In this video, Dr. Thomas Clanton discusses lateral ankle instability. He addresses the allograft procedure versus the Broström procedure with the internal brace for the treatment of chronic ankle instability.
Compared to other joints in the body, the ligaments in the ankle have good blood supply, which helps with healing. The Broström procedure was the initial standard of car. Research started to find that the initial reconstruction would begin to stretch causing re-injury.
Orthopedic surgeons pioneered the internal brace system which was used during the Broström procedure. This internal brace system served as a "seat-belt" for the original tissue. The Broström procedure with the internal brace system promotes a rapid recovery and provides more strength to the native ligaments in the ankle.
In patients where the tissue is not strong, Dr. Clanton uses a semitendinosus graft, which is the same graft that is used during ACL reconstruction. Research has found that the graft has provided exceptional stability for patients with a poor native ligament.
Outcomes comparing the Broström procedure versus the semitendinosus graft in each patient populations have shown almost identical positive results.
