With this video [Part 1], we will discuss the action and reaction of the immune system to Mycobacterium leprae infection, and understand the specific immune system mediating cytokines related to the tuberculoid pole and the lepromatous pole of leprosy.     Thank you sir, sir If IDO increasing granuloma formation, then why it is found more in lepromatous pole

      That's a good question. IDO is required to inhibit granuloma formation by interfering with the tryptophan metabolism. The main function is to regulate granuloma formation. In the lepromatous pole, the expression of IDO increases on the immune cells. Hence they are inhibited to form good granulomas. This leads to the lepromatous pole with loose infiltrate rather than granulomatous infiltrate.

    is IL1b anti-inflammatory as you said?

      Thank you for your doubt. Truly speaking, IL1b is a lymphocyte mitogen and recruiter, and is mainly responsible of B cell activation and fever production. Increased levels of IL1b has been found in multibacillary Hansen cases, where they recruit Bcells and inflammatory cells for lepra reactions. However, this pushes the immunity to a more humoral based immune reaction rather than a cell based immunity, thereby decreasing the cell mediated damage to the bacilli. So acute inflammatory response is on the same lines as TNFa, that is recruitment of inflammatory cells which are readily available for infection. I hope this makes a bit of sense sir.
