    I was finally diagnosed with this condition late last year. I think I've had it for a long time, but the symptoms have come and gone over the years. Of course I went through multiple courses of treatment for things like bacterial infections first, but I knew it wasn't any of that due to the symptoms and finally a urogynecologist suggested it was a neurologic issue. I started looking around myself and found pudendal neuralgia. It matches my symptoms almost perfectly. A burning pain - for me in the vagina in the urethra area and closer to the perineum with urinary urgency - all of which is made worse when sitting and eases or goes away when standing, lying down or sitting on the toilet. The pain is usually completely gone overnight and slowly comes back as I start my day. Basically everything he said.
    I went to a specialist for multiple nerve block treatments, I use muscle relaxing suppositories at night, take pregabalin and I'm doing pelvic floor PT - which has revealed a lot of tension in my pelvic floor. My therapist thinks all of the coughing and sneezing that I did when really sick made the pelvic floor very tense which irritated the nerve.
    I will say that all of these things have made an improvement. I'm still not pain free, but it's much better than it was before I started treatment so hopefully with time it will ease and I can feel normal again.

      I am facing the same, I also have PCOS So I thought it's pain in my ovaries but, I feel fatigue and pain around my vaginal area and in my hip, I've been listening to anti cancer frequency thought it's cancer, but I get relief when I listen to peripheral neuropathy frequency by Good vibes on UA-cam. But I don't listen to consistently that's why the pain comes back.

    I was in tears watching this - thank you for being so clear and thoughtful in your explanation. This is EXACTLY what I’m going through and I can share this with anyone who needs to know.     I am a 70 years old retired woman surgeon. I have had irritable bladder and detrosal muscle symptoms for a few years. About two months ago I started experiencing perineal pain and severe urethral irritation with frequency. I had no urinary/bladder infection or cystitis. I saw a urologist and a gynaecologist who found all is normal including abdominal and intra-vaginal ultrasound. Paracetamol eased the pain a little. I researched my symptoms and believe I had pudendal neuralgia which as you say is twice as common in women but frequently passes undiagnosed. I started exercising my pelvic muscles and continued with my stretching exercise routines. The pain subsided completely within a week and I have been pain free since then. Of course it may return. Thank you for shining a light on this distressing condition that is rarely considered particularly by general practitioners.

      Nahid toubia , can u please share Ur exercises. I'm a physician and started having it 4 months back . I have endometrios , deep infiltrative and had a UTI after ocps withdrawal after which I strated having severe right labial and right sides vaginal pain which would get worse on sitting , improve on walking. Also had burning at the same site. Later started having pain in medial calf and last two toes , the sacral plexus has something to do with it
      I consulted pain management and they said it's pedundal neuralgia , took amitryptalline and walked , things got better but now again after I sat for a long time things got worse.
      Would be a great help if u can let us know how u got rid of it

    I was treated for fungal infection, yeast infection, herpes and clamydia and everything under the sun to find out why I was feeling this way. I thought I was crazy at first. I ended up not having any of them in the end, the doctors said I have this!! Its bad now but I’m just glad I don’t have any STIs. Thank you for your help :) I will be getting treatment.

      My doctor has prescribed antivirals for me for herpes and wants me to do a swab but I know he’s wrong. I know I have PN. The pain is like no other. However, I still have pain at night; it doesn’t cease at night

      Nothing helped me and I had pain 24/7. I ended up having severe PNE. The nerve was decompressed in three spots during my decompression surgery. I had my right pudendal nerve pinched by ligaments. My inferior rectal nerve was involved as well.

    I developed this at age 16 after suffering chronic constipation for a long time and running excessively, stress plus a strong enema at the hospital to try and relieve acute constipation. Nobody could tell me what caused the condition. It made my life hell. I had to drop out of sixth form and give up on going to university. I found it so stressful that it lead to chronic fatigue syndrome.
    I now understand this condition and started to in early 20s thanks to the headache in the pelvis book.

      My brother also got this because of chronic constipation. What treatment options are you pursuing? Nothing has worked in my brother's case unfortunately. We are hopeless as of now.

      I ve been suffering this for 13 years and I got constipation always I thought this maybe be related to constipation and finally I found someone who had this because of constipation I don't know If I will get cure from this some day I feel like will never end I do stretches helps a bit but never can come back to normal

      I think meditation with the pain is very important to be able to let go of the resistance to it and the guarding against the pain which keeps everything tense down there. Paradoxical relaxation you'd call it. For me stretches tend to aggravate things so I just do gentle yoga in the mornings.
      The meditation that I find most beneficial is called soften and flow. Focus on the area of pain, breathe into it, then on the out breath gently say in your mind soften and flow. Don't expect miracles but over time as you get more used to sitting with the pain rather than distracting or running away from it, things will improve.

    Pelvic floor therapy has helped me reduce most symptoms by 75%. I went 4 months with severe pain.All test came back showing good health from 3 different urologist. My 4th urologist suggested pain management and pelvic floor therapy. Starting my third month of treatment now. I have good weeks and bad weeks but no where near the pain before treatment started.

      @@hcs.c 95% better now. Little to no pain each day. Pelvic floor therapy in addition to breathing exercises to calm my nervous system really helped. I used wim hof method for breathing and a program called heal pelvic pain for men (online course) in combination with a local physical therapist. I had three flare during the couple months of recovery but now they have stopped.

    I can not thank you enough for this. After years of being told by doctors this pain "is nothing, it's all in your head" I think I have found a great starting point. You are describing exactly my symptoms and with this I think I can start finding appropriate treatment. Thank you again!

    I went thru this excruciating pain in my early 20s because of a yeast infection. I suffered with this for 4 years, and it went away with the birth of my son. After giving birth, i never felt that pain again 🤧🙏

    Look up the sacrotuberous ligament. Chiropractor did a technique to release that ligament and it instantly gave me relief from the sacrum and groin pain from the pudendal nerve.

    I’m in a FB support group for PN and in all of us there are so many differences in what aggravates it, what helps the pain (including meds) which areas are painful and which are pain free. However, in 100% of cases, upon waking the pain is entirely absent, for moments only. What happens in the brain during sleep (not relaxation since we’re still relaxed when the pain reappears just minutes after waking)? If we could answer this, that would be valuable intel on what is going on that so affects the nerves to cause such horrific pain.

      can come from nowhere walking just fine then scream the ceiling off shopping centre can happen anywhere massive electric shock in testies _ wow yes I understand. can get itch lying in bed pins and needles in backside and pennis horrific. so I go to the toilet and do internal massage horrible but stops the itch. life can be a bitch but it's bearable not worth hurting yourself over life's just to beautiful cheers lol Australia.

    Thank you. Mine isn't worse when sitting. It's worse when standing or laying on my back. Laying on my tummy feels best. It's been 2.5yrs and it started after rotator cuff surgery. I'm getting a diagnostic pudendal nerve block next week. I'm in perfect health other than this. I'm praying 🙏

      @Lisa Colorado I did not get it. It's a small intestine problem. I'm fasting right now then going gluten free forever and milk/cream free for a couple months.

    I think this is what I’m experiencing. After two years of pain and research I feel that this moist describes what’s happening to me. I hope I can see a doctor soon and mention this. I’ve actually lost my physically active job and health insurance due to these complications.

    What a wonderful content. I am a 39 year old woman, I have started with a pain in the hip area, but under the Gesäßmuskulatur. Any doctor can tell me what it is and for that reason I am trying to find out what it could be. this video is fantastic. So clearly and in detail everything explained. It is also amazing to understand and know more about the human body even if you are not a physiotherapist or a doctor

    So grateful you explained this! Mine is constant, sharp. Worse than childbirth on BOTH sides.
    Everyone says they have pudendal neuralgia like I do but they’re symptoms are very mild and only occur during sitting.
    Mine are constant and torturous

    This pain is truly a type of hell on earth. I have had it for 5 years, but it probably developed over a period of 30 years from intense mtn biking. I have had to become an expert on the subject, unfortunately. This is the sad reality: there is only pain management, no cure. Many providers promise cure, but the literature indicates cure is rare.
    Suggestions: nerve blocks, medications, lifestyle modifications, psychological treatments for pain management, PT, stretching, and avoiding hip flexion (like squatting, bending). I'd also recommend joining a support group because this condition raises risk for suicide, per existing research.
    Just don't give up seeking help- if you can cope with this, you can deal with anything else life throws at you ❤

      had it for 18 yrs _ hit me aged 44 now 62 full time cyclist. first sign came from nowhere hit the bedroom ceiling electric shock in testicles_ beginning of the nightmare. yes the pain is overwhelming at times but manage as it comes and goes. sex is the most soul destroying pain is just to much to cope. no actual pain sitting it's when I get up walk I'm crippled with spasms shopping centres restaurants etc. made my own seat/pad with a 2 inch gap I carry around in my bag ( seriously life changing ) I tell people I have a broken coxic bone which is actually true broken age 16 which I believe has contributed. so I'm back to the pool exercises etc drugs dnt work. my biggest concern is in my older age on my own how will I cope if in pain_never considered hurting myself as I have lived in chronic pain since a child and have copping mechanisms but I appreciate the living hell people go thru but there are solutions just takes mindset and dedication. mine flared up with vengeance after 2 good yrs_ so back to the drawing board but I know it will get better _ that bloody bike.

    A can relate to the guy you mentioned at the end of the video 😔. Same symptoms. I told the Doctor this could be my problem but he doesn’t now much about it.
    Doctor keeps telling me all my exams came up good, but I know what I feel and it gets me depressed having this pain and nobody be able to help.
    How did the guy you mentioned heal of this terrible problem?. I need help 😔.

    My insurance company demanded an EMG before I could have a block. I went to the EMG specialist, and I had to show the dam doctor a picture of the nerve. He never heard from it. I was beyond pissed off. I live daily in extreme pain, waiting to have this block done.

    4 years I am suffering from levator ani spasm and I did botox and I didn't help I am 67 and the pain so frustrating everday no stop but the pain disappear around 5 or 6 pm very strange even the 4 colorectal surgeon visited they don't know why now I am waiting for the pain management doctor to do pudendal nerve block injection and I hope it helps and I tried lyrica and valium suppository but only for one month it helped but I did not continue for not being addicted

      Hi @nadiazeidan54, let me know, whether the pudendal block helped! If your pain consistently improves around 5-6 pm, take a look at the TCM organ clock: according to this TCM clock, your kidney is supposed to be most active, while the large intestine is in its main resting phase. That would fit, wouldn’t it?
      Have you already tried gentle dilator training to relax the rectal muscles? This needs to be done frequently and regularly, but it has been shown to effectively relax the muscles.

      I've noticed some relief with this medication as well. I'm on a very low dose, but I started feeling improvement within a week. And far less side effects than I had when I was on gabapentin.

      @@MdnightWnd I've not noticed a improvement yet be taken pregabalin since the start of December on 100 mg per day with no side effects and they don't make me feel hungry which is a plus hopefully they will help

      My dosage is only 50mg per day and thankfully I haven't had any weight gain or increased hunger. However, I also take hydroxychloroquine daily as well and that suppresses my hunger so that might be contributing.
      My doctor told me it could take a month or more to get any relief from the pregabalin and also said the dosage might have to be increased. So hopefully with a little more time it will start to help you.

    I have heard this continually that pudendal pain subsides when laying down? I have been diagnosed with pudendal nerve entrapment however it only affects me while sleeping? As soon as I arise and walk in the house a bit it subsides and Im usually comfortable during my waking hour? All my symptoms exactly fit my diagnosis??

      It is possible that your entrapment originates from a muscle area that exerts pressure and stresses you while lying down. When you are up and have moved around a bit, this area "relaxes," so to speak, and the pressure is reduced.
      Would a high-resolution imaging scan perhaps provide more insight?
      Have you already tried specific exercises for the pelvic floor or the m. obturatorius internus? What about rectal treatment of the pelvic floor?

    After having a bartholin gland abcess for 3 weeks, two incisions and drainage that were unsuccessful and then it drained on its own a week later. 3 weeks after this I started experiencing all the symptoms that are explained in this nerve irritation. My gynecologist pressed on the nerve but no pain, but it comes and goes and is less in the morning and gets worse throughout the day. This is a tricky situation when nothing is felt at the gynecologist in the morning but is painful for the rest of the day.

    I'm a 26 year old man and not ashamed to admit but I've felt this condition on my right pelvic region, feeling a sharp, prickly pain or sensation emanating above and intermittently towards the right testes. Thank God the "ball" itself is fine but still the nerve pain persists, at least occasionally now thanks to more stretching.
    Any other recommendations would be helpful 🙂

    Hi, thanks for this video, I have one question regarding the story you mentioned about the young guy who used to bend his leg. Can you please explain more what is the treatment plan/exercises that he did? And how much time would it take?

      Hi @KevinHani, thank you for asking. The treatment plan combined a lot of stretching and foam rolling for the adductors on a daily basis. We added light strengthening exercises over time (he is now working out with the Copenhagen plank).
      We treated the obturator internus muscle, gluteus medius, and piriformis. Additionally, we introduced light strength training gradually.
      His commitment was amazing, as he learned how to meditate (not an easy task) to reduce his stress levels and improve his awareness of sitting and standing posture. This helped him decrease the provocation of the afflicted area.
      He also has been on some meds in the beginning but reduced the intake over time. we experienced some flare ups, but in the end he was good to go

    Thank you for your videos. It is very informative 👍. I fall over my back 2 years ago and after three weeks of the accident I felt pain around my vulva and during urination and around urethra. My doctor told me I have descending perineal syndrom and rectocele and the descending caus a nerve stretching. And told me that the fall was only a trigger point for the prolapse to become systematic. My question is ? Is it the fall or the prolapse which caused this firing pain??? And does physics therapy can help with the enormous pain I have lived with the past two years.

    I've started this symptoms recently after laser treatment for a large kidney stone, a month ago, my urologist says 'it will settle, my own doctor treated me for fungal infection. I have had repeated UTis for 6 months before the kidney stone was found. Does anyone know if there is a connection with shooting burning pains down my right leg to shin and toes, like an electric shock. I'm on muscle relaxants various pain killers and treatments for constipation? I need to go back to doctor as nothing helping.

      Hi @libellula3313. Thanl you for reaching out. The issue you are describing is probably not related to the pudendal nerve, as it does not innervate the leg or shin. However, kidney issues can produce all kinds of back problems, and back spasms can sometimes cause peripheral nerve issues, potentially leading to shooting pain down the leg.
      Please note that this is only my assumption based on the symptoms you described and not a diagnosis of any kind. Diagnosing via UA-cam or without seeing and assessing you in person is not possible.

      Hello @MariaKousar-k3x, what do mean by your first sentence?
      The pain is typically more intense while sitting, as the sacrum is subjected to greater stress. The weight of the upper body rests on the pelvis, and the potentially overstimulated gluteal and hip muscles are mechanically strained even further. Ergo: The already stressed pelvis experiences additional pressure while sitting, resulting in increased pain.

    When I was 7 or 8 years old I fell from the top of a tree all the way to the bottom. I hit the bottom branch in saddle position. Ever since then I've had immense pain that I don't know how to describe, i'm 20 now.

    Have recently been diagnosed with this . I have s1 and s4 disc bulges , and the nerve has been affected somehow . I get the feeling in the end of the penis , and it seems to be worse in the morning . When I walk and move gets less .     I get this sporadically, I’ll feel it for a few seconds, then I won’t get it again for months, for me it usually happens when I sit down too long and then I stand up and stretch my abdomen, it’s like there’s a rope attached to the left side of my lower left pelvis and runs down to the tip of my penis and it’s getting pulled casing a burning stinging pain on my penis tip to my lower left pelvis. I’ve been trying to explain this to people for so long and I get laughed at, they think I got a std, or they don’t know what I’m talking about and find it weird.

    Pudental nerve irritation can cause recurrent UTIs in both men & women. Urologists often diagnose this condition as Chronic pelvic pain syndrome (CPPS) in men. I have been suffering from cpps from last 13 years. I am 32 currently. Pain used to happen in my groin area & it was in vericose vein. Doctor classified it as vericocele but even he wasn't sure about pain because it was constant. Thankfully, I had the surgery in 2014 & pain never occurred again. I still have recurrent UTIs though. I would have a pelvic MRI & will look into this information that you have shared. Urologists across the world are stupid it seems.

    Hello doctor, I fell of from stairs and got myself injured and sit bone near tailbone got injured, but after few years i started getting bladder spasms with frequent urination and stiffness near tailbone.       I just learned my systems were from the sacrotuberous ligament. Chiropractor did a technique that released the ligament and gave me instant relief. The ligament is attached to your sacrum so your sacrum could be moved just a bit.

    I had my rectum anus and spinster muscle removed due to sepsis. The wound opened 11 days after surgery and took 10 months of nurses coming to my home every other day unpacking and packing the wound because of this I have severe adhesions entwined with not only the prudential nerve, but also all the nerves of my Pelvic floor and all the nerves that were left all those nerves that control the movement and the peristalsis my doctor did not dissect he cut through all of the nerves horrendous is that and I am left with crippling pain thank God I am an optimistic and positive person and I am working through this as best I can. I was 100% normal before this surgery, riding horses travel to see my kids and my grandchildren walking 10,000 steps a day taking care of the elderly volunteering so many things cooking going grocery shopping he took them for me and I am still going to win somehow I will win!!! Even my bladder is affected.

    A scan revealed that I have a bilateral fracture in my L5 which sits just above my pelvis. My doctor doesn’t think the fracture is significant enough to cause nerve issues. I beg to differ. Combined with internal haemorrhoids and bowel issues, I am convinced that something is definitely going on with my nerves. When I had a flare up, it was as though somebody was inserting a hundred hypodermic needles into my vagina and anus. My vagina was completely numb and I could feel pins and needles at different times running through my back. There was also a rash which covered the inside of my bum cheeks and spread onto my vagina which drove me insane - the itching and burning was horrible. I also suffered 2-3 UTI’s with my urine showing up with blood. Sleeping at night was difficult and I would resort to putting a bag of frozen peas between my legs and up against my vagina and anus which gave some relief. My doctor wants to treat me for herpes with antivirals but I know his diagnosis is wrong. I haven’t had sex in 5-6 years and a test done 12 months ago revealed I have no sexually transmitted diseases. It’s frustrating trying to get help.

    This is an incredible light bulb moment for me. I, a man, was sitting with my right leg crossed underneath my left thigh, for years.
    That must've contributed to what I went through recently. Am improving, just some pain and discomfort which will disappear, I believe, with simple correct sitting.
    Sitting on a travel pillow (donut u shaped) is recommended. Thank you doctor.

      @@SouravMax-dt8xn healing very well thanks. I think what happened was stress, caused temporary issue for months with peripheral nerve, which affects pelvic floor function, and continence.
      As body went into repair mode, everything got better. A big thing to do, at least a few times, is pelvic internal therapy.
      Use finger or pelvic wand, even better, into anus, and do myofascial release of any tight pelvic muscles.

      @@SouravMax-dt8xn I had the incontinence, pelvic pain, couldn't walk properly for a couple of months.
      I'm not an expert, but it sounds Ike it's your pudendal nerve. Fortunately, the pudendal nerve regrows and heals.
      The amount of time it takes differs depending on severity, but you will heal. For now, meds can help. I took pregabalin, was good, still taking for a bit longer.
      You'll get better for sure.

    Why Is nobody talking about the fact that sexual activity is a definite no go and non-existent and also having bowel movements are nearly impossible now?!?! Has nobody dealt with this as long as I have and nobody got into this point yet. And why is mine so bad and why is there nobody in my area that even knows what a prudential nerve is??!? Why have I been to 10 different specialists and nobody even knows what this is much less how to help me???! I am flat out telling these people what is wrong with me and they still can't. What open a book open Google??? I'm diagnosing myself For Fuck Sake!!! I did the hard part. I just do them to do the research on how to help me! I've been living with this for 20 years looking for answers!

      dear I am also suffering of sciatica and pudendal nerve issues as I am suffering of L45and S1 disc issue ..but still I am trying to manage ....yes ur sexual activities may compromise but still u can manage with fascia release and physical therapy only..if don't wannt to go for the surgery

    Pudendal Neuralgia is TMS (Mind+Body Syndrome). AidPlan does not recognize this, and the effectiveness of the treatment is thereby limited.

      @americandream9473 Anxiety is usually a part of the TMS mechanism, yes. I had pudendal neuralgia on my left side for years, diagnosed by numerous professionals. It was all TMS, though, and disappeared completely after 2 months of following a TMS protocol.

      The Plastic Surgeon did a Reconstruction. Told me.
      So the pain at the tip is bad when am sleeping.

    Hi Doctor, I had a fall 3 months ago. After a couple of days, I had a fever which lasted a week. And I felt discomfort burning sensation on my testicles, inner thighs, and anus. I also noticed the vein on my head of penis is very obvious and feeling a little swollen. Now, the feeling just comes and goes. I also have a very sore back pain as well as knee pain. May I know what condition am I having? I'm feel very lost and depressed.. 😢

    Who treated this guy, im literally that guy early thirties and 4 years of this pain. Where is this guy i will go there
