    ACL rehab video transcript:
    My name is Adam Brown, I am an experienced Musculoskeletal Physiotherapist and the CEO of Therapia. I’d to discuss your rehabilitation following ACL reconstruction surgery.
    So you have torn your ACL. At Therapia we have put this video together to help you navigate your recovery to help you to get the best possible outcome following your injury.
    The first question patients usually have is “Do I need surgery?”. And the answer is “maybe”.
    Not everyone who tears their ACL needs to have it repaired. Following a well-constructed rehab plan, many patients can return to all of their previous activities without having to undergo surgical repair.
    The question we need to ask is “Is the knee “functionally stable” enough to do all of the things you normally do without an ACL?”
    Functionally unstable knees will “give way” or “collapse” when you plant your foot and try to pivot or change direction. These instances of giving way may be present at first and then go away after your swelling goes down and your physiotherapist teaches you how to get your muscles properly supporting your knee. If your knee continues to “give way” even after the swelling is down and your muscles are working well - this is a good indication that you will need surgery.
    Each time a knee “gives way” it is a subluxation of the tibia on the femur and it can potentially damage your other cartilage as well as the menisci inside your knee.
    If you participate in sports, like soccer, that require “cutting” or rapid changes in direction, there is a higher likelihood you will need to have your ACL reconstructed in order to return to those sports.
    If you participate mainly in sports and activities that do not require “cutting” and after your initial rehabilitation phase your knee stops giving way, there is a good chance you will not require surgery.

    An ACL rupture is a significant injury and its reconstruction is quite invasive. You will need to commit yourself to a lengthy physical therapy process in order to come out the other side with a great knee. It takes about 1 year before patients begin to feel like the knee is “normal’ again and it takes 6 months of intense rehabilitation in order to return to sport.
    Your Physiotherapist will guide you through this process using established protocols and considering your own specific needs as well as your reponse to treatment.
    Weeks 1 - 2
    Manage pain and swelling
    Gentle ROM exercises
    Stop crutches
    Return to work
    Over the first two weeks you will be managing pain and swelling, as well as beginning some gentle exercises to restore joint motion and muscle function.
    Most patients are able to stop using crutches and return to work or school within the first two weeks. This of course depends upon your specific job.
    Weeks 2 - 6
    More aggressive physio for joint mobility
    Work on full knee extension
    Strengthen quads, hams, calves, hips, core
    Weeks 6 - 12
    Muscular strength
    Muscular endurance
    Proprioception and balance
    Joint range of motion (if needed)
    Functional Exercises
    Over this time the focus will be on;
    • Muscular strength
    • Muscular endurance
    • Proprioception and balance
    • Joint Range of Motion (if not already full)
    • Functional exercises
    Your Physiotherapist will progress your program based on how your knee is responded to the increases in stress. It is critical at this time to progress exercises at the appropriate pace to avoid having the knee swell up creating a temporary setback.
    Weeks 12 - 24
    Functional exercises
    Single leg
    Proprioception and balance
    Plyometrics
    Sport specific
    • Functional exercises (squats etc)
    • Single leg exercises
    • Proprioception and balance
    • Plyometric or power exercises
    • Sport specific exercises
    Return to sport stages
    Conditioning
    Conditioning + Drills
    Drills + Scrimmage
    Return to Competitio
    A good way to think about the stages of return to sport is;
    • Conditioning only
    • Conditioning and drills
    • Drills and scrimmage
    • Return to competition
    5-10% tear ACL
    Strengthen core, hip, leg
    Consult your physiotherapist
    Between 5 and 10% of people who undergo an ACL reconstruction will tear their new ACL at some point. Ongoing strength training of the core, hip and leg muscles can help reduce your likelihood of re-injury and the future development of knee arthritis. It is important to have your Physiotherapist teach you these exercises before your rehab is complete.     I had done my acl and pcl surgery 3 weeks ago.Guys if tear your ligament it is time cosumd process.You have to take rest for 2 mong and you can play after 9 to 12 month.so play safely.abd those who have ligament tear dont worry brother u can lead normall life even you got 95% strenth after 1 year.so dont worry its a common injury.Be strong and u have to mentaly strong.you can and you will do it.

      Now i can walk normally and i fell much better and my knee is stable now but i walk slowly.Next week i will go to the hospital for meet my doctor and my physiotherapist.i wish soon i can start my 2nd phase physiotherapy.

    Great video, tore my ACL, got surgery now I'm 3 months post surgery. I want to get back to the sports I played because I love competition. I've been playing soccer since I was 5 and I can't keep living knowing I can't ever play sports I love. This might be the hardest 9-12 months of my life :/

    I was diagnosed with a torn lateral meniscus in my right knee about 40 years ago and about every 10 years or so Dr's have told and continue to tell me I need surgery now and should have had it years ago. I've had knee pain for decades but something always kept me from the surgery.

    HI Whilst i was running straight line i felt i crack in my knee not big pain but later on started swelling but i could carry on for the rest of the day with not much pain is anything to worry about

    1 week after acl/meniscus surgery, I still have some swelling and pain on my knee. looking for knee bending exercises.

      @@oliviahopkins5371 take pain killers before you do physical therapy. That’s what I do. That helped me a lot.

    moderate joint effusion
    chronic acl tear with no intact ligament in situ
    large bucket handletear of the medial meniscal body with thickening and complex tear of the anterior and posterior horns
    no fracture dislocation
    knee space is normal.
    the patella ,medial and lateral patellar retinacula are normal
    quadriceps and patellar tendoms found normal
    lateral meniscus appears normal
    pcl appears normal

    I got infection after 2 months post acl reconstruction surgery my doctor removed screws but graft retained in acl debridement / cleaning Surgery is thier any chance to get back again and run without further surgery

    Thanks ....this situtation is similar to me.....after 4 month rest i can walk and run freely but when i do studies on sitting on my bed with my legs folded sometimes by leg feels that bone has something slipped and when i stretch my leg i hear a sound of bones

      @@278shivanandbaganavar7 did you not really do any exercises after the surgery? you asking me home workouts? you should have known some/ most of the workouts to get your knee working again since you are the one done surgery? lol I am wondering if this is the reason why you still cannot bend fully?

      There are different reasons why you may still be unstable with higher level physical activities at this time. I would highly advise you are assessed by an experienced physiotherapist

      Hi Meera,
      You can do squats and deadlifts. No jumping or jumping lunges. Lunges also ok just not jumping.

      @@sagarisbusy It's been two weeks now, walking around with Crutches and still on pain medication. My knee joint is still swollen and stiff, making it difficult to bend over my knee. My first Physiotherapy appointment since after knee surgery is next week, they gave me a late appointment because of the Christmas holiday.
      My worry now is the stiff and swollen knee joint and me not able to bend over my knee.

    I recently tore my ACL, MCL and my medial meniscus. Tomorrow I go to meet with surgeon to discuss the surgery. Biggest injury before this was an ankle sprain.. I'm scared as fuck, but so ready to get this post surgery first two days over with! Also there is a thing that they are taking out of your knee the day after surgery. Anybody know what that is? And does it hurt as bad?

      @@anannyasingh6311 I’m good. Back to sports like nothing ever happened. First few days after surgery are tough. Lots of pain and it’s gonna be a problem to use bathroom. Make sure you have somebody around who can bring you food and other stuff that you need without having to get out of bed as much

      @@anannyasingh6311 it’s a loooooong and mentally hard process. You really just gotta take it one day at a time. I walked without crutches really fast after surgery but it’s not what’s suggested 😬 it depends on each person. You need to get ur range of motion first. Sports ain’t coming soon. Takes about. 6 months until you can start jogging a bit and it’s all strengthening the muscle after that.
