- This video discusses "what is FDAR charting" for nurses, and how to chart in the FDAR format with examples. This video is great for nursing students and new nurses who are not familiar with how to chart in the FDAR format. I provide several FDAR charting examples to help you learn how to chart in this format. I discuss each part of the FDAR and what to chart in each section: F (Focus), D (Data), A (Action), R (Response) and how to chart in different scenarios when you don't have to chart a data or response part.

I am new grad working at Nursing Home, They are charting Progress notes using Matrixcare software. I couldn't figure out what form they are using such as SOAP or SBAR. Finally, I figure out by watching your video. It is F-Dar format.

I have been out of the field for so long and now have my first job, learning how to chart again is my biggest issue. What can I do to be at my top peak?
I'm back in it after manyyyyy years. I am having the same issue and I'm slow with all my patients. If you found any tips please let me know
hey Sarah, What does Patient oriented x 3 means? I am a little bit confuse
+Jose Mercado Great question. It means that the patient knows who they are (can state their name), where their at (location), time and event (the year and who is the current president etc.) Sometimes it is referred to as oriented x 4....depending on how your were taught. I hope that helps!

Is there any explanations for NARRATIVE CHARTING?
What is the focus when taking over the patient
At my nursing school they only have "DAR" would the F be included the D ?
Some nursing schools teach it differently. The F stands for focus...which is the purpose of why you are writing the note. You could include it in the data if you felt it was necessary. Thanks for watching the video :)
