I finish taking them the pain return 2 days later. Back to the hospital they did a scan my eyes was inflammed, more eye drops for another 2 months. Same again when the drops finish the pain came back. This went on now up to this day August 2020. I am worried as what if all these eye drops is going to have on my body, how long would it be taking these eye drops. I wish I did not had this surgery done I am worried about my sight. Can you advise me please. I need help they don't have any answers for me.

If you replace an SPA for an Li60 due to negative dysphotopsia, do you routinely place the Li60 in the sulcus? Or do you feel confident enough in to put it in the bag. I'm making this exchange this coming week and was planning to go for the sulcus, it's the first time I've used the Li60.
LI61AO usually goes into the bag.
