  • Starting as a cancer surgeon and researcher, Dr. Peter Attia would never have predicted where his professional career would lead. Between long work days and grueling swimming workouts, Peter became an incredibly fit endurance athlete somehow on the brink of diabetes. That turned out to be a benefit for the rest of us because what followed was a self-described obsession with health and longevity.

    Dr. Attia, please don’t EVER dumb down your podcasts to make them less technical than they are. I’m 71, a retired lab tech, and I just love that you make me stay on my toes to get to the meat of the subject. References are included so I can always dig into the research if I need to. My body doesn’t work as good as it once did, but stuff like this keeps my brain sharp. Heartfelt thanks!!

    Dr Attia is the King.. A genuine person before all, who you know will explain the truth to his best ability (most self-tested). He makes it clear what he says may not be correct or he may have followed other routes before but he is trying relentlessly and passes this feeling along. Keep it up Peter..

    Always a joy to listen to Peter Attia! Great questions and interaction in this interview. Love the depths to which Peter's "cyclical strategies" are explained as well as played out in his own life. I also appreciate his lack of allegiance to any one dietary strategy, but instead looking at the patient, what their goals are, and considering the best individual approach. I didn't realize he also has a podcast. Will follow that one too! This podcast makes 2 for 2 outstanding podcasts for The Diet Doctor!! -Julie Cottrell, MS, RD

    Dr. Scher, your questions with other “woke” doctors are first class. Attia can often appear arrogant and bored in interviews. But it was clear from your insightful questions that you pulled him out of that posture and produced an outstanding discussion helpful to many of us working on healing and optimizing our own health. Also, your interview this week with Dr. Carrie Diulus was outstanding. Again, your probing and informed questions prompted a great discussion helpful to so many of us. Particularly her experience with the metabolic effects of resistance training vs. cardio training for a diabetic was new and exciting, since I am pondering the same in my life as a fully keto-adapted, glucose sparing former T2 diabetic. As is so often the case with Dr. Eenfeldt, he has made exactly the right choice with whom to affiliate for this podcast. 👏👏👏🙏👍👍

    Wow! First you had Gary Taubes for your inaugural podcast, and now you have Dr. Peter Attia. I can't think of a better way to get attention to a fledgling podcast than having these two intellectual titans be your first two guests. Great job! Now, if you can just get both Dr. Ted Naiman and Dr. Jason Fung to be your next guests. That would be awesome.

      @DCD I always find comments like this amusing. It takes all of a single sentence typed by someone completely unknown with no qualifications on the internet to present skepticism about someone like Attia, despite his endless citing of research and argumentation on his own end.

      @DCD Again, all you're doing is hand-waiving. Where's your science? Attia cites dozens of studies per article. Are you even familiar, remotely, with the literature on this topic? Are you aware of the randomized controlled clinical trials on low carb diets? Do you know what good experimental design is?

    Listening to this podcast continues to provide me "food for thought", as I travel my own journey of "experimentation" in eating by having removed, for the most part, sugars, refined carbs and grains, as well as seed oils, from my eating plan (I don't like to use the word "diet", as people get a misguided idea of its purposes). It's too bad, on one hand, that we have to rely so much on seeing our doctors for blood test requisitions, as sometimes it would be helpful to know "now" how our bodies are responding to changes in our eating etc. 'I live in Canada, so finding Diet Doctor has been so rewarding! Being retired gives me more time to learn. Thank you Dr. Sher, and Dr. Attia! I enjoyed watching this podcast.

    Great interview! Peter's podcast "The Drive" is by far my favorite, mostly because it doesn't feel the need to talk down to the listener... By that, I mean that I constantly 'look up' definitions, terms and other subject matter that would normally make the podcast 2-3 hours if they explained everything. It makes me a less "lazy" student of this science. Diet Doctor is on the forefront of all the great news coming out from this lifestyle... for that, I thank you.

    I'm going into my third year on the nutritional keto diet. My blood beta-hydroxybutyrate ranges from .60 to 1.90 mmol/l. I am not too concerned about the ketone level because the level will fluctuate given the balance between ketogenesis and ketolysis. Stephen D. Phinney, MD, Ph.D., co-author of "The Art and Science of Low Carbohydrate Performance" uses .50 mmol/l as the minimum.
    My workouts are no impaired since I have been on the keto diet. I still get a pump with high rep exercises which indicates glycogen storage not detrimentally reduced. I'm 185 lbs with body fat at 9.7% measured by DXA scan. In fact, my lean mass has increased over the ten months period between two DXA scans.

    Lots of if's here,
    My blood glucose rises for a protein/fat only meal and my ketone level drops also plus if I eat any carbs the effect is greatly enhanced.
    Since there are no essential carbs I'm surprised to hear Attia refer to this type of diet as extreme? I have to eat less than 20 grams a day to stay in ketosis which runs from 0.9 tp 2.9 on carb-less days. I check my glucose and ketones three times a day and will continue until I find what drives my glucose.
    It's not cut an dry.
    I'm losing between 1/2 and 1 pound a day which is OK because I'm still obese, I was prediabetic but not now and I'm also fasting for 24 hours a day and I'm down 25lbs...

      Why would want to stay in ketosis? Are you epileptic?
      "Since there are no essential carbs" - stop parroting idiotic lies of the keto-sect.

      Late reply, but maybe the amount of protein in your meal is too high. Excess protein converts to glucose... you can Google the research.

    It would be great to have a doctor or scientist or just plain old wise person in the more “mature” age cohort (i.e., over 60) discuss these things. Someone who does NOT exercise like crazy or have incredible athletic ability or a history of amazing endurance feats, but still managed to live long and healthy, is not on 12 medications, is not a gym rat, and can give us regular mortals some info on what they have gleaned from their observations over the decades. For us Boomers, all this mTOR talk, etc., from buff young men is interesting but perhaps not very helpful. Thanks in advance, if you can find such a person 🙂

    Can someone please rephrase what Dr. Attia says at 35:45 about mTOR and cancer and degenerative diseases? He says something like “it’s almost sure that restricting calories and protein protects from cancer but who knows about degeneratives”?

    ❤️Hi it’s Amazing it’s not a new thing to our ASSYRIAN PEOPLE its been our favorite dishes for over 7000 thousands of years. We have been using it till today. beef lamb and chicken broth soups and we have a special dishes made Especially in winter the first one called PAJA it’s all made from sheep’s, lambs bones from his head and lags. With the garbanzo beans.The Second dishes is made from beef or lamb Bones as soup we called TASHREEB with Garbanzo beans. We use it as a meals . Serve it with green onion Spinach, Parsley and relish With delicious bread.

    Another issue that I have experienced is that when I go on a high fat low carb diet is that my LDL jumps, have anyone experienced that on ketosis?

    Would like to hear Peter's take on Aubrey de Grey. For example, Aubrey says that Rapamycin is the best of the Calorie Restriction mimetics.. yet CR itself has less impact as you go from worms (500% increase) to longer-lived species like mice (150%), dogs (10%) or monkeys (1%). Therefore, any strategy that utilizes CR, either directly by fasting or indirectly via Ketones/Rapamycin/Metformin, will not significantly impact longevity in humans (

      Master energy regulator of the cell is AMPK. Always was. Rapamycin in BS and has major side effects. Rapamycin is also PRO-diabetic, whereas AMPK (activated through exercise or fasting) is ANTI-diabetic.

    Started vit D3 4000 iu then stayed at 8000iu from october UK. Then a vit K complex a vit B complex Magnesium glycinate 400mg per day. Collagen also zinc & copper tablet also tumeric ginger pepper. Also an 8 blend myshroom capsule. Vit c 1000mg per day also a 1000mg Omega 3 capsule twice day and flaxseed oil caps also a multi vit on days dont take zinc & copper what use think also chorella spirlunia tabs

    About G6 vs Libre - for some people G6 is more accurate, however, for other Libre would be more accurate. Don't know why, but it changes from one person to another.

      Lydia Maddux Kind of. He said that G6 (for him) is way more accurate than Libre. I guess its true for many other... but not everyone. I am a Libre user for budjet reasons. I am T1D so I test my blood anyway quite a lot...

    Question.... I am a physician following a plant based diet - question for you is that concerning the LCHF diet - the longevity of the Okinawans is well documented - their diet was a far from the LCHF diet as you can get - they consumed large quantities of sweet potatoes, seaweed, tofu and small amounts of pork. What is your thought? Also has anyone been on a LCHF diet long enough to know the risk of cancer?

      Your comment is factually wrong. That notion that Okinawans eat a plant based diet is based on a 1949 report when the island was still devastated from WWII.
      The quote “blue zones “ are a myth. They and the original peoples before contact with the western diet were high in sea foods , Offal and low GI fruits and some vegetables. The key is that these diets are LOW IN FRUCTOSE , the five member sugar which is converted to fats !

      You’ll also note that low protein rice eaters are usually much smaller with petite body frames. But once you introduce sugar & refined carbs/processed foods, they look just like the SAD. Our bodies are incredible at utilizing food sources!

    Interesting, I've done keto for about 5+ months. Fair weight loss, but slower than most. Had been exercising pretty regularly before last blood work. Had been struggling to lose weight while exercising. Bloodwork showed high fasting glucose 105 plus insulin levels at 29.5 very high with ketones at 3+ Weight loss terrible at that time. I got injury and was sedentary for a couple weeks. Weight loss returned fairly significantly. Question would be, did the exercise raise the insulin and glucose and how would ketone levels be that high with high glucose and insulin? Great talk, this whole health conversation is very interesting.

    I find it a little amusing how Attia claims that he was disappointed in himself for failing to achieve adequate health through the diet and exercise protocol he was doing but was too proud to blame any system for having "control" over him (or words to that effect). He then goes on to describe how his entire thinking was tied up in the energy balance model. Well, Peter, that's part of what Scher was getting at. You were the product of a system that was (and still is) promoting energy balance and calories as the reason people get fat and sick. It's just a nonsense to believe it and any right-minded person should be a little miffed that such institutional dogma gets perpetuated despite astonishing lack of evidence for it.
    David Diamond admits being annoyed by the cholesterol dogma and mythology he used to believe before actually looking at the evidence and it all figuring out.

    Are there any actual "chronologically" older experts on aging/ longevity who have actually reversed their own personal biomarkers? If these "experts" had such great advice, they would apply it to themselves; No successes so far, only failures, getting biologically older each day, and have all the usual chronic illness we all suffer from. So far, lots of BS walking around in white lab coats with PHDs and MDs, but not actually anybody who reversed and maintained a reduced biological age.

    so far only hearing from Carnivore cheerleaders...looked pretty hard for people from the graveyard of Carnivore but other than a few angry, disgruntled Vegans...silence...And most the Vegans that were saying much were exVegan Carnivores!

    Hmm, not enjoying this podcast, I am medium literate in practical application of LCHF, practically I have resolved T2DM, NAFLD, obesity, IBS. But I've never heard of this chap and just like Taubes podcast it is like overhearing two experts chatting in the canteen in such a way as to subtely discourage eavesdroppers. Maybe the host needs to be a non-expert and can make sure foundational context, eg what the heck is MTOR, and why should I care about it, type questions. Maybe get a third party to ask the dumb questions for the rest of us intermediates.

      Exactly! When listening to a subject containing things I am not a specialist in such as biology/medicine/science then you're constantly pausing and googling or taking notes for later. That's a much better and effective way of learning.

      That is kind of ridiculous, Big Ken. Nobody expects you to understand Calculus if you haven't even learned Algebra yet. There are already a lot of sources that have simplified the science.

    I like the way Pete's mind works. But not being a technician I have no idea if he actually has anything useful to say. I'm certainly not going to look up every other word he says.
    Having a large vocabulary makes it easy for him to use a lot of words to say I don't know. I'm afraid the carnivore diet probably would put him out of business. That is probably why he uses big words when he doesn't need to and doesn't define the big words he does need to use.
    I did enjoy the beginning of the interview. But in the end I'll have to give him a thumbs down. There just wasn't enough useful content to justify sitting through his entire podcast.
    I think the available science has gone beyond Keto and more towards carnivore.
    Just from this electricians observation keto seems to be a little too hard to maintain for most people even though it is much better than a low fat diet. Carnivore maybe the rule rather than the exception.
