    Do you think a zirconia crown or is always the best option?? What would you choose between a metal crown vs ceramic crown?
    In the past, it was easy to suggest a type of posterior crown for a patient. It was either a metal or PFM. Today, we have so many more material options that, even we dentists get confused suggesting the best option.
    This video will give u a clear understanding of the different dental crown materials available to us and the different case scenarios where each would be preferred. You will learn about the different types of dental crowns and the explanation for using Metal crown, a PFM (Porcelain fused to metal crown), PFZ (Porcelain Fused to Zirconia crown), Zirconia Crown and Lithium Disilicate Crown (Emax) depending on the case.

      Tilite crowns are considered as PFM.. Where medical grade alloys as they claimed are used in casting. During its launching, they claimed that it will eliminate the common problem of PFM (grey lines in long term) but clinicians observe it in long term practice and as expected, it failed.

    Very informative and detailed, however I'd like to point out that metal implants may have an impact during MRI scans as seen from other videos so this can be taken into consideration by those opting for monolithic / layered metal crowns.

    Hands DOWN the best video I have come across researching crown procedure. I went to my current dentist. Obviously she is in "business" to make money which I'm AOK on. Nevertheless I'm naturally hesitant to just.. proceed.. with a important decision, car repair as an example. Unfortunately dentists mirror auto mechanics in that they don't have good sales abilities. Understandably they want to take the path of least resistance. So. I have to get my tooth fixed. NOW I'm 'squared away' with the information necessary to have the proper dialogue BEFORE going forward. I'm DONE researching. Thank you DOCTOR!

    Hi Doc, great video. I've got a problem , can you help me please?. I had 2 gold crowns installed on premolars right side upper jaw 5 wks ago. The gold crowns are 70% gold, rest are various noble metals including 8% copper in the alloy mix. In 2016, I had 3 gold crowns installed left upper jaw. A premolar, & 2 molars. I'm now tasting a copper metalic taste in my mouth. I never had this problem till the recent gold crown installation. I have no amalgam fillings. Both left & right side lower jaw ,I have PFM on my molars. I am in good health, I don't take any type of medication or recreational drugs. My dentist advised it a galvanic reaction between dissimilar metals in my mouth causing the metallic taste. My dentist has recommended removing 2 new gold crown & replacing them with monolithic zirconia crowns. What do you recommend? I look forward to your reply. Regards James in Queensland, Australia.

    I currently have ceramic bridges. I have a cavity and needs to be treated. My dentist suggested Zirconia bridge for long term use. But they are very expensive and my insurance is not covering them. My insurance covers porcelain bridge. Please advise look wise if porcelain bridges ok. Thanks.

    Sometimes when we have to remove the crown zirconia crown is the most difficult one.
    For long term relationship which crown would you suggest for the patient.?especially for molars where esthetics may not be the concern.
    Somehow I feel pfm better over zirconia. Your opinion please 🙏

    What type of dental crowns and inlays/ onlays do Dentist’s themselves use for their posterior molars. I had a Dentist show me his yellow high noble gold crown. Dentist’s are no longer trained in conservative gold restorations because of costs and esthetics.

    I have done with root canal yet to put crown, because of small teeth hight doc suggested me to go with pfm but I wanted zirconia, as per doc zirconia will not fit and give strength as pfm will for the back teeth, plz suggest me should I go with zirconia or pfm as per my docs advice

      Hey!! So, the risk of oral cancer increases with a crown which irritates a part of your oral cavity be it metal, PFM, Zirconia etc. So if you have a faulty crown which is hurting your tongue or oral mucosa, you should get that replaced.
      However, there are studies supporting that metal crowns could be a contributing factor for oral cancer...especially if you have other dissimilar metal based prosthesis in your mouth (silver filling/other metal crowns)...this produces galvanic currents...which again irritates your oral mucosa/tongue etc and may result in oral cancer.
      Don't worry..i'm sure you will be fine with the metal crown...if you still have any apprehensions, you can keep checking for any whitish/ reddish lesions in the oral cavity near your crown time to time. And if present, you may have to replace the crown.
      Also, this is possible phenomenon only if u have other fillings or crowns of a different metallic material.

    Hands down for posterior molars high noble metal gold crown, inlay or onlay are the best. Durability and biocompatibility.

      I'm looking at having crowns put in on one upper molar on each side. One will be just a crown on my natural tooth and the other will be a dental implant with a crown. I've decided to definitely go for a zirconia implant but am trying to choose between gold and zirconia for the crown. I was already leaning towards gold. Your comment might have just pushed me over the edge so thank you.

      Gold restorations if done by a Dentist who specializes in gold inlays, onlays and gold crowns can do a far better job than having composite materials and white colored crowns. Gold have been used for centuries. Check out Dr. Stevenson dental gold restorations. Beauty is in the eyes of the beholder.

    Chrome cobalt ceramic alloy is the strongest and can be made thinner to 0.3mm copings. Best for long span bridges with less bulky connectors.
    The PFM is considered as gold standard and all monolithic and all materials are compared to the very good pfm in all ways.
    I have seen very few dentists know how to prepare a for a monolithic or pfm crowns.
    Chrome cobalt pfm crowns are the best,bas compared to all the new materials available today.
    Porcelain fusednto zerconia is a failure because zerconia is too hard and not flexible like cocr alloy so the ceramic fractured easily and not advisable.
    99 % dentists do not know how to prepare the crown properly and do not know to take good impressions. They do not take complete impression and also proper bite.
    Even for a single crown full arch impression is required so the technician can check lateral movements, so dentist do not have to grind high points in lateral movements.
    Most dentist avoid making temporary crown. So in few days the prepared crown shifts and the properly prepared crown painstakingly by dengan technician doesn't fit, then the dentist blame the technician for not making proper crown.
    Most dentist are very careless I have seen in my 37 years working as a dental technician.
    The reality and truth is that even prothodontist do not know how to prepare the crown and take proper impression.
    CrCo Pfm crown will remain gold standard always. Cad Cam tools are only as good as the operator. Most of the Cad/ cam operating in dental labs are not dental technicians and they do not know dental anatomy at all. Only an experienced and expert dental technician can produce properly fitting dental restorations with the use of Cad/cam.
    If an sculptor do not now anatomy of the body hoe cam he sculpt a human sculpture on Cad software. The same is true for dental technicians, who has not learned dental anatomy in his/her life properly, and the same is true for dentists. How many dentists can carve a single central incisor properly. One in thousand.
    I do not criticize dentists but the truth can not change. No dental college in India, make dentist make 100 pfm crowns in his college studies, non.
    I can go on writing but I will not waste more time.
    All dentists know the truth of themselves.
    99 percent dentist do not follow the ethical rules set by Indian dental council. How many dentists know about the ethics book pdf available on the site of Dental council of India.

    I think one disadvantage is wrong about monolithic zirconia .: the wear generated by zirconium oxide on the antagonist enamel is less than that produced by f porcelain as long as it is well polished (Jung et al. 2010, Tambra et al. 2010, Preis et al. 2011, Rosentritt et al. 2012, Kim et al. 2012, Mitov et al. 2012, Janyavula et al. 2013, Stawarczyk et al. 2013, Preis et al. 2013, Stober et al. 2014, Burgess et al. 2014).

      Yes, I think I mentioned that as a disadvantage in the beginning...but later on in the video, I did stress on the importance of zirconia polishing to avoid tooth wear.

    I went to a new dentist, because my dentist retired, I'm 62 and had some bleeding when I brush my teeth only in the front bottom left of my mouth, the new dentist cleaned my teeth and told everything was ok, except I needed 3 crowns on the teeth that I chew with because they had been filled so much they might crack or break, but what else can I do, I can't afford those,do I take my chances with nothing, my old dentist just retired 1 year ago and never mentioned crowns or caps, he would just clean my teeth every 6 months and my teeth were fine, should I get a second opinion, because I kind of feel like it's a money thing, oh yeh the new dentist said they will handle everything for 1700.00 for each tooth, I need 3 teeth crowned! Help from everyone is appreciated, thank you.

      You can go for either. Metal if you are not too concerned about the esthetics as the ceramic / porcelain on the PFM could to chip off over time exposing the metal.

    The original veneer on my front tooth fell out before and was reattached by my current dentist 3-4yrs ago. Recently it completely cracked and I will need to replace it with a crown. Which type should I get to cover that darkness if he isn’t able to clean it and something that looks natural yet strong?

      Depends on how discolored the tooth is. If it is mild, it can be masked with either appropriate thickness of Emax or High translucent zirconia crowns. For severe cases, an opaque layer will be needed...so an option like layered emax or layered zirconia will be good in my opinion.
      Don't worry, your dentist will probably check the degree of discoloration and will suggest you the best option. :)

      Or the use of a blocked out Zirconia. It works perfectly. Either layered or Multilayer full anatomical Zirconia if there is sufficient thickness. Layered is the best in my opinion

      It can last many years...provided the fit of the crown is not compromised and you take care of your oral hygiene.

    Hello ma'am.Recently I have done my rct with pfm on upper right last molar tooth.Since then there is mild headache( everytime) at one point just above the molar teeth.There is no problem with biting and chewing the food.Please help me.

      Hey there! There is mostly a point on your crown which is high, it needs to be adjusted. I would recommend you to visit your dentist and get that fixed.

    Thanks! Great info! One question pls. Pls do more videos in CAD/CAM crowns, inlay/onlay restorations, individual crowns vs joined blocks where no tooth is missing but multiple adjacent crown are required. Also, pls be honest, why do dentists charge such high prices for crown, when the actual lab rate is just a fraction, at least in India. Also, pls educate us on the guarantees that come with the PFM CAD/CAM crowns, is it just a gimmick, will they provide a replacement if chipped in guarantee period? What is DMLS crown? PFM vs PFZ, is PFZ more expensive?

      Coming to your question....Well, you are not really paying for the lab charges, the dentist is charging you for their skills and medical advise. Even if something as small as crown cementation goes wrong, it can have devastating effects on the surrounding gums or opposing teeth or your neuromuscular system. So, everything has to be done with utmost precision.
      Guarantees are provided by the lab...and in my experience, if your crown chips off or fractures, they do give a replacement for the same.
      DMLS is direct metal laser sintered...in simple terms, 3-D printed crowns...they have a better fit compared to conventional ones.
      PFZ is more expensive than PFM crowns, but they have better esthetics and fit as well.

My question about the high cost of crown was because more than one dentist told me a PFM manual crown was X amount and same done by CAD was 2000 rs more. Based on what you just told me, if the cost of the crown is made up of the dentist charges, his/her work for both types of crown is the same. Then why such a steep price hike in charge? This prompted my question.

    I need to get crown for tooth #3. Which is a better crown Mono Zerconia or PFM? I'm kinda having second thoughts after watching your video regarding polishing the Zerconia because the opposing tooth is natural and can get damaged. Should I just get a PFM?

      Zirconia is better…almost everywhere polished zirconia is given now, so shouldn’t be a problem

      No side effect of zirconia crowns that I know of...zirconia crowns are pretty good, so you can go for it.

    Mam, what is the treatment for advanced Gingivitis

      For gingivitis, you should start with scaling and root planing followed by proper oral hygiene instructions to the patients. Most cases of gingivitis resolve with this conventional protocol.
      If the case has progressed to periodontitis, apart from the interventions mentioned, surgical procedures may also required with or without antibiotic coverage.
      Every case is different, you need to 1st identify the cause and then plan your treatment protocol.
      Refer the case to a periodontist for better results.

    Mam, just one more thing.there was this patient whose upper right 6 (UR6) crown was very short.so i gave her PFM for UR 5,6,7.she wants to submit this in her office for reimbursement.Is it ok to write in this way in prescription: Diagnosis- insufficient crown UR 6 Treatment- 3 unit PFM given for UR 5,6,7.
    (And then rate with cost of treatment.)

      I guess you could write your prescription that way....
      But, why give a 3 unit FPD for a case of short clinical crown height with UR 6...why not do a crown lengthening procedure to increase the height?

      Yup! Emax will definitely give you best natural appearance...these days you have natural looking Zirconia crowns as well. So it comes down to the case that is in front of us. The ultimate selection depends on a few factors like discoloration, amount of enamel present for bonding, tooth structure we have, occlusion etc.

      Porcelain veneers are great too...minimally invasive and very natural looking.
      Decision between crowns and laminates depends on the case.
      Not every case is suitable for porcelain laminates.
      So, if you are looking for a smile makeover/design, it all has to go through a process of treatment planning which involves clinical and radiographic examination, digital photographs, study models etc. A mock treatment is usually planned on the models 1st and the best suggestion is given to the patient accordingly.

    I am having pfm crowns in front and they feel totally artificial... Very thick...like something is against my tounge , my tounge keeps constantly touching the crowns and it is covering my gum line so i feel sensation in the gum line under the crown...having constant tooth ache..but my dentist says everything is fine and i am overthinking..what should i do?? I am very depressed and don't feel like living
    I want to throw them away and live with my natural teeth

    Hi Doc..my dentist gave me two crown option for molar teeth (upper right 6 &7). first is 3M Lava with 5year warranty and 2nd is 3M lava with 10 years warranty. Which ll be the among these two options according to you? Is there any any major difference between these two models.

      Zirconia crowns abrade the opposing tooth only if it is not polished. Even before glazing, it is better to get your crowns polished. Convey the same to your technician.
      Also, if you happen to make any adjustments on the crown before final cementation, make sure you polish the occlusal surface again. Either you can keep a zirconia polishing kit at your clinic and do it yourself or send it back to the lab.
      But, it is mandatory that zirconia crowns are polished :)

      You can go for Lithium Disilicate crown (Emax)...Wo best option rahega if you want it to mimic a natural tooth.
      Other option would be a translucent zirconia crown.

    Not Mentioned:
    1. Translucent zirconia has the special advantage of hopefully being able to see when a tooth starts decaying beneath the crown.
    2. Opposing zirconia teeth don't wear as the other teeth wear, which later leads to toothaches as these teeth start carrying more than their share of the chewing forces.

      Both zirconia and emax can be fabricated with minimum thickness of 0.5mm.
      Zirconia is better suited for posterior crowns as it has better strength.
      However, according to me, if your case demands a minimum prep, the crown (either zirconia or emax) should preferably be bonded on the tooth for added strength.
