Super Mario 3D Land World 4-4 is easily the scariest level imo, besides being a general ghost house level it’s mostly because of a certain Easter egg that’s straight out of creepypasta
Strangely enough, the haunted mansion levels in Mario games aren't the scariest to me, at most I might be startled by an unexpected twist or something similar for a few moments and then I'll shrug it off quickly. So what does startle me? I've nicknamed them chaser levels, basically any level where you're being chased by something, whether it be rising lava, poison gas clouds, or something else. For whatever reason I can't handle being chased by something, no matter what thing it is, whenever I come across a level with that kind of thing, I'll chicken out and ask a friend for help on it, and I can't even seem to handle watching someone else play them. And if I'm caught off guard by one, I'll quite literally panic in reaction to it. (Little jab at myself with this, funniest reaction has to be when I went into Super Mario 3D World's 6-7, Fuzzy Time Mine, blind and not knowing what to expect. Went in thinking it'd just be a normal auto scroller, only to be absolutely terrified to realize I was being chased by a cloud made up of a bunch of black puffballs lol)
The spooky music from Super Mario 64 is one of the creepiest soundtracks I've ever heard. Rule the World from TheFatRat & AleXa (Korean: 알렉사) and Legends never die from League of Legends (Remix with Alan Walker as well as Original only with Chrissy) for example are two of the most beautiful songs I've ever heard. You should also listen to this songs. They are so beautiful !
I'm surprised that World 4-4 from Mario 3D Land wasn't mentioned because of the ghost girl at the end of the level and it appears behind the fence and it will appear and disappear. I remember that will kinda gave me the chills when I was younger. Also, Nico is so underrated for his new videos and needs more views and love.

A great selection! Not gonna lie, I thought Sirena Beach from Sunshine was a bit unnerving, with Boos haunting the hotel now and then, and a freaky boss that references The Shining of all things!

There’s also another Van Gogh art style level in the Mario franchise. In Super Mario World 2: Yoshi’s Island, in the 2nd to last level before Baby Bowser’s Castle called Keep Moving, there’s a section where in the background, there are elements of the Starry Night painting. The whole game’s art style a painting or coloring book.

Number 3 deserved to be number 1. The main issue of the clown is THE SOUND IT MAKES! When he first chases you, he starts to scream his lungs out and everytime is a jumpscare because you can't see him. I lost lives so many times during that level because of him, I hated it! 😆 SM64 Haunted House wasn't that bad, but probably I would have been scared too, if I played it during my childhood. 👍

Might I add that the ominous xylophone theme that plays for Banshee Boardwalk makes it creepier... and when you realize it's the game's real Rainbow Road Also, Painted Swampland isn't the only level in the game with that style. 9-5, Spinning Platforms of Doom, has this style too. And yes, Forever Forest creeped me out a bit when I first played it earlier this year. The music, sense of loss, creepy flowers, atmosphere, those stupid Piranha Plants, and the feeling you could choose the wrong path and have to start over all contribute to that... …but the scariest thing is a Blue Shell on the final lap of Rainbow Road in Mario Kart Wii when you need 1st to beat your brother and win the GP
