Pandemonium! [PS1, 1996]

This is one of those games I have a strong nostalgic-laden memory of, so it's actually a big disappointment to discover many years later that Pandemonium has aged incredibly poorly. Not only is the gameplay rough by today's standards (the 3D movement is confusing, and the 2D platforming is really rough to line up jumps), that intro video is really hard to watch. It's not the primitive CG, it's the voice acting and general tone. This game was cool at the time, but you can absolutely tell this was made in 1996.
